Case Manager-RedirectJob descriptionThe Redirect addiction-related diversion program is a street-based, long-term, and unconventional approach to case management. Redirect case work is non-coercive, non-time delimited, street-based, and built on multiple pathways to recovery, including harm reduction, and steeped in Stages of Change theory and motivational interviewing practices.This position will manage a caseload of approximately 15-25 participants and will work in conjunction with local police departments and FAN&rsquo;s Comeback Quick Response Team (QRT). The House Manager promotes a structured, recovery-oriented, and supportive living environment while ensuring compliance with house rules, program expectations, and recovery residence standards.This position serves as a role model for residents and provides oversight, support, transportation, and coordination of services to assist residents in maintaining stability, sobriety, and progress toward their recovery goals.Essential Duties and ResponsibilitiesResident Supervision and Support<ul><li>Monitor and maintain a safe, clean, and supportive living environment.</li><li>Enforce house rules, policies, and resident agreements consistently and professionally.</li><li>Conduct resident orientations and review program expectations.</li><li>Provide support, encouragement, and accountability to residents in recovery.</li><li>Assist residents with conflict resolution and problem-solving.</li><li>Conduct regular house meetings and document attendance.</li><li>Monitor resident compliance with curfews, chores, treatment participation, and recovery activities.</li><li>Assist with resident intake, discharge, and transition planning.</li></ul>Transportation Responsibilities<ul><li>Transport residents to and from:</li><li>Medical and behavioral health appointments</li><li>Substance use treatment services</li><li>Employment opportunities</li><li>Educational or vocational programs</li><li>Court appointments</li><li>Probation and Parole meetings</li><li>Recovery support meetings (AA, NA, SMART Recovery, etc.)</li><li>Community resources and approved activities</li><li>Maintain transportation logs and documentation.</li><li>Ensure vehicle inspections, cleanliness, and routine maintenance requirements are completed.</li><li>Immediately report accidents, incidents, or vehicle concerns.</li></ul>Program Operations<ul><li>Conduct routine room inspections and property walkthroughs.</li><li>Assist with random drug and alcohol screenings as required by program policies.</li><li>Document resident progress, incidents, rule violations, and resident concerns.</li><li>Complete incident reports and communicate significant events to leadership promptly.</li><li>Maintain confidentiality in accordance with HIPAA and agency policies.</li><li>Coordinate with treatment providers, case managers, probation officers, and community partners as authorized.</li><li>Participate in staff meetings, trainings, and supervision.</li></ul>Facility Management<ul><li>Monitor household supplies and submit purchase requests (If applicable).</li><li>Ensure cleanliness and sanitation standards are maintained.</li><li>Coordinate maintenance requests and report safety hazards.</li><li>Conduct emergency preparedness and safety checks.</li></ul>Qualifications Required<ul><li>High School Diploma or GED.</li><li>Minimum of two (2) years of experience working in behavioral health, substance use treatment, supportive housing, recovery housing, case management, social services, or a related field.</li><li>Valid Maryland Driver's License.</li><li>Clean driving record and ability to be insured under agency vehicle insurance.</li><li>Ability to pass background screening and drug testing requirements.</li><li>Strong communication, organizational, and interpersonal skills.</li><li>Basic computer skills and ability to complete electronic documentation.</li></ul>Preferred<ul><li>Associate's or Bachelor's Degree in Human Services, Social Work, Psychology, Criminal Justice, or related field.</li><li>Experience working in recovery housing or residential programs.</li><li>CPR/First Aid Certification.</li><li>Peer Recovery Specialist, CPRP, CFRP, or related certification.</li></ul>Physical Requirements<ul><li>Ability to lift up to 25 pounds.</li><li>Ability to travel locally throughout Maryland.</li><li>Ability to stand, walk, climb stairs, and perform routine inspections of the residence.</li><li>Ability to transport residents safely using agency or approved vehicles.</li></ul>Benefits<ul><li>Competitive Salary: $45,000 annually</li><li>Paid Time Off (PTO)</li><li>Paid Holidays</li><li>Professional Development Opportunities</li><li>Supportive Team Environment</li></ul>Pay: From $45,000.00 per yearBenefits:<ul><li>401(k)</li><li>Dental insurance</li><li>Health insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Work Location: In person

We are seeking a community-based Case Manager to join our team in Colmar, PA serving clients in Montgomery County.
The Case Manager is responsible for developing, coordinating, and implementing services for adult consumers as identified in the goal-oriented service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life.

The Recovery Coach will:
<ul><li>Link consumers with a mental health or behavioral health diagnosis to resources and encourage, teach, and guide them to advocate for themselves.</li>
<li>Case Manager will complete comprehensive assessments, service plans, and environmental matrixes to determine level of care and frequency of contact with consumers on their caseload.</li>
<li>Complete other paperwork requirements from OMHSAS, Magellan Behavioral Health, Montgomery County, and Merakey and will be monitored by Case Manager and completed timely.</li>
<li>Have appointments with consumers in their homes, community, and hospitals and should be familiar with Montgomery County, including being able to access resources to meet client's needs.</li>
<li>Carry a caseload between 25-30 clients and Case Manager will meet with clients according to matrix score, either every 14 or 30 days while maintaining Merakey's productivity standard.</li>
<li>Attend supervision meeting with Manager monthly and team meetings weekly.</li>
<li>Work typically Monday-Friday: 9:00am - 5:00pm but shift may vary at times based on client's needs. This role includes optional on-call.</li>

This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
Earn $22.29 per hour.

An additional $2.00 per hour is available with the selection of our Enhanced Pay option!

Requirements:
The ideal candidate will possess the following qualifications:
<ul>
<li>Valid driver's license with clean motor vehicle report.
</li><li>Basic computer skills and experience with an EMR system preferred.</li>
<li>Case Management experience in mental health or behavioral health is preferred.
</li></ul>Educational Requirements:
<ul><li>Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or</li>
<li>Registered nurse; or</li>
<li>A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years of experience in direct contact with mental health consumers; or</li>
<li>A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.</li></ul></div>
