This individual possesses expertise that qualifies him or her to perform all nutrition-related duties at Cornerstone Family Healthcare WIC program, including providing and overseeing high risk care participants.
Description of Duties:<ul><li>
Conduct a complete nutrition assessment, determine nutrition risk, and enroll participants.</li><li>
Conduct breastfeeding assessment and provide participant centered counseling that helps mothers initiate breastfeeding, establish milk supply, exclusively breastfeed, and continue breastfeeding.</li><li>
Address common breastfeeding concerns and issue breast pumps.</li><li>
Develop and implement individual care plans (ICPs) and oversee high risk care to all participants.</li><li>
Refer high-risk participants to other health-related and social services, as necessary.</li><li>
Prescribe, tailor and authorize food packages based on participants&rsquo; needs. Reissue food packages.</li><li>
Provide and document participant-centered nutrition education/counseling for participants.</li><li>
Ensure the efficient flow of the assessment and education processes.</li><li>
Track high-risk participants&rsquo; progress in improving their health and document for outcomes.</li><li>
Schedule participants secondary nutrition education appointments and high-risk contacts.</li><li>
Participate in in-service training programs for professionals and paraprofessionals.</li><li>
Perform other related duties as assigned by the Site Supervisor and Program Director.</li><li>
Attend and participate in monthly department, All Staff, and other required meetings.</li><li>
