CAMBA&rsquo;s Rising Up Men&rsquo;s Shelter is a 141 bed men&rsquo;s employment shelter located in the Williamsburg section of Brooklyn. This shelter will provide an array of services with a strong focus on connections for residents to achieve stable employment to lead to permanent housing.
Position: Case Manager Supervisor
Reports To: Director of Social Services

What The Case Manager Supervisor Does:<ul><li>
Maintain professional relationships with clients and client confidentiality.</li><li>
Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders.</li><li>
Plan and organize program activities to maximize program contract's goals and performance targets as outlined in DHS&rsquo; Performance Incentive Plan.</li><li>

Assist with the development of program content to address residents needs related to housing, substance abuse treatment, mental health and employment.</li><li>
Provide assistance, interventions and follow-up in regards to critical incidents with clients.</li><li>
Encourage teamwork among direct reporting staff as well as among peers.</li><li>
Troubleshoot client and direct reporting staff program problems and make decisions in accordance with program policies, procedures, and protocols.</li><li>
Conduct and document one-on-on supervision of direct reporting staff to assess and improve work performance i.e. identify training needs, professional development etc.</li><li>
Overcome resistance to change from clients, direct reporting staff, and supervisors/funders.</li><li>
Administer constructive discipline to direct reporting staff, as needed.</li><li>
Monitor clients' progress toward their goals (dates achieved) and document via progress notes.</li><li>
Review all documentation related to clients' progress for accuracy, completeness, and clarity.</li><li>
Review and approve time sheets.</li><li>
Prepare performance appraisals for directed reporting staff.</li><li>
Conduct chart auditing of client files (i.e., paper and electronically).</li><li>
Ensure staff utilization of electronic database and conduct oversight to ensure data integrity.</li><li>

Program Description
Brooklyn Community Services (BCS) operates family shelters providing safe, supportive, and dignified transitional housing for families with children experiencing homelessness. Our shelter programs are guided by a trauma-informed, strengths-based philosophy and offer a comprehensive continuum of services&mdash;including case management, housing placement, recreation, and supportive programming&mdash;to help families achieve stability and permanent housing as quickly as possible. All shelter staff are expected to uphold BCS's commitment to equity, cultural humility, and client-centered care.
Position Overview
The Social Worker provides clinical social work services to families residing in the shelter, offering psychosocial assessments, crisis intervention, mental health and domestic violence screening, and brief therapeutic support. Clinical services are provided under the LCSW supervision of the Director of Social Services, in accordance with NYS Education Law Article 154. Working in close collaboration with case managers and the Director of Social Services, the Social Worker addresses the clinical and behavioral health dimensions of family functioning and ensures families are connected to appropriate specialized services, while maintaining the confidentiality standards required under NYC Local Law 35 (2023).
Key Responsibilities
Clinical Assessment and Intervention
<ul><li>Conduct biopsychosocial assessments for newly admitted families and ongoing clinical assessments as needs emerge, under LCSW supervision.

</li><li>Facilitate brief individual and family counseling sessions as clinically indicated, under the supervision of the Director of Social Services (LCSW).

<ul><li>Maintain strict confidentiality of all information shared during the provision of mental health services; information from clinical sessions may not be disclosed to shelter staff, DHS, or contractors without the resident's express written consent.
</li><li>Apply mandatory reporting exceptions to confidentiality as required by Social Services Law Title 6 and Mental Hygiene Law &sect;9.46, and document such disclosures appropriately.
</li><li>Maintain clinical records separately from shelter case management records to protect resident privacy.
</li><li>Contribute to LL35 annual reporting data, including types of services provided and caseload information.
</li></ul>Care Coordination and Referrals
<ul><li>Develop clinical referral plans and connect families to mental health treatment, substance use services, domestic violence programs, and other specialized resources.
</li><li>Maintain collaborative relationships with community-based mental health providers, hospitals, and behavioral health systems.
</li><li>Consult with and provide clinical guidance to case managers on complex cases involving mental health, DV, trauma, or child welfare, within applicable confidentiality requirements.
</li><li>Participate in interdisciplinary case conferences, critical incident debriefs, and team meetings.
</li><li>Coordinate with case managers on integrated service planning that addresses both housing and clinical goals.
</li></ul>Documentation and Compliance
<ul><li>Maintain complete, accurate, and timely clinical documentation in CARES and BCS systems in accordance with DHS standards and LL35 confidentiality requirements.
</li><li>Participate in regular clinical supervision with the Director of Social Services (LCSW).
</li><li>Adhere to all applicable professional ethics standards, NYS licensing requirements, and BCS policies.
</li><li>Participate in ongoing professional development and required training.

