POSITION SUMMARY:

The Residential Aide/Medication Aide is responsible for assisting the residents in their activities of daily living, and in maintaining the social and physical environment for their comfort, safety and well-being in accordance with Department of Health regulations, community standards, and also as directed by a supervisor. The ideal candidate must possess strong interpersonal skills in order to deal with the physical and social needs of our resident population. The ability to communicate calmly, patiently, and effectively with residents, families, visitors is a must.

This is a Full-time hourly position in our Assisted Living unit from 3pm-11pm. You will work a 5 day on/2day off rotating schedule in which ever other weekend is a must. 


PRINCIPAL RESPONSIBILITIES:
<ul>
<li>Work under the supervision of the Registered Nurse/LPN in accordance with NYS DOH Regulations, and community standards.</li>
<li>Provide residents with assistance in Activities of Daily Living (ADL&rsquo;s), including but not limited to bathing, dressing, grooming, toileting, changing bed linens, laundry, incontinence care as needed.</li>
<li>Follow the resident&rsquo;s ISP (Individualized Service Plan/Care Plan), and the HHA Care Plan, and complete daily task sheets/assignment.</li>
<li>Completing required documentation accurately, and timely.</li>
<li>(If trained as med tech-HHA), Assist with and properly document medications, following accepted practices, and in accordance with DOH and Community standards.</li>
<li>(If trained as med tech-HHA), order meds when running low, receiving/signing for medications from pharmacy, notify LPN/RN is refills needed, or any questions, issues, or concerns with medications.</li>
<li>(If trained as med tech-HHA), document on 24 hour report, and shift change report.</li>
<li>Observe assigned, and all residents, and report any changes, or concerns regarding resident, or care to LPN/RN/Supervisor.</li>
<li>Respond to call signals/pull chords/requests for help, call for emergency services (911) if needed. Report incident on designated forms, and complete appropriate paperwork.</li>
<li>Assist with dining services as necessary.</li>
<li>Encourage residents to attend activities, assist to get to activity as needed.</li>
<li>Attend required (12 hour minimum) in-servicing, and as directed by supervisor/managers.</li>
<li>Collect, wash, dry, fold, distribute laundry/linens.</li>
<li>Follow, participate in all established fire/safety components, including in participating in monthly drills, and fire safety training.</li>
<li>Report and accidents/incidents immediately.</li>
<li>Meet/greet all residents, families, visitors, departments in a friendly, respectful, helpful, courteous manner with resident/family satisfaction always being a priority.</li>
<li>Provide all/only services within your Home Health Aide Certification/Scope of Tasks.</li>
<li>Frequent contact with residents, families, visitors, and other departments.</li>
<li>Flexibility with the work schedule, including day/evening/night hours to meet the residents needs and may be required to do mandatory overtime.</li>
<li>Ability to manage multiple interruptions, and ability to make independent decisions when conditions warrant, and to demonstrate good judgment in areas of responsibility.</li>
<li>Represent the Community in such a way to exhibit maximum professionalism reflecting accuracy, attention to detail, and overall competency.</li>

Minimum Requirements

Supervise and document parent&rsquo;s administration of medication to their children.</li><li>Transport residents to scheduled and/or emergency medical appointments as directed. Deliver appropriate documentation to the Nurse/Parent Educator for the upkeep of medical records.</li><li>Transport and accompany clients for residential activities (i.e., grocery shopping, recreational activities, etc.) and responsibilities (i.e., Board of Social Services, WIC, vocational training, volunteer services, employment, self-help meetings, etc.).</li><li>Facilitate residential conflict resolution and provide crisis intervention as needed.</li><li>Complete documentation in daily communication log, medication log, incident reports, visitation, etc., and ensure appropriate staff communication across shifts.</li><li>Implement and ensure completion of daily activities (i.e., meals, chores, shipping, psycho educational groups, recreational activities, client appointment, and transportation).</li><li>Provide residential service information to supervisors for weekly multidisciplinary team conferences.</li><li>Participate in appropriate case management with referral/funding agencies, legal system and outsourced resources as needed.</li><li>Facilitate and record fire drills and other emergency drills procedures as instructed.</li><li>Ensure facility compliance with regulatory licensing requirements.</li><li>Maintain records as required by organization policies and regulatory and accrediting agencies.</li><li>Participate in training programs to enhance teamwork, team collaboration, communication and program development.</li><li>Participate in program, committee, and clinical staff meetings as indicated.</li><li>Participate in program evaluation and ongoing efforts to enhance quality of services.</li><li>Participate in building maintenance needs and upkeep as requested.</li></ul>
