Qualified Mental health Professional- OTP</b></div><div>
The Qualified Mental Health Professional will provide crisis intervention and clinical support to individuals involved in diversion and re-entry programs who are incarcerated or recently released. The primary goal is to stabilize mental health crises, assess client needs, and facilitate connections to appropriate community resources and treatment services. The position requires collaboration with correctional facilities, community partners, and healthcare providers to support successful reintegration and reduce recidivism. The Qualified Mental Health Professional (QP) will provide crisis response support, linkage to outside agencies to whom participants are referred, care coordination, and connection to Emergency Departments and the Behavioral Health Urgent Care Center. The Guilford County Sheriff&rsquo;s Office Re-entry Program and Jail Diversion Program serves individuals who interface with the criminal justice system due to serious mental illness and substance use. The Guilford County Sheriff's Office, in collaboration with Foundations Health High Point will provide Medication Assisted Treatment (MAT) services for individuals detained in the Guilford County Detention Center who have a Substance Use Disorder (SUD).</div><div></div><div><b><br>
Responsibilities:</b></div><ul><li>
Conduct timely, comprehensive mental health and substance abuse crisis assessments for individuals involved in diversion and re-entry programs.
</li><li>Develop individualized care plans focusing on crisis management, mental health stabilization, and linkage to treatment.
</li><li>Provide immediate crisis intervention and stabilization techniques to individuals experiencing acute mental health episodes.</li><li>
Coordinate and facilitate referrals to community-based mental health and substance use treatment providers, housing agencies, social services, and other support systems</li><li>
Monitor client progress and adjust service plans as necessary, ensuring continuity of care during and after release.</li><li>
Collaborate closely with correctional staff, probation officers, social workers, and healthcare professionals to support integrated care.</li><li>

Job Summary:</b></p>

The primary responsibility of the QMHP is to provide family oriented, individual and/or group rehabilitative mental health services, skills training and development, medication training, parenting skills training, and intensive case management services to clients of the agency per state specified standards and requirements. QMHP Case Managers provide interventions in the community, home, school, and office to clients assigned to their caseload.</p>
<p>
Essential Duties and Responsibilities:</p>

<ul><li>Provide curriculum based rehabilitative services to families, individuals and groups.</li>
<li>Maintain all case records (notes, treatment plans, and uniform assessments) in accordance with CFGC standards.</li>
<li>Meet the following monthly benchmarks:
<ul><li>Provide services at least two times per month to each assigned client.</li>
<li>Provide a minimum of 100 direct client contact hours each month.</li>
<li>Provide a minimum of 80% of client services in the community setting each month (80+ hours).</li>
</ul></li><li>Document with the understanding that
<ul><li>all clinical notations are considered legal documents and can be used in the court of law.</li>
<li>all documentation should be true, accurate, and never falsified in any manner.</li>
</ul></li><li>Present all assigned cases for review by supervisor.</li>

