<div>This full-time APP role supports the Transitional Care and Remote Patient Monitoring Program through scheduled and on-demand telehealth visits, panel management, and clinical escalation support for nursing and care coordination teams. Providers care for patients remotely while working onsite alongside an interdisciplinary team to promote collaboration and continuity of care. The position follows a Monday&ndash;Friday schedule consisting of five 8-hour shifts with no call or weekend requirements. Education &amp; Certification:
<ul><li>Master&rsquo;s Degree in Nursing (MSN) or equivalent education in Physician Assistant or Nurse Midwifery programs.
</li><li>Minimum of three years experience as a Nurse Practitioner, Physician assistant or Nurse Midwife preferred.
</li><li>If you are a new graduate applying for a role in Nashville or Knoxville, you will be required to commit to six (6 months) of training at our main Health Center in Memphis, and pass your probationary and training period to be transitioned to your location of choice.
</li><li>Active and unrestricted Tennessee Nursing License.
</li><li>National Certification as a Nurse Practitioner, Physician Assistant, or Certified Nurse Midwife.
</li><li>CPR Certification; BLS. ACLS certification preferred.
</li><li>DEA Licensure required.
