The Health Home program is an innovative new model of care management focused on improving the health outcomes for individuals with severe mental illness &amp; other complex chronic illnesses through coordinated networks of medical, psychiatric, behavioral health, social service &amp; housing providers. Staff will use cutting-edge health information technology &amp; a unified plan of care to ensure all clients connect to &amp; maintain primary medical care, prevent hospital stays, &amp;/or reduce the length of hospitalizations. Essential Duties and Responsibilities: The person filling this position is expected, under general supervision, to: (1) aid individuals and/or families requiring social service assistance; (2) interview and evaluate applicants for services, formulate service plans and goals, and aid clients to implement service plans; (3) locate and make use of appropriate community resources for clients; (4) evaluate actual living conditions of clients though home visits; including the following: 
<ul><li>Maintain professional relationships with clients and client confidentiality.</li><li>Practice Universal Precautions/Standard Protocol &amp; Procedures.</li><li>Comply with any and all Federal, State, City and company security and privacy polices intended to protect the security and privacy of individually identifiable health information.</li><li>Review all documentation establishing clients' eligibility for program and make file copies (e.g. Medicaid status and confirmation of HIV status).</li><li>Assist clients in completing all intake applications and forms.</li><li>Create and maintain client files.</li><li>Carry a caseload of approximately 50 clients.</li><li>Conduct initial intake or assessment of clients and clients' families and/or periodic reassessments.</li><li>Conduct case conferences prior to finalizing all assessments and reassessments.</li><li>Follow-up with clients and with referral organizations regarding client contact and progress with referral organization.</li><li>Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency.</li><li>Recommend and implement strategies to persuade clients to participate more fully in this process.</li><li>Monitor clients' progress toward their service plans goals via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via service plan outcomes and detailed progress notes (i.e. time of service, type of service, etc.).</li><li>Recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area, change in Medicaid status).</li><li>Provide all required information for weekly/monthly/quarterly/annual reports (e.g. # of clients in managed care programs, # of clients housed, etc).</li><li>Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, entitlements, etc.</li><li>Escort clients to appointments (educational, medical, social service, etc.).</li><li>Follow-up with clients for a period of time after successful completion of their primary goals to assure client stability.</li><li>May plan, coordinate and facilitate social/peer support events, including group facilitation for clients.</li><li>May prepare marketing materials for the program.</li><li>May input client data and client progress information into automated database.</li><li>May reach out and market the program to the community in order to recruit clients.</li><li>May prescreen clients over the telephone for eligibility and may schedule intake appointments.</li><li>May assist clients in completing applications for benefits and entitlements, and may process applications on clients' behalf.</li><li>Tasks may be modified, expanded and/or assigned over a period of time.

Requirements:
 
<ul><li>Associate's Degree and 4 years applicable experience OR Bachelor's degree (B.A. or B.S.W.) and two years of applicable experience</li><li>Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues.</li><li>Bi-lingual English with any one of the following languages: Spanish, Russian, Haitian-Creole, Chinese (Mandarin or Mandarin/Cantonese) preferred.</li><li>Some evening/weekend availability may be required.</li><li>Ability to work in the field depending on needs in the client

Individuals and families receive services that will help overcome problems with public benefits, housing, education, health care and other issues that could impede their ability to maintain stable households.
 
 
 
 Position Overview
 
 
 
 The Case Manager, under general supervision, is expected to: (1) aid individuals and/or families requiring social service assistance; (2) conduct client interviews and evaluations to assess needs and develop service plans, including short-term and long-term client goals; and (3) assist clients in identifying and locating community resources.
 
 
 <ul>

 Prescreen clients for eligibility and schedule intake appointments.
 </li>

 Review all documentation establishing clients' eligibility, conduct initial intake or assessment of clients and clients' families, and conduct periodic reassessments.

Document client progress via detailed progress notes.

 Evaluate and recommend closing of cases in which clients: (a) have achieved stability, (b) have not demonstrated a willingness to participate (lost-to-service), or (c) have reached the allotted time of services 120 EAF/90 ESG without recertification or has become ineligible for services (e.g. exceed income guidelines).

 Meet daily, weekly, and monthly goals and tasks as assigned and in accordance with the funder guidelines.

ESSENTIAL JOB FUNCTIONS:

The Sheltering Case Manager will work with moderate to high needs clients to support and guide them through an extended level of assistance in order to remove barriers and facilitate stable housing. The Supportive Services for Veteran Families (SSVF) SSVF Case Manager will be responsible for the case management of a portfolio of Veteran families who have experienced or are at risk of homelessness. The SSVF Case Manager will work closely with each family on their caseload to help them achieve long-term housing stability in the shortest time possible. The SSVF Case Manager will connect the family with SSVF temporary financial assistance and help the family to obtain and utilize other available resources and benefits. The SSVF Case Manager is also responsible for developing and implementing a client-defined, goal-oriented Housing Stability Plan for each family on their caseload.