</b></div><div>The Family Support Specialist (FSS) Assesses, plans, links, advocates, coordinates, and monitors to assist children or young adults still in school in gaining access to needed health and dental services, financial assistance and benefits, housing, education, social services, and other natural supports using a family driven, youth guided planning process.</div>
<div></div><div><b>ESSENTIAL JOB FUNCTIONS:
</b></div><ul><li>Assures that the person-centered planning process takes place and that it results in the individual plan of service, and meets direct and indirect service activity expectations</li>
<li>Assures that the plan of service identifies what services and supports will be provided, based upon medical necessity, who will provide them, and how the FSS will monitor (i.e., interval of face-to-face contacts) the services and supports identified under each goal and objective.</li>
<li>Oversees the implementation of the individual plan of service, including supporting the child and family&rsquo;s dreams, goals, and desires for optimizing independence, promoting recovery; and assisting in the development and maintenance of natural supports.</li>
<li>Assures the participation of the child and family on an ongoing basis in discussions of her plans, goals, and status.</li>
<li>Identifies and addresses gaps in service provision.</li>
<li>Coordinates the child&rsquo;s services and supports with all providers, making referrals, and advocating for the child and family.</li>
<li>Assisting the child and family to access programs that provide financial, medical, and other assistance such as Home Help and Transportation services and Medicaid Enrollment and Spend downs.</li>
<li>Assures coordination with the child&rsquo;s primary and other health care providers to assure continuity of care.</li>
<li>Coordinates and assists the child in crisis intervention and discharge planning, and community supports after hospitalization.</li>
<li>Facilitates the transition process (e.g., from inpatient to community services, school to work, dependent to independent living), including arrangements for follow up services.</li>
<li>Assists the child and family with crisis planning. Participates as member of Children&rsquo;s Crisis Mobile Team.</li>
<li>Identifies the process for afterhours contact.</li>
<li>Monitors and ensures that applicable laws, rules, policies, and procedures are followed.</li>
<li>Reports cases of alleged abuse or neglect to agency Recipient rights Officer and/or with Michigan Department of Health and Human Services Protective Services Workers or Licensing Consultant as prescribed by law.</li>
<li>Provides accurate and timely documentation for the clinical records and data systems, in accordance with Medicaid and agency requirements. Refer to attached documentation grid for NCCMH standards.</li>
<li>May assist in transporting and accompanying individuals and parents to medical appointments, evaluations, etc.</li>
<li>Participates in meetings and other related duties as directed by the supervisor.</li>
<li>Complies with agency requirements for continuing education, peer review, credentialing/privileging, and QIDP and/or QMHP requirements.</li>
<li>May assist, arrange, or provide in-service training programs for consumers, direct support workers, foster care providers, parent, guardians, and interested community members or civic organizations.</li>
<li>Performs other duties as assigned by supervisor.</li>
</ul><div><b>PHYSICAL REQUIREMENTS:</b></div>
<div>Ability to walk, bend, stand, sit, lift to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in de-escalation techniques, which will be a training class provided to the job incumbent. <ul><li>Must have own phone and internet for communication, reliable access to messages, and multi-factoring capabilities.</li>

<li>Have reliable transportation and be able to drive a company vehicle. Must also possess a valid Michigan driver&rsquo;s license and have a satisfactory driving record.</li>

<li>Ability to negotiate conflict and maintain constructive working relationships with people at all levels of the organization and with community stakeholders, families, and clients.</li>
<li>Has knowledge of standards, policies and procedures regarding rights, confidentiality, Mental health code, HIPPA, code of ethics etc.</li>
<li>Demonstrates basic computer skills required for clinical documentation, communication and data entry.
</li><li>Ability to visit clients in their residence, educational or employment setting.</li>
<li>Irregular hours will be necessary periodically.</li>

<p>Qualified Mental Health Professionals provide the necessary mental health treatment and social support through individualized counseling and life skills training to adults 18 or older who, due to mental illness, would otherwise have trouble maintaining community stability and independence. Some major areas of focus include daily living skills training, accessing community resources, health and safety, financial management and social supports. Mental Health Support services are individualized, client centered and facilitated in the client home and their community.</p><p><b>EDUCATION and/or EXPERIENCE</b></p><p>To qualify a candidate MUST have at least a bachelor&rsquo;s degree in human service field or special education from an accredited college and with at least one year of experience and a VALID QMHP-A Certification.</p><p><b>Training</b></p><p>Staff must be trained certified in: CPR, First Aid, TOVA and become orientated to the company&rsquo;s guidelines, policy and procedures, and take part in staff trainings as they occur.</p><p><b>Additional Requirements</b></p><p>Employee must be certified as tuberculosis free</p><p>Employee must clear criminal background and CPS registry checks</p><p><b>Transportation</b></p><p>Case Managers are required to use their personal vehicles for visits and for engaging in community activities. Therefore, at the time of hire, Case Managers must provide proof of Driver&rsquo;s license and proof of Automobile Insurance.</p><p><b>ESSENTIAL DUTIES AND RESPONSIBILITIES </b>include but not limited to the following:</p><p>Assist in the development and ongoing monitoring and implementation of Individual Service Plan.</p><p>Work with clients to foster the development of new or improved coping, problem solving, and communication skills.</p><p>Case management functions of linking, arranging for services and referrals.</p><p>Maintain continuous contact with the identified consumer and other key stakeholders (with signed authorization to release information) in planning and service delivery.
