Shelter Case Manager: Job Description

Position Description: A full-time position providing case management services and eligibility screening to homeless Veterans in or entering the Shelter program; and providing coordination of shelter activities.

Position Type: Full time, Salaried, Exempt, Benefits Eligible.

Hours: Office hours are typically weekdays 9-5; minimum of 40 hours per week required; Hours vary based on need and assignments; Some nights and weekends may be required; After hours and weekend on-call shifts of approximately one weekend per month are required.

Reports to: Shelter Program Manager

Supervises: None

Collaborates with: Intake Specialist; Case Managers; House Managers; and Housing Program Managers; All other WH staff; External agencies and partners; Volunteers and the Public.

Basic Function: The Shelter Case Manager: Provides shelter, housing services and appropriate individualized case management actions and client advocacy for homeless individuals or families; administers program objectives and elements as appropriate; links residents to resources; formulates housing plans that promote moving towards self-sufficiency; ensures the safety of all residents by implementing, monitoring and enforcing the boundaries and objectives of the transitional shelter program.

Principal Duties &amp; Accountabilities:
<ul><li>
Maintains client files to include conversations, updates, progress towards goals, and documentation of any incidents, including critical incidents.</li><li>
Report critical incidents immediately to the Clinical Director</li><li>
Provides eligibility screening to applicants and collects and maintains accurate and detailed records.</li><li>
Coordinates and monitors all aspects of service delivery including intake, assessment, creates Individual Housing Plans with clients, treatment planning, budgeting, case conferences, referrals for adjunctive services, family meetings, group work, case management, advocacy, and liaison with other community agencies.</li><li>
Ensures all participants receive appropriate supportive services according to the policies and procedures of the Welcome Home Shelter program.</li><li>
Documents service delivery and maintains accurate and timely clinical documentation and is responsible for consumer confidentiality.</li><li>
Tracks intake, discharge and contact in the HMIS tracking system in a timely and accurate manner.</li><li>
Participates in outreach events targeted towards at risk Veterans and service providers.</li><li>
Transports veterans in a safe, cautious and responsible manner.</li><li>
Participates in all appropriate staff, supervision, and training meetings.</li><li>
Work a minimum of 40 hours/week</li><li>
Assumes other duties as assigned by Senior Management.</li><li>
Collects data necessary to meet funding requirements and statistical reports</li><li>

Minimum Qualifications:
<ul><li>
Applicants must possess a 4-year degree in Social work, psychology, sociology, human services, or a related field. In lieu of a 4-year degree, an applicant can substitute direct practice working with individuals experiencing homelessness, serious mental health conditions, and/or addiction issues. A minimum of 2 years of work experience will be required if no degree is held.</li><li>
Experience working with homeless persons or individuals/families with complex needs</li><li>
Mental health/human services experience.</li><li>
Understanding of Trauma Informed Care, Harm Reduction, Solution Focused Therapy and Strengths Based Therapy.</li><li>
Understanding and familiarity with culturally diverse populations.</li><li>
Possess (or be able to obtain within 30 days of hire) a valid class E driver&rsquo;s license, have reliable transportation available and a clean driving record.</li><li>
Must be 21 years of age or older</li><li>

CAMBA&rsquo;s Rising Up Men&rsquo;s Shelter is a 141 bed men&rsquo;s employment shelter located in the Williamsburg section of Brooklyn. This shelter will provide an array of services with a strong focus on connections for residents to achieve stable employment to lead to permanent housing.
Position: Case Manager Supervisor

What The Case Manager Supervisor Does:<ul><li>
Maintain professional relationships with clients and client confidentiality.</li><li>
Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders.</li><li>

This family shelter is funded by New York City Department of Homeless Services and offers families a supportive, structured therapeutic, safe, and drug-free facility. It provides comprehensive case management services to 169 families who are in a transitional residence program for homeless families. The three story facility has rooms of various sizes, common areas and provides on-site security and social services.

