Job Description:
Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.<ul><li>
Evaluate patients and perform appropriate therapy interventions</li><li>
Develop a comprehensive plan of care and coordinate multi-disciplinary care</li><li>
Perform prompt clinical documentation and visit notes</li><li>
Continued communication with physician, care team members, patient and family</li><li>
Comfortable working in all aspects of the home health setting</li><li>
Implement and maintain physical therapy protocols</li><li>
Manage patient treatment and outcomes directly, or through PTA supervision</li></ul>
Requirements:<ul><li>
Current Texas Physical Therapist (PT) license</li><li>
One year of physical therapy experience</li><li>
Experience with Homecare Homebase is helpful, but not required</li><li>
Previous Home Health experience is helpful, but not required</li><li>
Current Texas driver's license</li><li>
