Work to support and achieve those goals.</li><li>Assess own instructional effectiveness through the Professional Development Process (PDP).</li><li>Work collaboratively with county Vocational Rehabilitation Counselors to assist in the development of a coordinated student referral process to Rehabilitation Services.</li><li>Disseminate the coordinated student Vocational Rehabilitation referral system to all high schools in MPS and participate in staff development transition activities district wide.</li><li>Work to assist individuals with disabilities to find employment, retain, and upgrade employment.</li><li>Implement system to monitor student&rsquo;s progress on the job, orient work supervisors/job coaches to student needs and strengths, supervise work sites including district wide CBVT sites, sites developed by the high school, and OJT sites to ensure legal, appropriate, and safe working conditions.</li><li>Increase career-learning opportunities through the development of job shadowing, job tours, post-secondary education tours, and service learning activities.</li><li>Consult with staff, students, parents, and adult service providers regarding appropriate transition-related plans and activities including specific strategies to enhance occupational success of special education students.</li><li>Provide Career and other Transition assessments for the purpose of evaluation or re-evaluation.</li><li>Participate in ongoing and regular staff, team and individual professional development.</li><li>Participate in on-going job embedded professional development.</li><li>Incorporate cultural relevancy into daily instruction and instructional practices.</li><li>Hands-on, project based instruction.</li></ul>

 On a daily basis you will be responsible for ensuring that you lead your team promoting &amp; providing a high-quality care. Supervise qualified &amp; unqualified staffs. You will ensure appropriate learning opportunities are met for all staff, encouraging best practice to always deliver a high quality of patient care in line with the CAS standards framework.
 
 
 You will be a motivated, organised individual with excellent communication skills with a desire to develop your leadership skills and managerial skills.
 
 
 You will be responsible to the Senior Charge Nurse for clinical guidance &amp; professional management and will deputise for the SCN as required.

The Recovery Support Coordinator at Groups is a vital member of the care team, working directly with members to provide recovery support services and coordinate care across clinical, social, and community settings. The Recovery Support Coordinator assists a defined panel of 160-180 members with Health-Related Social Needs (HRSN), treatment engagement, seamless care coordination, and overall member wellness throughout their recovery journey. Recovery support interventions are an integral part of our care model, driving positive member outcomes and engagement in treatment. This position reports to the Recovery Operations Supervisor and receives clinical supervision from the local clinical leadership team.</div>

Responsibilities <ul> <li> Act as a liaison between the facility and the outside community. </li> <li> Connect daily with potential referral sources including hospitals, physicians, insurers, case management companies and health care agencies. </li> <li> Create and execute facility sales and marketing plans and strategies. </li> <li> Conduct facility tours for potential guests and families. </li> <li> Conduct admission screenings of potential guests; determine level of care, services required, equipment needs and insurance coverage as needed. </li> <li> Welcome guests and family upon admission. </li> </ul> Qualifications <ul> <li> Bachelor's degree </li> <li> Experience in healthcare sales, marketing, insurance or commensurate education preferred. </li> <li> Experience in managed care or insurance preferred. </li> </ul> About Ciena Healthcare Ciena Healthcare is Michigan's largest provider of skilled nursing and rehabilitation care services.
