<p>
The primary function of the Community Outreach Hygiene Promotor is to ensure the delivery of the WASH Hygiene promotion and community mobilization activities in the coordination and guidance of the Assistant, WASH - Hygiene Promotion or Officer WASH or WASH Manager or WASH Coordinator.</p>
<p>
Conducting hygiene campaigns, house-to-house hygiene promotion, elaborating clear and adapted messages/activities for beneficiaries, and overseeing the hygiene promotion and other WASH activities conducted by the community volunteers.</p>
<p>
The role provides data required in close cooperation with the team. Besides the Assistant, WASH - Hygiene Promotion ensures the capacity building of assigned local staff and stakeholders in WASH-related program implementation is delivered.</p>
<p>
To perform this job successfully, an individual must be able to perform each of the essential functions under limited or basic living conditions and a working environment.</p><p><br>
<b>MAIN TASKS AND RESPONSIBILITIES</b></p>

Assessment</b><br>
<ul><li>Support the WASH team in conducting rapid needs assessments at community and household levels.</li><li>Supports the Assistant, WASH - Hygiene Promotion in transitioning needs identified into activity plans and budgets.</li></ul></div>

Program Design and Implementation</b><br>
<ul><li>Deliver the implementation of the aims and objectives of the WASH response program in line with the WASH Coordinator or Manager's WASH guidance and WASH strategy.</li><li>Act as a mediator between IMC and the target communities.</li><li>In collaboration with other actors, deliver suitable hygiene promotion activities that suit the local context (communities, health facilities, camps, and Schools) and would be widely accepted by beneficiaries.</li><li>Conduct household visits to assess and promote household sanitation and hygiene behaviors.</li><li>Sensitize communities on hygienic behavior and disseminate key messages about Acute Watery Diarrhea (AWD)/Cholera through community radios.</li><li>Deliver hygiene promotion messages at strategic times, including during community events or religious gatherings and through once-monthly community broadcasts.</li><li>Educating food vendors and handlers on food hygiene is key to reducing infections. CHPs will organize training sessions, promote healthy environments, and reduce infectious diseases.</li><li>Improve hygiene and sanitation, eliminate open defecation, promote proper use of sanitation facilities, ensure access to clean water, and encourage hand washing.</li><li>Engage WASH committees to identify, analyze, and suggest practical solutions to the IDP and refugee communities&rsquo; WASH-related challenges.</li><li>Work with the WASH committee on the treatment of water facilities and point of use (POU) and post-distribution monitoring of items in their areas of operation.</li><li>Monitor the proper use, cleanliness, and maintenance of communal/shared sanitation facilities.</li><li>Carry out sanitary surveys around water sources to identify possible contaminants and mitigate water source pollution.</li><li>Submit weekly tally sheet reports to the respective supervisor in a timely and appropriate manner.</li><li>Distribute basic hygiene kits to vulnerable communities with the support of a WASH officer as their supervisor.</li><li>Identify locations/sites for WASH infrastructures (latrines construction/rehabilitation, borehole drilling/rehabilitation, etc.) in the beneficiary communities and coordinate closely with communities to identify informal settlements in host communities.</li><li>Participate in hygiene campaigns and drama shows in schools, community and IDP camps.</li><li>Collect health related data and keep records on activities of hygiene promotion.</li><li>Report challenges and successes during the activities and any other issues that require IMC follow-up/action.</li><li>If assigned, collect water samples from households and water sources and conduct quality tests using the Del Aqua Kit Record all results of the water test.</li><li>Timely reporting of Hygiene Promotion activities to the Assistant, WASH - Hygiene Promotion</li></ul></div>

The Farmworker Health Outreach worker will be responsible for completing outreach, basic health assessments, health education, referrals, and follow-up to migrant and seasonal farm workers. This position requires flexible hours (evenings and weekends) and a valid NC driver&rsquo;s license. This position is dependent upon grant funding.</p><p><b>

Role on the Care Team:</b></p><p>
The Farmworker Health workers place within the care team is to provide primary clinical support services to the patient. They contribute in clinical ways (such as taking blood pressure, blood glucose levels, encouraging medical office follow-up as indicated) that the physician may not be able to, considering the locations visited outside of the office.</p><p><b>
Duties and Responsibilities:</b></p><ul><li>
Works with team to plan and coordinate nightly visits to outreach camps, ensuring that all camps are visited in a timely manner and all necessary follow up visits are completed.</li><li>
Provides direct outreach to farmworker camps and families several evenings each week (6:00-9:30PM). Works with Farmworker and outreach teams (including interns and volunteers) to complete health assessments with farmworkers and their families at farmworker locations to ascertain services they may need.</li><li>
Works with team to disseminate information to farmworkers regarding clinic, local, and state services which respond to farmworker needs.</li><li>
Provides individual client health education, as well as group camp presentations on topics of need and interest to farmworkers. Refers farmworkers to HCCH services as appropriate. Is responsible for leading team education efforts on at least one health education topic per year with the goal of fulfilling grant requirements.</li><li>
Responsible for client case management services (i.e. assessment, referral, follow-up, and re-assessment) for farmworkers and their families in designated portions of the service area (Avery, Watauga, and Caldwell counties) in a timely manner.</li><li>
Transports and interprets for farmworker medical and social service appointments using program vehicle as available. May occasionally need to use personal vehicle and submit request for mileage reimbursement.</li><li>
Documents all farmworker encounters on appropriate forms.</li><li>
Works with team to enter all farmworker encounter data into database on a daily basis including tracking of clinical measures and all related data for UDS reporting.</li><li>
Will be staff or have program staff present with Newland evening clinic operations and interpreting.</li><li>
May supervise and recruit student interns.</li><li>
Works with team to help plan and implement projects (to be decided), including advocacy, community relationship building, and specific projects responsive to farmworker needs.</li><li>
Complies with all High Country Community Health employee requirements (including drug screening prior to hire date).</li><li>
Provides medical interpreter and document translation services to HCCH patients for staff when needed.</li><li>
Provides patient transportation to HCCH patients (and when required, family members)</li><li>
Coordinates farmworker (and their family members when necessary) transportations among outreach team members. When necessary, provides transportation to HCCH patients, farmworkers and their families.</li><li>

Preferred Education and Formal Training</i></b></p><p><b><i>
Associate&rsquo;s, Bachelor&rsquo;s or Master&rsquo;s degree with expertise in: Public Health, Nursing, Spanish, Health Advocacy, Social Advocacy and/or similar fields preferred. Knowledge of farmworker and/or migrant health issues also preferred. Demonstrated project management and supervisory skills. Prior experience conducting outreach is a plus.</i></b></p><p><b><i>