ABOUT OUR PROGRAM/DEPARTMENT</div> 

Supportive Services for Veteran Families (SSVF) is a case management program for homeless, or at-risk for homelessness, Veterans and their families. SSVF Case Managers will assist you with identifying and addressing barriers, increasing self-sufficiency, and securing permanent housing. We work closely with the VA and other community service providers to ensure that our clients are connected with any and all resources available to them.
We serve Veterans in the Anchorage Municipality and in the Matanuska-Susitna Borough.<div>

 RESPONSIBILITIES</div> 

Maintains a caseload of households in the SSVF Program. Works with households to assess housing needs, and provides case management and financial assistance to address those needs. Assists clients to secure housing if they are experiencing homelessness.
Utilizes Housing First and Motivational Interviewing approaches to engage and work with Veteran households. In collaboration with Veteran households, develops Housing Stability Plans, and connects households with additional VA and community resources to meet their identified needs in the shortest time possible. Assists clients in accessing and maintaining entitlements and benefits.
Establishes and maintains collaborative working relationship with community resources; attends and participates in appropriate community resource meetings and events.
Participates in street and shelter outreach to identify homeless Veterans and present the SSVF Program as an alternative to homelessness.
Completes thorough intakes and eligibility screenings with referred clients.
Enters client data and case notes into the Homeless Management Information System (HMIS).
Maintains accurate and complete records for each household&rsquo;s case management file. Completes eligibility and recertification requirements in a timely manner.
Adheres to policies and procedures outlined in the SSVF Program Manual.
Travels independently in Anchorage as needed; must be able to transport persons in their own vehicle. Conducts home visits to support clients in achieving housing stabilization.

Case Manager II
Minimum Education Qualification: High School Diploma or GED.
Minimum Experience Qualification: One (1) year as a CSS Case Manager I.
Case Manager III
Minimum Education Qualification: Bachelor&rsquo;s degree in social work, human services, social services, addiction, counseling, psychiatric nursing, or psychology. Four (4) years of directly applicable experience may be substituted for the required education.
Minimum Experience Qualification: Four (4) years experience equivalent to a CSS Case Manager.
Minimum Training Qualification: Counselor Technician Certification, Chemical Dependency Counselor I Certification, or Chemical Dependency Counselor II Certification.
A relevant combination of education and experience may be considered.
Case Manager IV
Minimum Education Qualification: Bachelor&rsquo;s degree in social work, human services, social services, addiction, counseling, psychiatric nursing, or psychology.
Minimum Experience Qualification: Two (2) years as a CSS Case Manager III.
Minimum Training Qualification: Counselor Technician Certification, Chemical Dependency Counselor I Certification, or Chemical Dependency Counselor II Certification.
Principles of trauma informed care, stages of change, motivational interviewing, client centered case management, and Housing First. Social Justice Advocacy, collaboration and partnership abilities, problem solving, communication, and customer service skills. Military veterans and their family members are encouraged to apply. The ideal candidate has experience providing case management activities to veterans and/or the homeless population and embraces a Housing First approach to helping homeless veterans individuals and families stabilize and return to permanent and traditional housing.<div>
WORK ENVIRONMENT</div> 

Physical demands: Work is performed in a standard indoor office environment, with field work within the MATSU area also frequently required. Occasional lifting of up to 25 pounds. Frequently sitting or standing at a desk for several hours at a time.
Travel: Traveling out of Alaska is rare. Occasional travel within Anchorage is required. Frequent travel within Wasilla is required.

 About the Role:
 
 
 
 
 The Case Manager is a great role at Woodland! The Case Manager is responsible for serving as the primary liaison between our adult patients with substance use disorders and outside stakeholders. The Case Manager regularly meets with the patients in their caseload to understand and resolve any barriers to treatment related to family, employment, legal, or financial issues. The Case Manager identifies and evaluates aftercare options to help set patients up for long-term success as well as facilitates and participates in multidisciplinary treatment planning. With a 360-degree view of the patient&rsquo;s world, the Case Manager is a key advocate, communicator, collaborator, and coordinator.
 
 
 
 The Case Manager must develop strong therapeutic relationships, communicate professionally with various stakeholders, maintain meticulous documentation, and remain organized and efficient to have an enduring positive impact on the lives of our patients struggling with addiction.
 
 
 
 
 Minimum Qualifications:

 Bachelor&rsquo;s degree in Social Work, Human Services, Psychology, or a related field.

 Minimum of two years of experience in case management or social services.
