HOW YOU&rsquo;LL MAKE AN IMPACT

The Clinical Supervisor &ndash; Rehab Services will have the role of providing clinical oversight, direction, and leadership to the Rehab Services programs of Adult Rehabilitative Mental Health Services (ARMHS), Peers, and Targeted Case Management (TCM). They will work in collaboration with the Program Manager and be officed in St. Cloud.
<ul><li>
Responsible for day-to-day operations of the Rehab Programs; attend to operational issues, including crisis management, consultation, staff and client complaints, and coordination of care.</li><li>
Maintain appropriate staffing levels, including hiring and training of Rehab staff</li><li>
Conduct performance evaluations for Rehab Staff in collaboration with the Program Manager.</li><li>
Familiarize self with Minnesota Statutes 245B.0623, 256B.0924, 256B.0616 and DHS requirements for each program.</li><li>
Provide monthly group and individual treatment supervision, as directed by program requirements.</li><li>
Review and sign all required documentation.</li><li>
Manage productivity standards and budget for Rehab programs to ensure financial viability of programs in collaboration with Program Manager.</li><li>
Be available for clinical client concerns of Rehab Services Staff.</li><li>
Provide recommendations and oversee all mandated reporting issues.</li><li>
Be familiar with implementation of health integration, including care collaboration agency wide.</li><li>
Complete Comprehensive Evaluations for Rehab clients to determine a recipient&rsquo;s eligibility for mental health services.</li><li>
Comply with all reporting mandates including the duty to warn, child protection and vulnerable adult, and ombudsman incidents.</li><li>
Contribute to peer reviews and chart audits, providing feedback to providers.</li><li>
Attend weekly and monthly management meetings.</li><li>
Other duties as assigned.</li></ul> 


SCHEDULE

Monday through Friday with some flexibility. Clinic hours are 8am- 5pm. May need to provide phone support to mental health providers who see clients until 6pm.

Summary: 
The Rehab Technician provides excellent customer service by assisting in the Rehab &amp; Performance Department at Orthopaedics East &amp; Sports Medicine Center (OrthoEast). Primary responsibilities include patient intake, assessing patient tolerance to treatments, instructing patients on exercises prescribed by physical and occupational therapists, fitting and applying orthopedic equipment, educating patients and staff on use of the DME, and maintaining patient records. 
This position requires compliance with OrthoEast's compliance standards, including its Code of Conduct, Compliance Program, and policies and procedures. Such compliance will be an element required as part of the job duties and considered during performance evaluations.

Position Description

The COPD Educator/Pulmonary Rehabilitation position is responsible for educating and assessing patients diagnosed with COPD in hospital. They will review the patient's history and care plan, educate the patient on the disease process, as well as connect them with resources both inside and outside of the hospital in order to help with the management of their disease and improve their quality of life. The COPD educator/Pulmonary Rehabilitation position is also responsible for reviewing referrals and identifying candidates who qualify for pulmonary rehabilitation. They will contact patients to explain the program and help to schedule them for their appointments as well as coordinating any needs they may have that act as barriers to participating. They will be able to support the pulmonary rehabilitation staff and be trained both in pulmonary rehabilitation as well as six-minute walk testing. 


Daily Tasks:

<li>Identify COPD inpatients from generated list and COPD consults and review care plan and history</li>
<li>Visit, assess and provide COPD education to inpatients on list</li>
<li>Identify and discuss recommendations of changes to treatment plan with hospitalist and PCP</li>
<li>Generate referrals to outpatient resources including pulmonology, pulmonary rehabilitation, community case management, palliative care</li>
<li>Documenting COPD consult in EMR</li>
<li>Review referrals for pulmonary rehabilitation and identify qualified candidates for pulmonary rehabilitation</li>
<li>Contact referrals and explain pulmonary rehabilitation program. Identify any barriers and connect patients to resources if necessary.</li>
