Clinical Care

<li>Perform comprehensive nursing assessments and ongoing evaluations of resident conditions

Position Summary</h2>
Bakersfield Post Acute is seeking compassionate and skilled Licensed Vocational Nurses (LVNs) for all shifts. The primary purpose of this position is to provide direct nursing care to residents and supervise the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs).
The LVN ensures care is delivered in accordance with federal, state, and local regulations, facility policies, and direction from the Director of Nursing or Nurse Supervisor to maintain the highest level of quality resident care.<h2 class="jobSectionHeader">

JOB SUMMARY:

RESPONSIBILITIES:

<li>Conduct comprehensive assessments of residents upon admission, change in condition, and periodically as required.</li>
<li>Participate in care plan meetings with residents, families, and the care team to discuss goals, progress, and necessary adjustments.</li>
<li>Provide direct nursing care to residents, including administering medications, treatments, wound care, and other necessary interventions.</li>
<li>Monitor and document residents' health status, including vital signs, symptoms, and responses to treatment.</li>
<li>Assist with activities of daily living (ADLs) as needed, such as bathing, dressing, grooming, and feeding.</li>
<li>Ensure all care provided is consistent with the resident's care plan and nursing standards of practice.</li>
<li>Prepare, administer, and document prescribed medications and treatments accurately and safely.</li>
<li>Monitor residents for adverse reactions and medication effectiveness, and report concerns.</li>
<li>Maintain accurate and up-to-date medication administration records (MARs) and ensure proper storage and handling of medications.</li>
<li>Serve as a liaison between residents, families, physicians, and other healthcare professionals to ensure effective communication and continuity of care.</li>

REQUIREMENTS:
