<b>POSITION SUMMARY:</b> The RN Care Manager is a professional nurse who collaborates with physicians, facilities, health care team members and patient/families to manage chronic conditions/complex illness and coordinate care for specific patient populations across the continuum and between care settings as an important member of the Patient Centered Medical Home team.
<br><br>
<b>What the RN Care Manager will:</b><br>
<ul>
<li>Participate and engage their patients in the assessment, planning, monitoring and evaluation of the ongoing care for chronic condition/complex illness management.</li>
<li>Establish plans of care including patient education, self-management, care coordination, and coordinating resources for effective, efficient and appropriate patient care.</li>
<li>Provide ongoing monitoring and evaluation of patient outcomes.</li>
<li>In addition to these responsibilities, the RN Care Manager also performs the duties of population health management, which includes risk stratification to identify high and rising-risk patients. This also includes care coordination and management, patient engagement, and addressing SIOH needs in proactive efforts to improve outcomes, reduce cost, and improve health equity.</li>
<li>The RN Care Manager will assist patients in optimizing their health status; providing interventions designed to avoid hospitalization and emergency room visits.</li>
</ul>
<b>What the RN Care Manager will need:</b><br>
<ul>
<li>Graduate of an accredited school of nursing.Bachelor of Science degree in Nursing required. Master's Degree in Nursing (or actively pursuing Master's Degree in Nursing) preferred. Recent case management experience preferred.</li>
<li>Current licensure to practice nursing in Michigan.</li>
<li>One year of experience in Case Management preferred and three years of clinical experience in nursing required.</li>

Summary of position/role of team</b>:
</p><ul><li>The Nurse Manager will provide administrative and clinical oversight of assigned staff in their cohort and their respective programming.
</li><li>Responsible for the supervision, training, and oversite of all nursing staff across their assigned cohort.
</li><li>Oversees scheduling and program needs within their designated crisis cohort.
</li><li>Other duties include onboarding, training, and development of newly hired RNs, assisting the Nursing Supervisor with ensuring the delivery and oversight of psychiatric care to patients involved in medication services provided at the Center's crisis programming.
</li><li>Work closely with a team of nurses, providers and Directors to address other related clinical and administrative needs.
</li><li>Oversees the quality of nursing services and ensures appropriate care is being provided.
</li><li>Provide direct clinical services under HRMC protocols. An essential job function is a reliable and on site, with regular attendance or whatever the job requires.</li></ul>

TYPICAL WORKING CONDITIONS/ENVIRONMENT</b></p>

The Nurse Manager will work throughout the Center in their assigned crisis cohort including on an as needed basis, but will not be working out in the community.
</li><li>The Nurse Manager is expected to cover any open shifts in either crisis cohort if unable to obtain coverage.</li></ul>

Document administered medications in the EMAR Document clinical services provided in the medical record on same day as service provided in a clear and concise manner.</li></ul>

5. Completion of required HRMC </b><b>training</b><b> to maintain status as credentialed provider</b></p>
<ul><li>
Maintain an active RN license in the state of Tennessee in accordance with the state guidelines</li><li>
Complete any Center Required Self-Directed Learning or Training Modules in the required time frames.</li><li>
Complete First Aid, CPR, and HWC trainings within the required time frames.</li></ul>
<p><b>
Schedule:
</b></p><ul><li>The Nurse Manager is expected to fulfill the agreed upon hours when hired.
</li><li>Unless otherwise approved, the Nurse Manager is expected to be present in the clinic from 8am-5pm and is expected to complete a minimum of 16 hours of clinical work per week which may include nights and/or weekends.
</li><li>Must be flexible with scheduling as the Nurse Manager will rotate being on call and is expected to cover open shifts in their assigned cohort on a PRN basis.

Experience / Knowledge:
</b></p><ul><li>1-3 years of psychiatric nursing post-graduation is required.

</li><li>Should possess sound knowledge of state regulations and best practice guidelines, psychiatric medications, mental health diagnoses, treatment modalities and interventions.
