Position Purpose</p>
<p>
The Case Manager performs all the duties of the Social Service Assistant&rsquo;s and provides Case Management services for men, women, and families enrolled in the Transitional Housing Program and/or in the ASSETS program. Case management services include screening/intakes, goal planning, appropriate referrals, regular meetings with clients, facilitation of life skills and other groups, and implementation of policies and procedures for record keeping.</p><p></p><p><br>

Qualifications: Bachelor's Degree in HDFS, Psychology, Social Work, or a related field. Must have knowledge of issues concerning the homeless population and have experience working with low-income and homeless individuals and families. Knowledge around issues of Domestic Violence, Mental Health, and Alcohol and Drug Abuse are highly desirable. Must have excellent abilities to collaborate and maintain positive relationship with other providers. Good organizational and communication skills are a must.</p><p></p><p><br>
