<p><b>DIVISION OVERVIEW: </b>The Housing Division provides safe, affordable housing paired with wraparound supportive services to individuals and families who have experienced chronic homelessness, mental illness, substance use disorders, or other significant barriers to stability. Guided by Housing First principles, the division works to promote long-term housing stability, improve health and well-being, and foster independence by integrating case management, clinical supports, and community engagement. Through partnerships with public agencies, funders, and community organizations, the division advances the nonprofit&rsquo;s mission to reduce homelessness and create equitable pathways to housing security and self-sufficiency.</p>

Starting at $44,000 - $47,000; contingent upon experience, education, etc.</p>
<p><b>POSITION SUMMARY: </b>The B.R.I.D.G.E To Home (B2H) Program<b> </b>serves individuals returning from Illinois prisons who face significant and often compounded barriers to successful reentry. Frequently excluded from traditional housing and support systems, these individuals are at increased risk of homelessness and recidivism. The program&rsquo;s primary goal is to provide permanent supportive housing for individuals traditionally considered hard to place, creating a foundation for long-term stability and successful reintegration into the community. Guided by low-barrier, housing placement principles, the B2H Program eliminates preconditions such as abstinence or mandatory treatment participation, offering immediate access to housing.</p>
<p><b>ESSENTIAL DUTIES AND RESPONSIBILITIES</b></p>
<ul><li>Collaborate with the Network Housing Developer to identify safe, affordable housing options appropriate for reentry clients, including those with complex barriers or limited income history.</li>
<li>Provide specialized case management services, including interviews and intake assessments, housing and rental eligibility screenings, and the development of individualized housing stabilization plans that reflect clients' reentry needs, goals, and risks.</li>
<li>Provides referrals and crisis intervention for the individuals&rsquo; identified needs</li>
<li>Review lease agreements with clients, ensuring understanding of terms, responsibilities, and compliance with any reentry program-specific requirements.</li>
<li>Educate clients on tenant rights and responsibilities, and facilitate orientation on lease terms, housing policies, and reentry support resources.</li>
<li>Conduct home visits, inspections and community-based outreach to monitor client well-being, provide tenancy coaching, and assist in resolving landlord or neighborhood conflicts.</li>
<li>Coordinate referrals to behavioral health, employment, education, legal aid, and wraparound services that address social determinants of health and promote housing retention.</li>
<li>Assist clients in building financial literacy skills, including budgeting, credit repair strategies, and obtaining credit reports to support long-term stability.</li>
<li>Support clients through the move-in process, including utility setup or reconnections, coordination of housing start-up kits, and basic household needs.</li>
<li>Enter accurate and timely case documentation into the ECR database; collect required documentation from clients and service partners for eligibility, assessment, and reassessment purposes.</li>
<li>Assist with monthly data collection, outcome tracking, and program reporting required by funders and oversight agencies.</li>
<li>Participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services.</li>
<li>Attend all required trainings related to reentry, trauma-informed care, housing stability, and agency policies.</li>

JOB SUMMARY</b></p>
<p>
Performs advanced case management work. Work involves coordinating case management<br>
activities developing program goals, objectives, and procedures; and evaluating outcome measures<br>

ESSENTIAL FUNCTIONS</b></p>

A. Performs complex pre-release case management for the reentry and integration of inmates<br>
discharging from a correctional facility; assists in the development of program policies and<br>

activities to produce an effective reentry transitional process.</p>

B. Reviews inmates referred to the program and tracks progress through the case management<br>
process; conducts inmate assessments to identify workforce and reentry barriers; implements<br>
criteria for identification of inmate needs; and develops and implements case plans to address<br>
removal of inmate deficiencies.</p>

C. Coordinates with appropriate agencies in obtaining necessary inmate identification documents;<br>

with and provides liaison with program staff, government agencies, inmate families, faith-based<br>
support groups, and other organizations.</p>

D. Develops and maintains records on participating inmates; documents case records and reviews<br>

MINIMUM QUALIFICATIONS</b></p>

<li>Complete and maintain all required individual and program-related documentation, which includes documenting the results of screenings, enrollments, service plans, reports, progress notes, discharge summaries, and other individual-related activities including accurate reporting of services.</li>

