<div></div><div><div><p>The Cape &amp; Islands Adult Community Clinical Services (ACCS) operated by the Department of Mental Health (DMH) is seeking a dynamic candidate who will provide direct care services to individuals in a residential group home setting. This position will act as a primary advocate for assigned Persons Served; assisting individuals to meet their rehabilitation goals, completing required daily documentation and maintaining a Person Centered, Trauma Informed approach.</p><p>
The selected candidate will work as part of a residential group home team within the ACCS program and will provide rehabilitative interventions to persons with major mental illness by participating in the development of comprehensive assessments as well as developing and implementing the clients&rsquo; Individual Action Plan. Additional responsibilities may include securing appropriate services for persons served by working with other programs within the agency as well as the community such as SSI, Mass rehab, day programs, psychiatric and medical services. Knowledge of all medications used within the program is expected as he or she will be distributing medications to clients and educating on proper usage. The incumbent in this position will maintain a therapeutic atmosphere through professional performance and attitude to promote quality care and person served growth. Transporting clients to and from appointments or activities using a program vehicle or their own vehicle is necessary for this role.</p><p></p><p><b><br>

Duties and Responsibilities (these duties and responsibilities are not all inclusive):</b></p><ul><li>
Engage persons, individually and/or in group, in the development of meaningful activities, such as recreational, educational, health and wellness, within the program and the community to foster an improved quality of life.</li><li>
Assist in the development and implementation of person served Individual Action Plans through direct observation of the person served, full knowledge of the person served history, and engagement with the person served in order to assist them in learning the necessary skills to live successfully in the community.</li><li>
Provide rehabilitative interventions and utilize coping strategies, natural supports and safety planning to assist persons served in building and using skills and accessing resources to achieve treatment goals, manage symptoms and live productive and meaningful lives.</li><li>
Knowledge of all medications used within the program, including their indications, risks and side effects, and the ability to teach person served about their medications and how to take them appropriately by utilizing MAP related material and training.</li><li>
Transporting or arranging transportation for persons served that need that level of service.</li><li>
Responsible for maintaining person served records in a timely manner by documenting in EMR and paper record as well as completing all other related forms to ensure compliance with DMH regulations and Rehab Options requirements.</li><li>
Ongoing yard, house and vehicle maintenance and cleaning as assigned by supervisor or designee to ensure that the condition of GLE site/yard meets Rehab Option, and Licensing. Keep emergency exits maintained at all times.</li></ul><p></p><p><b><br>

Encourages clients to express their feelings and discuss what is happening in their lives, and help them to develop insight into themselves and their relationships.</li><li>

Qualifications:</b></p>

This position requires a valid and active QMHA credential certification through MHACBO.</li><li>
A bachelor's degree in psychology, social work, counseling or a related behavioral sciences field OR a minimum of three years experience working with individuals with severe and persistent mental illness.</li><li>
Demonstrated ability to communicate effectively; understand mental health assessment, treatment and service terminology and individual, group, family and other counseling techniques; program policies and procedures for services and supports identified in an Individual Services and Supports Plan.</li><li>
Demonstrated effective communication skills and ability to provide culturally-competent treatment services to diverse client populations and maintain a cooperative working relationship with others in a culturally diverse environment.</li><li>

The Community Health Worker (CHW) must be a frontline public health worker who is a trusted member of the community served. The CHW supports the Guilford County Sheriff&rsquo;s Office Re-entry Program and Jail Diversion Program, which serves individuals who interface with the criminal justice system due to serious mental illness and substance use. The CHW focuses on supporting formerly incarcerated individuals and those at risk of incarceration to successfully reintegrate into the community, reduce recidivism and improve access to healthcare, social services and supportive networks.</div><div>

Under general supervision, provides one-on-one behavioral interventions with children, youth and/or adults receiving specialty mental health services in the client's home/residential placement/school or during community outings; performs related work as assigned.</p><p>
</p><p><b>Responsibilities and Duties</b></p><ul><li>Maintain assigned caseload of weekly billable insurance hours.</li><li>Responsible for ensuring services are provided in the community.</li><li>Participating in the person-centered planning process.</li><li>Providing direct community-based behavioral health services and support for the individual and family as indicated in the Person-Centered Plan.</li><li>Monitor and track consumer hours and annual intake dates and complete as assigned.</li><li>Assisting the Case Manager in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward goals.</li><li>Responsible for completing the OnTarget tasks assigned within 24 hours.
</li><li>Assisting the Case Manager with consulting identified non-medical providers, engaging community and natural supports and including their input in the person-center/ed planning process.</li><li>Ensure that all information obtained while at Carolina Therapeutic Services is kept confidential and will remain confidential beyond employment.</li><li>Ensure confidentiality training is updated.</li></ul><br>
