<p><b>Job Purpose/Summary</b></p><p>Case Manager will be responsible for screening referrals and enrolling individuals and families in the Emergency Shelter program. The program will provide short-term shelter placement for intact families, fathers with daughters, disabled, and/or transgender individuals who cannot be placed in traditional mass shelters. The Case Manager may provide ongoing case management and in-shelter services to clients in order to ensure success.</p><p><b>Duties and Responsibilities</b></p><p>List the primary job duties and responsibilities; provide specific examples of the activities required for the position.</p><ul><li>Provide information, financial assistance, referrals, and professional case management for individuals and families entering Emergency Shelter.</li><li>Provide a case plan, case notes, and case goals for each client seen. <p><b>Women&rsquo;s and Children&rsquo;s Crisis Shelter</b></p><p><b>Job title:</b> Shelter Case Manager</p><p><b>Department:</b> Emergency shelter</p><p><b>Reports to:</b> Shelter Director</p><p>For over 48 years, the Women&rsquo;s &amp; Children&rsquo;s Crisis Shelter has provided programs and services that form a critical continuum of response, healing, and prevention to survivors of domestic violence and their families. To encourage survivor self-determination, services are delivered through an intersectional, strengths-based, and trauma-informed lens that aligns with our mission of commitment to provide safety shelter and hope to victims of domestic violence.</p><p>Reporting to the Shelter Director, the Case Manager ensures that the day-to-day operations of the emergency shelter meet and/or exceeds the program&rsquo;s quality, outcomes, and services to clients. A Case Manager provides community-based case management services to clients, draws upon clients&rsquo; strengths to inform service planning, advocates on behalf of clients, collaborates with other service providers, provides resource referrals, monitors client progress, and manages crisis situations. This position also provides administrative support by maintaining client files, entering data, and completing required documentation in compliance with program and funder requirements. An Emergency Shelter Case Manager provides housing navigation and housing stabilization services to youth ranging from ages 16-22 living in a 24-hour residential facility. Services include facilitating meetings with providers or managing conservators, transporting, and meeting basic needs in a way that positively supports youth development as well as their individual goals.</p>

This <b>full time, non-exempt</b> position earns a competitive rate ranging from $24.86-$27.17/hr. depending upon experience. RESPONSIBILITIES</b></p>

<li>Provide client-centered, healing-centered case management services to diverse individuals who may have experienced chronic homelessness, systems involvement, mental health challenges, substance use challenges, a history of trauma, early pregnancy/parenthood, etc.</li>
<li>Identify and leverage clients&rsquo; strengths as part of service planning to help clients achieve their self-defined goals.</li>
<li>Work with teammates and other support staff to ensure fidelity of the Strengths Model of Case Management is adhered to and considered in interaction with and documentation of client-case manager engagement.</li>
<li>Assist clients with their day-to-day needs and development of life skills, which may include (but is not limited to) grocery shopping, meal preparation, cleaning activities, budgeting, navigating public transportation, and job search activities.</li>
<li>Assess clients&rsquo; mental and physical wellness, needs, preferences, and abilities, and utilize this information to develop tailored wraparound supports and referrals that are aligned with client preferences.</li>
<li>Regularly and effectively assess and manage incidents and crisis situations, with calm, care, and positive regard for client, using de-escalation and safety planning techniques as appropriate.</li>
<li>Analyze barriers and challenges as they arise and deliver safe, effective, and client-focused solutions.</li>
<li>Actively engage in the community alongside clients to support the achievement of client&rsquo;s self-defined goals, including collaborating with internal and external partners to advocate for client needs, coordinate services, and provide referrals.</li>
<li>Assist clients in building a network of support and (re)building relationships with family and friends.</li>

<li>Perform heavy data entry into agency/funder databases and ensure timeliness and accuracy of data entry.</li>

<li>Ensure required client consents are collected and documented and that clients have a thorough understanding of the program prior to consenting to participate.</li>
<li>Provide timely interventions and outreach services to clients when they appear to disengage from services.</li>
<li>Transport clients to agencies and services.</li>
<li>Prepare for and actively participate in supervision meetings and all other required meetings or trainings.</li>