JOB SUMMARY
<br>HealthRIGHT 360, a family of programs, is located across 13 different counties in California. HealthRIGHT
<br>360 provides an array of medical and behavioral health services to some of the most vulnerable
<br>populations, ranging from primary care, mental health services, detox, substance use disorder treatment
<br>in both residential and outpatient settings, and transitional services for the criminal justice population.
<br><br>
The Specialized Treatment for Optimized Programming (STOP) Area Six Program connects California
<br>Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based
<br>programming and services during their transition into the community, with priority given to those
<br>participants who are within their first year of release and who have been assessed to as a moderate to
<br>high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with
<br>detoxification, licensed residential treatment programs, outpatient programs, professional services, and
<br>reentry and recovery housing throughout the program area to assist participants with reentry and
<br>recovery resources.
<br><br>
The Case Manager works with a multidisciplinary team to provide casework services to clients, handle
<br>intake and initial placement in community-based services, transportation, case plan, case management,
<br>and interface with Division of Adult Parole Operations (DAPO) to assist clients in navigating the needed
<br>services to reach health and recovery. Case Manager I ensures that the quality of services delivered by
<br>the community-based providers (CBP) meet the standards set forth by the California Department of
<br>Corrections and Rehabilitation (CDCR) and HealthRIGHT 360.
<br><br>
KEY RESPONSIBILITIES
<br><br>
<b>Case Management Responsibilities:</b><br>
<ul>
<li>Maintains an assigned caseload of up to fifty (50) CDCR clients, or more depending on needs.</li>
<li>Provides learning experience opportunities and offers clinical support to assist clients in meeting their treatment goals.</li>
<li>Develops Case Management Plans (CMP) for clients.</li>
<li>Develops and monitors clients’ progress relative to their CMP through all of phases of the program and links them</li>
</ul>
to both internal and external resources based on their treatment needs.
<br><ul>
<li>Maintains progress notes in clients’ files.</li>
<li>Keeps AOR apprised of clients’ progress and develops exit and aftercare plans.</li>
<li>Travels to CBPs, sometimes requiring overnight stays, to provide outreach, case management, and assessment</li>
</ul>
services.
<br><ul>
<li>Corresponds with AOR and assigns authorization for readmissions and client transfers.</li>
<li>Assists with training of new hires.</li>
</ul>
<b>Administrative Responsibilities:</b><br>
<ul>
<li>Ensures ongoing communication with the CDCR personnel regarding</li>
</ul>
clients.
<br><ul>
<li>Maintains accurate and comprehensive files on each caseload client and community-based</li>
</ul>
providers.
<br><ul>
<li>Reconciles Activity Reports with community providers.</li>
<li>Completes reports as assigned.</li>
<li>Attends required trainings and meetings.</li>
</ul>
<b>Documentation Responsibilities:</b><br>
<ul>
<li>Coordinates with available internal and external resources (Parole, community-based providers, STOP staff, etc.) to develop/maintain action plans, transition plans, progress</li>
</ul>
notes and appropriate updates in support of the health and recovery needs of each caseload client.
<br><ul>
<li>Maintains accurate records by entering data into various electronic systems for all caseload clients in</li>
</ul>
accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
<br><ul>
<li>Helps ensure the accuracy, confidentiality and timeliness of all incident reports submitted.</li>
<li>Contact CBPs to request timely and sufficient reports.</li>
<li>Reports urgent incidents to supervisor.</li>
<li>Contacts AOR for additional information as needed.</li>
</ul>
<b>Placement Responsibilities:</b><br>
<ul>
<li>Assesses, schedules, and coordinates placements of referrals into appropriate community placements.</li>
<li>Coordinates transportation with Transportation Coordinator.</li>
<li>Screens paperwork for possible obstacles to treatment provision.</li>
<li>Conducts daily communication with Parole Agents.</li>
<li>Conducts presentations to community agencies to increase the pool of referrals.</li>
<li>Identifies community capacity needs.</li>
<li>Revises and submits placement requests.</li>
<li>And perform other duties as assigned.</li>
</ul>
QUALIFICATIONS
<br>Education, Certification, or Licensure
<br><ul>
<li>High school diploma or equivalent.</li>
<li>Possess a valid California driver’s license and automobile insurance.</li>
<li>Possess a valid CPR and First Aid certification or ability to obtain within 30 days of hire.</li>
</ul>
Experience
<br><ul>
<li>Minimum 2 years’ case management experience with the criminal justice population.</li>
<li>We will consider for employment qualified applicants with arrest and conviction records.</li>
<li>Must complete a background check and livescan.</li>
