Job Summary and Qualifications</h5>
The Social Worker (MSW) is responsible for non-clinical, biopsychosocial assessments and interventions for the inpatient floors and the outpatient emergency department. The MSW performs crisis intervention, patient/family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other identified resource needs. The MSW assists in post-acute care placement on all discharges, primarily with those patients identified with post-discharge behavioral health and patient assistance/resource needs. The MSW is dedicated to patient and family-centered care that values personal self-determination, prioritization, and engages in creative and compassionate problem-solving. The scope of this role includes intensive care units, Trauma, Oncology, NICU, Transplant, and ED. The MSW responds to physician, non-discharge planning social work consults throughout the facility.
What you will do in this role:<ul><li>
You will work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.</li><li>
You will identify and address barriers to discharge and recovery. These barriers include financial, emotional, psychosocial, medical, and spiritual.</li><li>
You will maintain a thorough working knowledge of the rules of Medicare, Medicaid, and private payer regulations and processes. You will possess a thorough understanding of managed-care concepts and length-of-stay management relating to patient and organizational fiscal responsibility.</li><li>
You will perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.</li><li>
You will coordinate and facilitate meetings between the care team and patients/families/caregivers. You will assist patients and families in making difficult decisions while honoring their preferences and values.</li><li>
You will intervene with patients/families/caregivers for psychosocial needs, assistance, and support in coping with chronic acute terminal illness. You will engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.</li><li>
You will provide emotional support to patients and family members during complex and/or emotionally charged issues, such as end-of-life, terminal illness notifications, death notification (with physician), and crisis interventions.</li></ul>

Job Summary
This position is responsible for providing comprehensive assessments on patients (pediatric and adult patients) and being the primary liaison between the patient / family and the treatment team. The Case Manager ensures that the patient&rsquo;s individualized treatment plan is carried out in a focused, cost effective, goal directed manner. As a member of the interdisciplinary team the case manager will also facilitate the development, coordination, and implementation of the discharge plan as well as any other outside arrangements to ensure continuity of care.

</li><li>Basic Life Support (BLS) accredited by the American Heart Association (AHA)
</li><li>Bachelor&rsquo;s degree from accredited college in Nursing or Social Work. Will consider RN with A.D.N. if actively pursuing BSN, with BSN completion required within 2 years of hire.
</li><li>Current licensure in Kentucky as a LCSW or CSW or RN.
</li><li>Must have 2-3 years experience as an RN plus utilization management or discharge planning/case management experience. Minimum two years experience performing utilization review and/or quality assurance activities in an acute care facility or HMO/PPO/PRO preferred. Must be able to adjust priorities quickly, organize multiple tasks simultaneously, and work interdependently with many levels of staff. Attention to detail; strong organizational, interpersonal and communication skills; and innovative problem solving skills required. Must be able to adjust work hours depending on depending upon departmental needs as determined by the director or manager.

Responsibilities:
<div>The Director of Rehab PT is responsible for the efficient management of rehabilitation services in their assigned account(s). This position also assists in integration of rehabilitation services in all settings. The Director of Rehab PT ensures that the highest standard of rehabilitation services is delivered and maintained.</div><div> 
1. Assumes responsibility for the efficient and profitable management of rehabilitation services in assigned site(s) of service. 
2. Provides direct patient care. 
3. Monitors the standard of clinical services being delivered and maintained in all sites and ensures an ethical and compliant product is being delivered. 
4. Operationally manages Powerback Rehabilitation personnel and consultants within facility or home care contracts. 
5. Ensures accountability to the provision of evidence based care and adherence to care delivery standards. 
6. Reviews monthly department and individual therapist outcomes and supports staff in identifying strategies to improve outcomes as needed. 
7. Assists therapy staff in identifying and removing barriers to treatment and providing clinical education to meet the needs of the patient. 
8. Takes responsibility for the development and support of all staff in their site to include rounding, coaching and mentoring. 
9. Assumes responsibility for annual merit evaluation of therapy staff. 
10. Assumes responsibility for hiring therapy staff (in conjunction with the Clinical Operations Area Director). 
11. Assumes responsibility for daily staffing and utilization with input from treating therapists, consideration of clinical requirements, and recommendations from the Clinical Director. 
12. Administers financial controls of revenue and expenses. 
13. Assumes responsibility for facility reports on a weekly and monthly basis. 
14. Assists Clinical Operations Area Director in annual budget preparation. 
15. Assumes responsibility for meeting annual budget goals as set by the Clinical Operations Area Director. 
16. Assists with clinical orientation and ensures that there is a quality orientation process for new rehab staff.

18. Promotes all Powerback Rehabilitation products and services whenever possible. 
19. Assists Clinical Operations Area Director in identifying and securing new contracts. 
20. Develops and maintains relationship with facility management team and staff as a representative of Powerback Rehabilitation. 
21. Ensures that the clinical management of the rehab department, including oversight related to case management, quality improvement, care planning, clinical utilization, and patient identification is overseen.
 
22. Completes monthly reports and formally reviews them with the facility administration. 
23. Performs other related duties as required.</div>
Qualifications:
<div>1. They must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
 
2. They must have a Master's degree in Physical Therapy; or 
3. They must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or 
4. They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience. 
5. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy. 
6. Additionally, the Director of Rehab PT must have three years direct patient care experience and one year of management experience. 
7. A thorough knowledge of Medicare and third party billing is also required.</div>

<li>Exhibits courtesy, compassion, and respect to patients, residents, families, caregivers, visitors, physicians, administrators, and co-workers.</li>
