JOB SUMMARY</p><p>
Full-time Social Services Director needed for our skilled nursing facility in northern Illinois. A four year college degree in a Human Services field of study, social work, psychology, sociology, counseling, etc preferred. The Social Service Director must be able to present themselves well in group settings and individual 1:1 sessions. An excellent command of the English language for both verbal and written skills is required.</p><p>
The ability to multi-task is very important. Social Service Director candidates must possess strong leadership and time management skills and must have a strong desire to work with the variety and diversity of populations found in long and short term care facilities. Candidates with prior experience in long term care will be given preference. Salary commensurate with experience.</p><p>
The Social Service Director will work a full-time schedule 8 hours daily with some weekends and some evening hours. The Social Service Director will participate in the Manager on Duty rotation. The Social Service Director must be able to accept and utilize professional direction, consultation and work harmoniously with their peers.</p><p></p><p><br>

ESSENTIAL DUTIES AND RESPONSIBILITIES:</b></p><p>
Director of Social Services provides Social Services for chronically mental ill homeless single women. Residence operates under contract to the New York City Department of Homeless Services (DHS) and as a contract agency, has reporting responsibilities to DHS and is subject to inspections and audits by DHS, the New York State Office of Temporary and Disability Assistance and the Coalition for the Homeless of New York City.</p><p>
</p><p>Essential Duties and Responsibilities:</p><ul><li>Provide ongoing supervision to the Social Services Staff.</li><li>Supervise the Case Management Services for all clients.</li><li>Provide coverage for cases if requested by the Director.</li><li>Respond to DHS inquires.</li><li>Provide overall coverage in the absence of the Director.</li><li>Meet all performance expectations established for the shelter by Care for the Homeless.</li><li>Ensure that the program meets the contractual obligations and reporting requirements of or the Department of Homeless Services. This family shelter is funded by New York City Department of Homeless Services and offers families a supportive, structured therapeutic, safe, and drug-free facility. It provides comprehensive case management services to 169 families who are in a transitional residence program for homeless families. The three story facility has rooms of various sizes, common areas and provides on-site security and social services.</p>

What The Director of Social Services Does:</b><br>

1. Direct Service</div>
<div>
a. Provide socialization/stimulation to residents in the facility.</div>
<div>
b. Monitor or assist with facility smoking programs.</div>
<div>
c. Conduct socialization/psychosocial groups as directed by supervisor.</div>
<div>
d. Assist CNA/Nursing staff at meal time with passing trays and clean up.</div>
<div>
e. Assist with crisis intervention as needed.</div>
<div>
f. Assist residents going into the community who require supervised access.</div>
<div>
g. Assist residents who may require 1:1 sessions and/or other interventions.</div>
<div>
h. Provide 1:1 sessions under the direction of the Social Service Director/PRSD/PRSC to promote self-maintenance, socialization, group attendance, etc.</div>
<div>
i. Assist residents when going to appointments outside the facility.</div>
<div>
j. Assist with all ancillary appointments in the facility: dentist, podiatrist, audiologist and psychologist.</div>
<div>
k. Assist residents with the management of personal items.</div>
<div>
l. Assist with contraband management, including room searches.</div><br>

<div>
2. Record Keeping</div>
<div>
a. Planning Groups/Individual Sessions.</div>
<div>
b. Maintain accurate records, including documentation of 1:1 sessions at a minimum of one time per month or more frequently as indicated and group session attendance/summaries.</div>
<div>
c. Conduct groups/activities as components of the facility&rsquo;s psychiatric rehabilitation program around: self-maintenance, social functioning, community living, occupational skills, substance abuse interventions and symptom management.</div>
<div>
d. Assist with setting up for group activities and ancillary service programs, as well as encourage resident participation.</div>
<div>
e. Participate in treatment, program planning and in-service trainings.</div><br>

3. Facility Training and Activities:</div>
<div>
a. Participate in treatment, program planning and in-service training.</div><br>

<div>
4. Miscellaneous:</div>
<div>
a. Ensure that all elements of the facility&rsquo;s Abuse and Neglect policies are maintained and enforced.</div>
<div>
b. Ensures HIPAA compliance is maintained at all times.</div>
<div>
c. Possess ability to make independent decisions when circumstances warrant such action.</div><br>

Required/Preferred Education &amp; Experience:</i></b></p>
<div>
1. High School diploma or GED equivalent required. CNA certification preferred.</div>
<div>
2. Minimum of 1 year experience in a skilled/long term care facility or a mental health facility preferred.</div>

3. Bachelor Degree in Psychology or Social Service</div><br>