What The Director of Social Services Does:

ROLE SUMMARY
The Part-Time Weekend Shelter Lead provides weekend coordination and operational leadership for REST&rsquo;s Essential RESToration Center (ERC), RESToration Transitional Center (RTC), and crisis response services. The ERC is a low-barrier emergency shelter along with the RTC, a mid-barrier transitional center that serves cisgender and transgender women who have experienced the sex trade and/ or sexual exploitation. Guests may stay for 30&ndash;90 days and receive stabilizing support, including access to food, clothing, crisis intervention, housing assistance, and advocacy. ERC and RTC staff also support REST&rsquo;s 24-hour hotline, providing crisis support, information, and referrals, along with emotional support to individuals seeking assistance.This position is responsible for supporting smooth weekend operations by coordinating staff coverage, overseeing weekend scheduling needs, and serving as the primary point of contact for on-call program concerns. The Weekend Lead models trauma-informed, person-centered care while ensuring that shelter and hotline services remain safe, responsive, and consistent with REST&rsquo;s policies and standards.
Central to this role is the ability to build trusting relationships, maintain a safe and welcoming environment for guests and staff, and support effective decision-making during crisis situations. The Weekend Lead provides guidance and support to weekend staff, assists with de-escalation and problem-solving, and helps ensure continuity of care across shifts.
This position reports to the Swing Shift Supervisor and works collaboratively with shelter staff and crisis response team members to maintain safe and effective weekend operations.
Responsibilities

What The Case Manager Does:
The person filling this position is expected, under general supervision, to: (1) immediately formulate a plan and prepare clients for permanent housing placement; (2) engage individuals requiring social service assistance; (3) interview and evaluate clients, formulate Independent Living Plans and goals; and (4) locate and refer to appropriate community resources for clients; including the following:<ul><li>

Case Manager, Atlantic House Men's Shelter

<div>Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports, they need to thrive. We take a comprehensive approach by offering more than 180 integrated programs in: Education &amp; Youth Development, Family Support, Job Training &amp; Employment Support Services, Health, Housing, and Legal Services. CAMBA operates 6 single adult shelters and 6 family shelters and one drop in center. Each shelter offers clients a supportive, structured therapeutic, safe, and drug-free facility. Homeless (often mentally-ill and substance-abusing) men and women are provided with comprehensive services to stabilize their condition in order to successfully transition into to permanent and/or supported housing.
CAMBA&rsquo;s The Kings Inn Family Shelter is funded by New York City Department of Homeless Services and offers families a supportive, structured therapeutic, safe, and drug-free facility. It provides comprehensive case management services to 100 families who are in a transitional residence program for homeless families. The two story facility has rooms of various sizes, common areas and provides on-site security and social services.
Position: Case Manager I
Reports To: Director of Social Services

What The Case Manager I Does:
The person filling this position is expected, under general supervision, to: (1) aid individuals and/or families requiring social service assistance; (2) in collaboration with clients, interview and evaluate clients and formulate independent living plans; and (3) locate and make use of appropriate community resources for clients; including the following:<ul><li>
Maintain professional relationships with clients and client confidentiality.</li><li>
Practice Universal Precautions/Standard Protocol &amp; Procedures.</li><li>
Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.</li><li>
Establish and maintain professional relationships with clients&rsquo; and client confidentiality in order to engage them in case management services.</li><li>
Review all documentation establishing clients' eligibility for program and make file copies.</li><li>
Create and maintain client files.</li><li>
Input client data and client progress information into CARES database.</li><li>
Conduct initial intake and psycho-social assessment of clients and clients' families' needs and periodic re-assessments.</li><li>
In collaboration with clients, prepare initial and periodic revisions of independent living plans including short-term and long-term client goals.</li><li>
Assist clients in attaining their goals by identifying and locating community resources for clients.</li><li>
Refer clients to permanent housing opportunities and appropriate services both within and outside CAMBA.</li><li>
Schedule appointments for clients with referral organizations.</li><li>
Escort clients to appointments (i.e.-housing, entitlements, educational, medical, social service, etc.).</li><li>
Assist clients in completing applications for benefits and entitlements, and/or process applications on clients' behalf.</li><li>
Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency.</li><li>
Recommend and implement strategies to persuade clients to participate more fully in this process.</li><li>
Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via progress notes.</li><li>
Follow-up with clients and with referral organizations regarding client contact and progress with referral organization.</li><li>
Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.</li><li>
Provide all required information for weekly/monthly/quarterly/annual reports.</li><li>
May recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area).</li><li>
May follow-up with clients for a period of time after successful completion of their primary goals to assure client stability.</li><li>
May complete Relocation Case Review (RCS) for referral for aftercare services.</li><li>
May prepare marketing materials for the program.</li><li>
May prescreen clients over the telephone for eligibility and may schedule intake appointments.</li><li>
Tasks may be modified, expanded and/or assigned over time.</li></ul>
Education, Experience, Licenses, and/or Certifications Required:<ul><li>
Bachelor's degree (e. g., B.A., B.S.W.) or</li><li>
Four years of relevant experience, or</li><li>
Associate&rsquo;s and two years equivalent experience.</li></ul>
