Bethany House Services is seeking a compassionate, organized, and trauma-informed Case Manager to support families experiencing homelessness in our residential shelter. The Case Manager will carry a caseload of 12&ndash;15 families and provide comprehensive, strengths-based, and housing-focused services that promote safety, stability, and long-term self-sufficiency. The ideal candidate is a Licensed Social Worker (LSW) with experience working with families, public and private funded programs, and Job and Family Services (JFS) systems.

Encourage clients to enroll in life-skills classes workshops for family members.</li></ul>
Trauma-informed, family-centered practice<ul><li>
Apply trauma-informed care principles (safety, trustworthiness, choice, collaboration, empowerment) in all interactions and documentation.</li><li>
Utilize motivational interviewing, de-escalation, critical time intervention (CTI) and strengths-based approaches to engage caregivers and children.</li><li>
Maintain appropriate boundaries and uphold confidentiality and client rights.</li></ul>
Case management and documentation<ul><li>
Maintain an active caseload of 12&ndash;15 families, meeting agency productivity and contact standards including at least one 30-minute 1:1 with clients once per week.</li><li>
Provide families who have exited the shelter program with limited follow-up services and support through the BHS post-shelter support program and Aftercare.</li><li>
Complete timely, accurate documentation in HMIS and agency EHR per HUD, JFS, and agency policies.</li><li>
Actively work to maintain appropriate documentation and conduct monthly file (peer) reviews.</li><li>
Track outcomes and contribute to continuous quality improvement and performance metrics.</li></ul>
Shelter collaboration and operations<ul><li>

 About the Role:
 
 
 
 The Shelter Case Manager will work with individuals and family residing in the Loudoun Homeless Services Center (LHSC) Emergency Shelter, primarily focusing on rapid rehousing initiatives. The role will provide low-barrier, housing-focused services that align with the Housing First model and trauma-informed. The Case Manager will welcome all participants with dignity and respect, supporting them in achieving their housing goals swiftly and effectively. Day to day responsibilities include conducting assessments to identify barriers to housing, assisting participants in accessing resources and benefits, facilitate goal setting, budgeting, and empowering participants in their progress towards securing and maintaining housing. Once assisting in locating housing is secured, the Case Manager will work to link households with services and resources to help them stabilize in their new housing and neighborhood. Providing the households with a warm handoff that may include assistance with tenant education, income enhancements, and connections to community resources. This role requires a fast-paced approach, as participant relationships are often time-limited. The Case Manager must effectively collaborate with landlords, community partners to prioritize a permanent positive housing solution for participants.
 
 
 Duties and Responsibilities:

 Ensure each participant feels welcome in the shelter, has basic needs items and supplies throughout their stay, and understands community guidelines and safety procedures

 Assist with developing a rental assistance package for participants utilizing a variety of funding sources, properly document the distribution of funds

 Assist participants in accessing financial resources, including rapid re-housing rental assistance, and prepare and submit necessary documentation

 Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees, deployed staff, and involved agencies to ensure participants receive needed services in a timely manner

 Assure that all relevant information pertaining to participants is disseminated to all appropriate staff members in a timely manner

 Encourage and support the household to focus their efforts on attaining the objectives specified in their Service Plan

As the Clinical Coordinator, you will be responsible for administering the operations and personnel of the clinical division within the department. This includes direct supervision of four full-time counselors, one full-time case manager, as well as interns and practicum students. 
Qualification:

<li>Master&rsquo;s degree in social work, counseling, psychology, or related field.</li>
<li>Minimum of two (2) years of supervisory experience.</li>
<li>At least one (1) year of program administration experience.</li>
<li>Eligible to obtain and maintain practicing licensure, including LCSW, LMFT, or LMHC credentialing.</li>

Key Responsibilities:

<li>Supervise and support clinical staff, including hiring recommendations, performance management, and conflict resolution.</li>
<li>Oversee the training and supervision of interns and practicum students.</li>
<li>Participates in the training of agency staff.</li>
<li>Ensure accuracy and timeliness of required documentation, including but not limited to, case notes, treatment plans, CANS, clinical recommendations, and advocacy letters.</li>
<li>Manages the distribution of counselor clinical case load including shelter placed youth and community-based services.</li>
<li>Review client referrals from DCS, Probation, Safe Place, and parents/guardians for service appropriateness and placement for agency services. Meets with Shelter residents and family members to resolve conflicts and provide counseling, to correct and/or understand their behavior.</li>
<li>Participate in a 24-hour on-call rotation to respond to emergencies, including responding to calls, assessing situation and needs of site visitors, providing initial counseling, and taking action as situation demands.</li>
<li>Occasionally requires some work on evenings and weekends.</li></ul>
