The LEAP Project is designed to provide transitioning reentrants from the Northampton County Prison and Lehigh County Jail with critical job readiness and employability skills they need to get and keep a job once released by creating and operating an American Job Center, called the LEAP-Center, in the Jail. The LEAP Reentry Employment Specialist serves as the liaison between Northampton County Prison, Lehigh County Jail, parole offices, PA CareerLink&reg; Lehigh Valley and the LVWIB staff. The Reentry Employment Specialist supports program participants in the successful transition of LEAP pre-employment services received in the Jail to services received at the PA CareerLink&reg; Lehigh Valley site upon release from jail.</p><p></p><p><b>

The Peer Support Specialist provides vital support and advocacy for individuals involved in the criminal justice system, assisting with their transition from incarceration to community living. CASE MANAGER

Community Health Worker- OTP Clinic</b></div><div>
The Community Health Worker (CHW) must be a frontline public health worker who is a trusted member of the community served. The CHW supports the Guilford County Sheriff&rsquo;s Office Re-entry Program and Jail Diversion Program, which serves individuals who interface with the criminal justice system due to serious mental illness and substance use. The CHW focuses on supporting formerly incarcerated individuals and those at risk of incarceration to successfully reintegrate into the community, reduce recidivism and improve access to healthcare, social services and supportive networks.</div><div>
The Guilford County Sheriff's Office, in collaboration with Foundations Health High Point, will provide Medication Assisted Treatment (MAT) services for individuals detained in the Guilford County Detention Center who have a Substance Use Disorder (SUD).</div><div></div><div><b><br>
Responsibilities:</b></div><ul><li>
Conduct outreach to individuals in jail, prison, or community settings to engage them in reentry and diversion services.</li><li>

This position also facilitates career readiness programs and services at Norton Correctional Facility.</p><p></p><h2 class="jobSectionHeader"><b><br>
Job Responsibilities may include but are not limited to the following:</b><b>
</b></h2><p>Provides services within the Career Resource Center located at Norton Correctional Facility, including but not limited to employment needs and documents (resumes, cover letters, IDs, job searches, available training/schooling opportunities), community support and resources for the releasee in their area of release, and any educational or grant opportunities (finding, applying and providing the services necessary to the resident releasing). Delivers career readiness programs and other services to incarcerated citizens at Norton Correctional Facility. This includes assessing for risk/need related to job/career readiness; developing individual employment plans in coordination with all relevant partners and the resident; takes steps to implement the plans; delivers all the services available in the resource room; makes connections to and develops employers; and facilitates/co-facilitates Career Readiness and other CBT groups after proper training.</p><p>
Works with returning citizens during and after transition to carry forward career readiness plans and support all necessary steps for the returned citizen to achieve sustained livable wage employment. This includes continuing to address barriers; find resources; communicate with employers; address related issues such as housing, identification/DLs, transportation and the like. Communicate closely with parole, risk reduction specialists, workforce center staff, supports, resources, and anyone else as necessary to support the goal of sustained livable wage employment.</p><p>
Carries out special projects as assigned by supervisor and the Executive Director of Programs and Risk Reduction to address system capacity statewide related to preparing returning citizens to achieve and supporting them in achieving sustained livable wage upon employment. This includes collaborating with adult and higher education partners on projects or pieces of the overall statewide plan; communicating with employers; communicating with returned citizens; tracking/collecting/collating data and information about paths, projects and outcomes; developing and providing information and training supporting the work of the Career Navigators statewide; and otherwise.</p><p>
Enter Athena (KDOC Resident Management System) and Program Tracking entries as necessary, maintain data and information related to processes and impact, and provide the information to the statewide reentry team, as requested by the Reentry Director or Supervisor.</p><p>
