A multi-site Skilled Nursing operator in New York is seeking an experienced Regional MDS Coordinator (RN) to oversee and standardize MDS operations across five facilities.
This is a permanent, senior-level leadership role requiring extensive Skilled Nursing and multi-site MDS experience. The Regional MDS Coordinator will directly impact PDPM performance, Case Mix Index (CMI), regulatory compliance, and revenue integrity across the organization. 
Position Overview
The Regional MDS Coordinator will establish, audit, and optimize MDS processes across all five facilities to ensure consistency, reimbursement accuracy, and survey readiness.
This role works closely with Corporate Clinical Leadership and Facility Administrators to drive documentation integrity, regulatory compliance, and operational performance. 
Non-Negotiable Requirements<ul><li>
Active New York RN license required</li><li>
Minimum 3+ years of MDS experience in Skilled Nursing Facilities</li><li>
Demonstrated experience overseeing or supporting MDS operations across multiple facilities strongly preferred</li><li>
Deep understanding of PDPM reimbursement drivers and CMI optimization</li><li>
Strong working knowledge of CMS regulations and NY DOH survey processes</li><li>
Experience conducting internal MDS audits and compliance reviews</li><li>

<div>The MDS (Minimum Data Set) Coordinator/Nurse is an RN or LPN that conducts federally mandated assessments of the residents at a long-term care facility. MDS Coordinators are responsible for collecting integral data and compiling it into a thorough assessment to help determine the functional capacity with appropriate plan of care and to determine the reimbursement for all payer sources in relation to the RUG-IV 66 and RUG-IV 48 system established by the Centers of Medicare and Medicaid Services.

Essential Job Functions:
<ul>
<li>The MDS Coordinator reports to the facility Administrator</li>
<li>Completion of all OBRA, PPS and Managed Care MDS</li>
<li>Completion of corresponding Admit MDS Tracking Forms, Death in the Facility Tracking Forms and any Discharge Assessments required per the RAI Manual</li>
<li>Completion of all Nursing Care Plans and the coordination of the other disciplines to ensure timely initiation of their Care Plans and/or revised in conjunction with the OBRA schedule and exacerbation of the problem requiring review of the problem, goal or interventions</li>
<li>Care Plan Conferences will be held within the first 21 days of admission and every 90 days thereafter as a minimum standard of practice</li>
<li>Coordination of the Care Plan Conference letters for residents and families (Social Service provides the invitations to the residents and the front office sends the invitation letters to the family members)</li>
<li>Completion of the monthly OBRA calendar by the 20th of the month</li>
<li>Completion of the weekly OBRA, PPS and Care Plan schedule for the IDT</li>
<li>Transmission of OBRA/PPS MDS Assessments to CMS per the Guidelines</li>
<li>Completion and Certifications/Re-certifications when a resident is receiving Medicare Part A Benefits</li>
<li>Coordination of the AB Notices and Medicare Cut Letters</li>
<li>Completion of the 100 day Medicare Part A and Managed Care Log</li>

<li>Coordination of the Insurance/Managed Care/Medicare Replacement caseload and re-authorization for services</li>

<li>Reviewing the 24 hour report daily to monitor for any potential Significant Changes in Status and need for an new Full Comprehensive MDS Assessment and/or revisions or development of new Care Plans</li>
