The primary purpose of this position is to participate in developing plans of patient care for each resident and to review the resident&rsquo;s medical and nursing treatments to ensure they are in accordance with the resident&rsquo;s care plan and wishes.
Essential Duties and Job Responsibilities:
<ul>
<li>Review resident care plans for appropriate goals, problems, approaches, and revisions based on nursing needs.</li>
<li>Ensure all personnel involved in providing care to the residents are aware of care</li>
<li>Schedule and facilitate care plan</li>
<li>Monitor and guide the completion of PPS and OBRA</li>
<li>Ensure accuracy of clinical assessments and documentation.</li>
<li>Ensure legal compliance with all state and federal guidelines, including, but not limited to following MDS processes.</li>
<li>Manage the utilization review (UR)</li>
<li>Oversee the MAR&rsquo;s treatment, flow sheets, and physician&rsquo;s</li>
<li>Chart nurses&rsquo; notes in an informative and descriptive manner that reflects the care provided to the resident and resident&rsquo;s response to care.</li>
<li>Coordinate with other departments to ensure consistency in resident care.</li>
<li>Participate in facility surveys by authorized government</li>
<li>Report findings and make recommendations to management.</li>
<li>Act in a supporting role to the Administrator and Director of Nursing of the facility.</li>
</ul>
Qualifications:
<ul>
<li>Nursing degree from an accredited college or</li>
<li>In accordance with state and federal regulatory requirements, maintain a current, unencumbered, active license to practice as a licensed nurse in this state.
<ul><li>Oregon: RN is required</li>
<li>Washington: RN is preferred</li>
</ul></li><li>MDS certification</li>
<li>Knowledge and experience in the RAI process, CAAs, RUG categories, and MDS 0.</li>
<li>Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care communities.</li>
