Duties and Responsbilities<ul><li>
Provide specialist assessment and management for older adults with multimorbidity, frailty, polypharmacy issues, functional decline, cognitive impairment, and geriatric syndromes.</li><li>
Lead Comprehensive Geriatric Assessments (CGA) in inpatient, outpatient, emergency, and community settings.</li><li>
Manage acute and chronic medical conditions common in the elderly, including falls, delirium, dementia, stroke, mobility disorders, and end-of-life care needs.</li><li>
Lead ward rounds in geriatric wards, frailty units, acute medical units, orthogeriatric services, or rehabilitation settings as appropriate.</li><li>
Support perioperative medicine for older or high-risk surgical patients, collaborating with surgery and anesthesia teams.</li><li>
Participate in on-call rota for acute or general medical services as required.</li><li>
Coordinate safe discharge planning and continuity of care with community, social care, and rehabilitation services.</li><li>

Develop and implement clinical pathways and protocols for frailty, falls, dementia, delirium, and complex discharge management.</li><li>
