Case Manager-RedirectJob descriptionThe Redirect addiction-related diversion program is a street-based, long-term, and unconventional approach to case management. Redirect case work is non-coercive, non-time delimited, street-based, and built on multiple pathways to recovery, including harm reduction, and steeped in Stages of Change theory and motivational interviewing practices.This position will manage a caseload of approximately 15-25 participants and will work in conjunction with local police departments and FAN&rsquo;s Comeback Quick Response Team (QRT). Our team works to identify individuals impacted by substance use disorder (SUD) and provide a continuum of care to ensure participants get the treatment, support, and ongoing connection they need for their recovery journey.Duties will include:<ul><li>Meeting participants where they're at, both throughout the community and in their recovery process (in-home case management).</li><li>Transporting participants.</li><li>Promoting the development of a comprehensive service delivery system by identifying participant needs and gaps in services to meet those needs better.</li><li>Collaborating with FAN&rsquo;S Quick Response Team while carrying out duties of case manager.</li><li>Providing assessments for participants within the program and meeting with participants shortly after identified (within 24 hours).</li><li>Conducting initial assessments of the client&rsquo;s needs, desires, strengths, and barriers.</li><li>Together with the client, preparing the necessary action steps for the client and case manager to achieve established goals.</li><li>Setting up regular follow-up meetings with the client, based on the client&rsquo;s needs for intensity &ndash; but no less frequently than once per week.</li><li>Assisting participant in accessing services, resources, and supportive benefits, as needed.</li><li>Intensive case management, harm reduction strategies, identifying proper treatment, and following-up across 12 months with each participant.</li><li>Following up, recommending exit, or reevaluating plan, as needed.</li><li>Attending and participating in case review meetings with Case Management Supervisor at FAN.</li><li>Tracking and developing appropriate data measures for program evaluation on electronic management system.</li><li>Supporting and effectively communicating with team members, including employees and contractors.</li><li>Staying knowledgeable about all FAN programs and national trends.</li><li>Representing FAN in a professional manner while performing duties.</li><li>Other duties as assigned.</li></ul>Qualifications:<ul><li>Bachelor&rsquo;s degree in the human services or related field, with a proven track record of successfully working with diverse populations. Provide case management with crisis intervention, discharge plan, group &amp; individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.</li><li>Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.</li><li>Facilitate placement including pre-placement visits, and transportation as needed.</li><li>Shift coverage including part of the on-call team.</li><li>Provide field-based services as required by program and client need.</li><li>Maintain confidentiality of client&rsquo;s care.</li><li>Participate in scheduled program meetings.</li><li>Prepares and maintains client&rsquo;s case management goals and facilitates monthly housing meetings.</li><li>Maintains daily log of client contacts. Records data input of client&rsquo;s demographic information</li><li>Works closely with program direct service staff to coordinate appropriate housing options for clients relative to functioning level and individual needs and preferences</li><li>Provides outreach in the community and agencies and attends meetings to provide program education</li><li>Other relevant duties as assigned.</li></ul> 
QUALIFICATIONS<ul><li>For Housing two years of experience and an AA degree in social service or related field or High School Diploma and 5 years&rsquo; experience.</li><li>Valid CA driver&rsquo;s license and current auto insurance per contract requirements required.</li><li>Experience providing mental health and/or substance use disorder services preferred.</li><li>Cultural competency understanding required.</li><li>Bilingual preferred.</li><li>Knowledge of Microsoft Office and EHR software preferred.</li><li>Per program contact requirements, must satisfactorily complete a Tuberculosis test prior to hire and once annually thereafter.</li><li>CPR/First Aid certification (preferred), and bi-annual thereafter.</li></ul>
