JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) provides intensive case management services to eligible clients, meeting participants at least weekly. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. Each Case Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available.
DUTIES AND RESPONSIBILITIES

The Supportive Services for Veteran Families (SSVF) SSVF Case Manager will be responsible for the case management of a portfolio of Veteran families who have experienced or are at risk of homelessness. The SSVF Case Manager will work closely with each family on their caseload to help them achieve long-term housing stability in the shortest time possible. The SSVF Case Manager will connect the family with SSVF temporary financial assistance and help the family to obtain and utilize other available resources and benefits. Maintains a caseload of households in the SSVF Program. Works with households to assess housing needs, and provides case management and financial assistance to address those needs. Assists clients to secure housing if they are experiencing homelessness.
Utilizes Housing First and Motivational Interviewing approaches to engage and work with Veteran households. In collaboration with Veteran households, develops Housing Stability Plans, and connects households with additional VA and community resources to meet their identified needs in the shortest time possible. Assists clients in accessing and maintaining entitlements and benefits.
Establishes and maintains collaborative working relationship with community resources; attends and participates in appropriate community resource meetings and events.
Participates in street and shelter outreach to identify homeless Veterans and present the SSVF Program as an alternative to homelessness.
Completes thorough intakes and eligibility screenings with referred clients.
Enters client data and case notes into the Homeless Management Information System (HMIS).
Maintains accurate and complete records for each household&rsquo;s case management file. Completes eligibility and recertification requirements in a timely manner.
Adheres to policies and procedures outlined in the SSVF Program Manual.
Travels independently in Anchorage as needed; must be able to transport persons in their own vehicle. Conducts home visits to support clients in achieving housing stabilization.

General Description
Provides professional social services to seniors and adults with disabilities. Responsible for assessing needs, developing and monitoring service plans, and coordinating resources to promote independence, dignity, and choice.

Essential Functions

<li>Conduct assessments to determine eligibility and service needs using approved state tools (e.g., Oregon ACCESS, CAPS).</li>
<li>Develop, implement, and update person-centered care plans in accordance with regulations and agency policy.</li>
<li>Coordinate services such as in-home care, residential placements, transportation, and medical or social supports.</li>
<li>Monitor consumer progress, reassess needs, and adjust service plans as needed.</li>
<li>Maintain accurate and timely case documentation and reporting.</li>
<li>Collaborate with internal teams and community partners to support consumers&rsquo; wellbeing.</li>
<li>Provide information, referrals, and technical assistance to consumers, families, and providers.</li>

Minimum Qualifications - Experience and Education

A qualified applicant will have a minimum of four (4) years of equivalent combination of education and/or experience which demonstrates the knowledge, skills and abilities required. Any combination of the following qualifications will be considered:

<li>Bachelors degree in social sciences or any related field</li>
<li>Experience working directly with consumers in social service type settings.</li></ul>

 Special Requirements
<ul>
<li>Background Check. All positions must pass a basic criminal history check. Conviction of a crime may not necessarily prohibit employment. Some may require additional clearance, including but not limited to national or state fingerprinting.</li>
<li>Driving for Agency Business. Applicants must possess a current driver's license in their state of residence and acceptable driving history. May require proof of personal automobile liability insurance [ORS Chapter 806] and use of a personal vehicle for business use.</li></ul>

Work Environment and Physical Demands

Case Managers work in both office and community settings to meet the needs of consumers. The position involves a combination of desk-based tasks, in-person visits, and collaboration with community partners. Work is performed in an office environment as well as in the community, including consumers&rsquo; homes, care facilities, and partner agency sites. The noise level is typically moderate and consistent with a standard office environment. The position requires regular travel within the service area using an agency or personal vehicle. Work may be subject to frequent interruptions and occasional exposure to emotional or crisis situations. Contact with the public in homes, facilities, or community settings may involve exposure to contagious illnesses, domestic animals, or individuals experiencing behavioral or emotional distress.

Case Managers are regularly required to sit, stand, walk, reach, bend, talk, and hear. The position involves frequent use of hands for computer work, documentation, and telephone communication. Occasional lifting or carrying of materials or equipment weighing up to 20 pounds may be required. The position also requires the ability to perform tasks involving fine motor skills and sustained attention. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position.

What you'll do as a Case Manager for Hep C:<ul><li>
Facilitate referral to care and system navigation for SUD, HCV, HIM, SMI, and social services to address Social Determinants of Health (SDOH)</li><li>
Design, develop, implement, and supervise case management, after care, and follow-up policies and procedures for the program</li><li>
Provide specialized case management and aftercare services to clients and conduct follow-ups as scheduled</li><li>
May supervise clients in the community, carry a caseload of active clients, conduct group and individual counseling sessions, assist clients in achieving adjustments to specific problems or situations of a difficult or complex nature</li><li>
Monitor emotional/psychological status of clients and arrange referrals to ancillary services.</li><li>
Conduct workshops and seminars for clients and the community and maintain relations with referral sources and Hep C, health, addiction, social, educational, recreational, and vocational organizations for in-treatment and aftercare planning</li><li>
Write reports, maintain necessary records, and assist other staff in discharge and aftercare planning.</li><li>
Develop and implement procedures to complete clinical records and correspondence while maintaining compliance with confidentiality procedures and applicable state/federal regulations.</li><li>
