JOB SUMMARY: Community Support Services (CSS) are medium-intensity community-based services that promote mental health recovery for people with persistent mental health issues. Services are customized to the needs and preferences of the individual. CSS helps restore individuals to their best possible level of functioning, while building the independence, resiliency and coping skills needed to live in their communities. CSS staff have flexible hours in order to be available to individuals who might need help outside normal hours of operation and are available to respond rapidly to early signs of relapse or crisis. 


Duties and Responsibilities include:

Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the consumer&rsquo;s individual recovery plan.</li><li>
Partner with the individual and CORE services provider in the development of the individual service plan.</li><li>
Assist the individual with the transitioning between levels of care. Link and/or serve as advocate with other agencies to garner needed resources for consumers.</li><li>
Provides assistance and support in crisis situations.</li><li>
Provides assessment, monitoring, and assistance to individuals.</li><li>
Establishes professional and ethical relationships with individuals, families, and coworkers</li><li>
Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community.</li><li>
Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Policy &amp; Procedure.</li><li>

<li>High school diploma or G.E.D and one (1) years of experience to include lived experience, previous work experience in other medical professions working with co-morbid mental health conditions, such as CNA&rsquo;s Emergency Medical Technicians and other healthcare personnel.</li>
<li>Knowledge of basic nursing skills and interventions, as well as mental and physical health conditions and terminologies</li>
<li>Certification as a CNA or its equivalent is acceptable documentation of nursing skills knowledge</li>
<li>Proficient understanding of client advocacy and client rights as defined in Title 17 and Title 9</li>
<li>Must be at least 18 years of age</li>
<li>Must be CPR, First Aid, and CPI certified (hands-on course) on date of employment or prior to providing direct client care and maintain current certification throughout employment</li>
<li>All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver&rsquo;s license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment.