<p><i>Additional Responsibilities for the Emergency Shelter</i>:</p>
<ul>
<li> Manage emotional, physical, and verbally escalated situations by applying an emergency behavior intervention to de-escalate youth back to normalcy, as well as physical restraints utilizing &ldquo;Handle with Care&rdquo; techniques when necessary.</li>
<li> Follow DFPS Minimum Standards for incident reporting, implementation of safety plans and action plans in coordination with Youth Care Workers and Program Director of Emergency Shelter.</li>
<li> Coordinate intake and discharge of client services, including consent for services and risk assessment for supervision guidelines.</li>
<li> Follow health and safety guidelines per DFPS minimum standards and LifeWorks safety planning.</li>
<li> Rotate in the on-call duties which includes being available to on-duty staff and may involve taking calls, responding to emails, covering shifts or providing on-site support in an emergency.</li>
<li> Maintains required program training including 40 hours within the first month of employment and 20 hours of ongoing annual training.</li>

We&rsquo;re looking for a Housing Case Manager to join our 24-hour adult shelter team and support individuals experiencing homelessness as they work to secure housing, build stability, and self-sufficiency. In this role, you will support residents, foster a safe and respectful environment, and help keep programs running smoothly while delivering short- and long-term case management, supportive services, and access to job training and placement opportunities. Through consistent, onsite support, you&rsquo;ll play a key role in ensuring residents feel supported, seen, and genuinely set up for long-term success.</div><div></div><div><b><br>

Education or Training Equivalent to:</b></div><div><ul><li>

Qualifications and Abilities:</b></div><div><ul><li>
Minimum of two (2) years&rsquo; experience working with under-resourced or low-income populations; experience working with individuals experiencing homelessness preferred.</li><li>
Bilingual (English/Spanish) preferred</li><li>
Experience in residential and/or housing programs.</li><li>
Knowledge of Housing First and harm-reduction principles.</li><li>
Working knowledge of SSI, SSDI, and SDI.</li><li>
Experience with data entry systems (HMIS, Salesforce, or similar).</li><li>
Proficiency with Microsoft Word, Excel, cloud-based systems, and Adobe.</li><li>
Comfort working in a culturally and ethnically diverse environment.</li></ul></div><div></div><div><b><br>

Incorporate Cal AIM care coordination and housing navigation strategies into individualized care and housing plans, in collaboration with healthcare, behavioral health, and community-based partners.</li><li>
Provide housing-focused case management for individuals and in group settings.</li><li>
Identify and address barriers to housing, including chronic homelessness, mental and physical health needs, substance use disorders, under-employment, lack of income, and other systemic barriers.</li><li>
Skillfully prevent and/or resolve conflicts using a strength-based approach.</li><li>
Respond to urgent and emergency situations, including coordination with EMTs, fire, law enforcement, psychiatric services, and community partners as needed.</li><li>
Prevent and resolve conflicts using a strength-based, trauma-informed approach.</li><li>
Provide supportive services, guidance, and referrals to community resources.</li><li>
Engage program participants regularly and consistently to support program participation and progress toward housing stability.</li><li>
Maintains awareness of the overall shelter environment and collaborates with the Program Manager to identify, report, and help address facility&ndash;related needs or concerns to support a safe, functional, and welcoming space for participants and staff.</li></ul></div><div></div><div><br>

Maintain accurate and timely data entry in HIPPO (Salesforce database) and HMIS.</li><li>
Meet all documentation and case management deadlines, including intake, enrollment, reassessments, housing goals, and discharge documentation.</li><li>
Participate in quality assurance, monitoring, and audit processes, including responding to documentation reviews and corrective actions.</li></ul></div><div></div><div><b><br>
Case Management, Housing-Focused Services, and Outcomes</b></div><div><ul><li>
Develop, implement, and regularly update individualized, client-centered housing and care plans.</li><li>
Monitor, document, and adjust plans proactively to address changing needs or barriers.</li><li>
Coordinate services with internal teams and external partners to ensure continuity of care and timely referrals.</li><li>

Property Management and Housing Placement</b></div><div><ul><li>
Manage a full caseload, weekly meetings with strong organization, time management, and prioritization skills.</li><li>

Participate in weekly supervision, program meetings, all-staff meetings, and required training.</li><li>
Maintain confidentiality and comply with all agency policies, including HIPAA where applicable.</li><li>
