</b></h1>The Associate Healthcare Director (AHCD) is responsible for assisting the community&rsquo;s Healthcare Director (HCD) in leading the health and wellness activities for the community&rsquo;s resident population and assuring that the resident care department operates efficiently and effectively. The AHCD will assist in leading the team of medication technicians and nursing assistants in the provision of quality care and services while maintaining compliance with all applicable local, State, and Federal Regulations and Harmony Senior Services policies and standards
<p></p><ul><li>Evaluates residents prior to move-in to determine whether their care and service needs can be met in the community. Conducts re-evaluations as indicated by the resident&rsquo;s condition and/or State regulations.
</li><li>Develops a service plan unique to each resident based on the evaluation and the resident&rsquo;s stated preferences. Revises the service plan as indicated by the resident&rsquo;s condition and/or State regulations.
</li><li>Collaborates with each resident&rsquo;s physician, pharmacist, third party healthcare providers, and other members of the healthcare team to promote coordinated care and services.
</li><li>Participates in the oversight of the community&rsquo;s medication management program. Collaborates with the pharmacy, conducts med tech training and skill validation, performs routine audits and other activities that promote safety in medication administration.
</li><li>Maintains a working knowledge of all applicable local, State, and Federal regulations.
</li><li>Assures that resident evaluations and documentation meet all requirements and collaborates with the HCD to assure that the resident care department is survey ready at all times.
</li><li>Participates in the development of, and compliance with, plans of correction as applicable.
</li><li>Assures that infection control standards are maintained to prevent or limit the spread of communicable infectious illness.
</li><li>Provides training and skill validation for med techs, personal care aides and others to promote safe resident care.
</li><li>Participates in the hiring process as needed by conducting interviews and providing onboarding training and coaching to new resident care team members.
</li><li>Participates in community and resident care meetings as needed when the HCD is unavailable.
</li><li>Provides leadership that promotes a culture of collaboration and teamwork.
</li><li>Establishes and maintains a collaborative and positive working relationship with all community team members.
</li><li>Communicates compassionately and regularly with residents&rsquo; families. Promptly responds to questions or concerns and participates in effective problem-solving.
</li><li>Completes all State and Harmony initial, annual and other required training.
</li><li>Partners with the Harmony Square Director and Healthcare Director on managing Behavior Expressions and the overall wellness needs of the Harmony Square residents.

</li></ul><p>Qualifications/Skills/Educational Requirements:
</p><ul><li>Level of formal Education: Current valid and unencumbered license to practice as an RN or
</li><li>LPN in the State in which the community is located.
</li><li>Areas of Study: Nursing. Experience in caring for geriatric individuals and those with
</li><li>Alzheimer&rsquo;s and other forms of dementia strongly preferred.
</li><li>Years and type of Experience: 1-2 years&rsquo; experience required; 3 or more years' experience

<div><p><b>Summary</b>: Responsible for enhancing and improving the SRIM experience specifically in<br>
heartfelt care for our residents and their families. Demonstrates and promotes SRIM&rsquo;s core value<br>
of the golden rule. Treats others including residents, families, visitors, and associates in the same<br>
way we would like to be treated. Attends to all the daily living needs of residents; assistance with<br>
ADL&rsquo;s, meal service, light housekeeping, and laundry while encouraging resident independence.</p><p><b>
Duties and Responsibilities</b> include the following. Other duties may be assigned.<br>
1. Provides assistance to residents with all activities of daily living including dressing, grooming, peri-care, toileting, and hygiene.*<br>
2. Speaks to and interacts with residents at all levels of care while showing empathy, kindness, compassion, and respect.*<br>
3. Observes and reports to supervisor daily.*<br>
4. Changes in the resident's health and emotional wellbeing.*<br>
5. Serves and assist residents in the dining room at breakfast, lunch, and dinner daily.*<br>
6. Responsible to initiate and participate in leisure activities provided to residents as described in the activity calendar.*<br>
7. Always maintains a clean, safe, and orderly environment for all residents.*<br>
8. Performs daily general housekeeping duties following established cleaning schedules.*<br>
9. Ensures and supports a dignified caring atmosphere for residents, families, visitors, and associates.*<br>
10. Responds promptly and courteously to resident's request for assistance including phone calls and requests from family members and visitors.*<br>
11. Communicates effectively in a language that each resident is capable of understanding.*<br>
12. Ensures residents and or responsible parties sign residents out when they leave the community.*<br>
13. Maintains a safe and secure working environment free.*<br>
14. Responsible for objects, situations that could cause harm, falls to residents, families visitors, and associates.*<br>
15. Practices good body mechanics and safe working habits.*<br>
16. Observes and reports safety hazards immediately to supervisor.*<br>
17. Provides assistance to associates when requested or required.*<br>
18. Knows, understands, follows established policy, and procedures in emergency situations.*<br>
19. Responsible for being essential personnel during emergencies and or inclement weather.*<br>
20. Reports or remains at community until released by supervisor.*<br>
21. Works as a team player with other associates and management in accomplishing work assignments.*<br>

27. Refrains from discussing resident care issues with families, visitors, and refers all inquiries to immediate supervisor.*<br>
28. Knows, understands, and complies with HIPAA regulations.*<br>
29. Assists with training of new employees.*<br>
30. Assists in mentoring new employees.*<br>
31. Responsible for without asking, takes the initiative in work, seeks out additional information, offers solutions, and suggestions.<br>
32. Responsible for willingly taking on special projects or assignments outside of regular duties.<br>
33. Provides a variety of housekeeping and or laundry duties in maintaining the community in an orderly, clean, and sanitary condition.<br>
34. Performs all other reasonable duties as assigned or as requested.</p><p><b>
Supervisory Responsibilities:</b><br>
Directly supervises employees in the Resident Services Department. Carries out supervisory<br>
responsibilities in accordance with the organization's policies and applicable laws.<br>
Responsibilities include training employees; planning, assigning, and directing work; appraising<br>
performance; rewarding and disciplining employees; addressing complaints and resolving<br>
problems.</p><p><b>