</li></ul>Qualifications
<ul><li>Education: Master's degree in Social Work (MSW) required.
</li><li>Licensure: LMSW required; LCSW preferred. Clinical services provided by an LMSW in this role are performed under the supervision of the Director of Social Services (LCSW), consistent with NYS Education Law Article 154. An LCSW in this role would satisfy an additional LL35 mental health professional unit for the shelter.
</li><li>Experience: Minimum of two years of post-graduate clinical social work experience; experience in shelter, housing, family services, or behavioral health strongly preferred.
</li><li>Knowledge: Working knowledge of trauma-informed practice, domestic violence dynamics, child welfare, mental health, NYC community-based resources, and NYC Local Law 35 (2023) confidentiality requirements. Familiarity with DHS shelter standards and CARES system a plus.
</li><li>Skills: Strong clinical assessment, crisis intervention, and documentation skills. Ability to collaborate effectively in a multidisciplinary team environment while maintaining appropriate clinical confidentiality boundaries.

 Park Slope Shelter for Women serves 100 MICA (mentally ill chemical abusing) women at the Park Slope Armory in Brooklyn.
Position: Case Manager
Reports To: Supervisor II (Clinical)
Location: Brooklyn, NY
What The Case Manager Does: 
 
The person filling this position is expected, under general supervision, to: (1) immediately formulate a plan and prepare clients for permanent housing placement; (2) engage individuals requiring social service assistance; (3) interview and evaluate clients, formulate Independent Living Plans and goals; and (4) locate and refer to appropriate community resources for clients; including the following:<ul><li>
Maintain professional relationships with clients and client confidentiality.</li><li>
Practice Universal Precautions/Standard Protocol &amp; Procedures.</li><li>
Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.
</li><li>Conduct initial intake and assessment of clients&rsquo; needs and periodic re-assessments.</li><li>
Create and maintain client electronic and paper files.</li><li>
Prepare initial psychosocial evaluation and update according to regulations.
</li><li>Obtain and maintain relevant documentation and release forms from clients (i.e., birth certificate, proof of income, medical documents, release of Personal Health Information, etc.).</li><li>
In collaboration with clients, conduct periodic revisions of independent living plans including short-term and long-term client goals.</li><li>
Assist clients in attaining their goals by identifying community resources and by referring clients to appropriate services both within and outside CAMBA (i.e., On-site medical services and psychiatric services, etc.).</li><li>
Work with clients to break through barriers to their goal(s) attainment.
</li><li>Provide support, training and coordination with the other social services staff.</li><li>
Assist clients in advocating for themselves in order to gain housing stability.</li><li>
Recommend and implement strategies to persuade clients to participate fully in the process in order to transition into permanent housing.
</li><li>Monitor clients' progress toward their goals via regularly scheduled face-to-face contacts/sessions.</li><li>
Document all client encounters via progress notes using DHS CARES Database and printout notes for client&rsquo;s paper file.
</li><li>Ensure CARES Database is updated to reflect client request for services (i.e., car fare, appointments, passes, referrals, etc.).</li><li>
Follow-up with clients and with referral organizations regarding client contact and progress with referral organization.</li><li>
Provide all required information for weekly/monthly/quarterly/annual reports.</li><li>
May act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.</li><li>
May schedule appointments for client with referral organizations.</li><li>
May escort clients to appointments (educational, medical, social service, etc.)</li><li>
May assist clients in completing applications for benefits and entitlements, and may process applications on clients' behalf.
</li><li>May follow-up with clients for a period of time after successful completion of their primary goals to assure client stability.</li><li>
May prepare marketing materials for the program.</li><li>
Tasks may be modified, expanded and/or assigned over a period of time.</li></ul>
Education, Licenses and/or Certifications Required:
