<p>Job description:</p><p><b>POSITION SUMMARY: </b>The psychiatric rehabilitation case manager provides services and functions as part of a team providing rehabilitative services to patients living in the community.</p><p><b>DUTIES AND RESPONSIBILITIES:</b></p><p>1. Provide rehabilitative services that may include service linkage, assessment and planning of services, patient advocacy, monitoring of service delivery, patient outreach, and documentation.</p><p>2. Provides crisis intervention services.</p><p>3. Provides liaison services for patients with various community agencies.</p><p>4. Functions as part of a treatment team to serve and meet the needs of patients.</p><p>5. Participates in the development and implementation of patient treatment and rehabilitation goals.</p><p>6. Provides transportation to patients as required.</p><p>7. Document all clinical activities in accordance with agency requirements.</p><p>8. Participates in 24-hour emergency coverage rotation and provides crisis intervention services.</p><p>9. Participates in ongoing clinical supervision according to agency policy.</p><p>10. Works schedule according to needs of agency.</p><p>11. Attend committee meetings as assigned.</p><p>12. Maintain a minimum caseload of 18 patients.</p><p>13. I. JOB SUMMARY</b></p><p>
Performs routine administrative and supervisory program work. Work involves assisting in establishing program goals and objectives; assisting in developing program guidelines, policies, and procedures; assisting in developing schedules, priorities, and standards for achieving program goals; assisting in evaluating program activities; and supervising the work of others. Works under moderate supervision with limited latitude for the use of initiative and independent judgment.</p><p><b>
II. ESSENTIAL FUNCTIONS</b></p><p>
 A. Assists in planning, implementing, coordinating, monitoring, and evaluating the Rehabilitation Tier Program; assists in the development of program guidelines, policies, procedures, rules, and regulations; and monitors compliance with policies and procedures.</p><p>
B. Participates in coordinating program referrals, enrollment, and completion processes for offenders; and provides liaison with other departments and divisions to explain and provide technical assistance on agency and program statutory requirements.</p><p>
C. Assists in conducting special investigations and program analyses; prepares and reviews reports on the effectiveness of program services; and oversees the collection, organization, analysis, and preparation of material in response to requests for program information and reports.</p><p>
D. Confers with program staff on program issues and problems; and provides training and technical assistance in the program area.</p><p>
E. Supervises the work of others.</p><ul><li>Performs a variety of marginal duties not listed, to be determined and assigned as needed.</li></ul></div><div><p><b>
III. MINIMUM QUALIFICATIONS</b></p><p><b>
A. Education, Experience, and Training</b></p><p>
1. Bachelor's degree from a college or university accredited by an organization recognized by the Council for Higher Education Accreditation (CHEA) or by the United States Department of Education (USDE). Major course work in Public Administration, Business Administration, Criminal Justice, or a related field preferred. Each year of experience as described below in excess of the required one year may be substituted for thirty semester hours from an accredited college or university on a year-for-year basis.</p><p>
2. One year full-time, wage-earning criminal justice, technical program support, or program administration experience.</p><p>
3. One year full-time, wage-earning experience in the use of Microsoft Office Suite or equivalent to include word processing, spreadsheet, database, or presentation software programs.</p><p>
4. Experience in the supervision of employees preferred.</p><p><b>
B. Knowledge and Skills</b></p><p>
1. Knowledge of the principles and practices of public administration and management.</p><p>
2. Knowledge of state and federal laws, rules, regulations, and statutes related to corrections and criminal justice.</p><p>
3. Knowledge of agency and departmental organizational structure, policies, procedures, rules, and regulations preferred.</p><p>
4. Skill to communicate ideas and instructions clearly and concisely.</p><p>
5. Skill to coordinate with other staff, departments, officials, agencies, organizations, and the public.</p><p>
6. Skill to interpret and apply rules, regulations, policies, and procedures.</p><p>
7. Skill in administrative problem-solving techniques.</p><p>
8. Skill in the use of computers and related equipment in a stand-alone or local area network environment.</p><p>
9. Skill to prepare and maintain complex records and files in an automated system.</p><p>
10. Skill to review technical data and prepare technical reports.</p><p>
