Position Summary
 Intercollegiate Athletics has long been an integral part of Sacred Heart University life.  Principal Duties & Responsibilities
 In collaboration with the Associate Athletics Director for Academic Services, the individual will assist in developing, implementing, and monitoring all student-athletes progress in relation to all five prongs of the NCAA Life Skills Program (Academics, Athletics, Personal Development, Career Development, and Service). Key goals are to increase outreach and retain student-athletes in coordination with other support services campus-wide, such as the Learning Center, Academic Advisement, The Counseling Center, Campus Ministry, Residential Life, and Student Life.
 Administration
 <ul>
<li>Working to assist student-athletes by referring and collaborating with established University-wide support services, faculty, staff, and departments.</li>
<li>Participate in meetings with the Associate Athletics Director for Academic Services, the Deputy Athletic Director/SWA, the University Registrar, and the Faculty Athletic Representative to certify the academic eligibility of all student-athletes.</li>
<li>Update policy and procedures manual on an as-needed basis.</li>
<li>Help prepare and report on semester and annual grade reports of team GPAs and general student-athlete academic progress and standings.</li>
<li>Collaborate with Athletic Training and serve as the primary liaison for H.E.A.R.T Club (Helping Every Athlete Return Together). Conduct a minimum of two (2) meetings per semester.</li>
<li>Act as a Supervisor for all SAAc ( Student-Athlete Advisory Committee)- related activities. This includes but is not limited to, working with the Executive Board, scheduling monthly meetings, MAAC Conference calls, Community Service Initiatives, the MAAC Building Communities Award, and the Pioneer Cup, and representing SHU during the yearly MAAC SAAC Conference and monthly conference calls.</li>
<li>Distribution of books for those student-athletes on athletic scholarship.</li>
</ul>
Academic Support
 <ul>
<li>Support student-athletes in scheduling athletic and academic commitments, resulting in a successful time-management environment in collaboration with academic advisement.</li>
<li>Assist in organizing and administering student-athlete tutoring programs.</li>
<li>Assist in evaluating the NCAA Life Skills Needs Assessments of individual teams and/or individual athletic cohorts to produce related programming addressing identified issues facing student-athletes.</li>
<li>Report on coordinated community service projects to better the community at large and support the mission and goals of the University in collaboration with Service Learning.</li>

Advising
 <ul>
<li>Assist and monitor student-athlete's degree progress toward successful graduation.</li>
<li>Manage changes in registration, including registration holds, as well as priority registration.</li>
<li>Help to advise student-athletes during registration for classes, and monitoring for compliance with NCAA, MAAC, and University requirements.</li>
<li>Meet with coaching staff and team administrators to share information on student-athletes' academic progress when needed.</li>
<li>Build relationships across campus to promote student-athlete welfare and community integration.</li>
<li>Review and create action plans to identify and for student-athletes at risk, meeting with them one-on-one in coordination with other campus support services (i.e.: Executive Director of Student Success, JLC Academic Support Services, First-Year Experience, and Academic Advisement) and monitor/ report out on a continual basis to assure satisfactory academic progress.</li>

<li>Meet with coaches and key University staff/faculty on a continual basis to promote a collaborative environment to benefit all student-athletes potential successes outside of their sport.</li>
<li>Attend NCAA and MAAC meetings for academic advisors, as required.</li>
<li>Responsible for MACC Yearly Speakers and corresponding coordination of events.</li>
<li>Help to prepare NCAA, MACC and Athletic Department reports as needed.</li>
<li>When possible, attend athletic events to maintain maximum knowledge of each intercollegiate sport relevant to various assignments.</li>
<li>Responsible for adherence to all Department and University policies and procedures, as well as the rules, regulations, bylaws, and interpretations of the NCAA and the MACC.</li>

Care Lync is a Social Services agency focused on providing Support Coordination (Service Coordination/Case Management) services to adults diagnosed with Intellectual, Developmental and Physical Disabilities and Autism Spectrum Disorder throughout Pennsylvania who are enrolled in the Department of Human Services, Medicaid Waiver Program. General Summary
The Support Coordinator / Service Coordinator / Case Manager provides case management activities necessary to meet the needs of clients assigned to the consolidated care team. This position is responsible for the development of individualized care management plans; implementation of the care plan through brokering and coordinating services; and the monitoring and evaluation of all clinical outcomes to ensure that services are provided in a professional, comprehensive, and cost-effective manner. The role assists in the location, coordination and monitoring of needed services and supports through the development and implementation of the Individual Support Plan (ISP). The Support Coordinator / Service Coordinator / Case Manager will assist the person(s) receiving services in identifying and communicating his/her requests and needs for services and supports, which may include housing, medical, social, vocational and other community services.
Essential Duties and Responsibilities
Complete assessments to assist in identifying each participant needs and desires for service delivery.
Develops a Person-Centered Service Plan for each participant and modify at least annually or when there is a significant medical or social change.
Ensure the participant choice of providers by providing information for the participant to make a fully informed decision.
Ensure the Behavioral Support Plan and Crisis Plan (if the participant has a Behavior Specialist) is consistent with the ISP.
Report all incidents of participant abuse, neglect or exploitation immediately to supervisor/director.
Actively coordinate with other individuals and/or entities essential in the physical and/or behavioral care delivery for the participant, including other care coordinators, to ensure seamless coordination between physical, behavioral and support services.
Responsible for researching and developing alternative solutions to participants needs.
Ensures confidentiality regarding sensitive material and private health information of each individual served.
Attends required trainings and recertification classes.
Confers regularly with supervisor/ director(s) to understand and consider factors such as participants needs, abilities, learning levels, and physical limitations.
50% travel to participants as needed
Qualifications
BS/BA degree in related course work
Experience with Microsoft Office, state-maintained databases.
Must maintain proficiency in company sponsored training and certifications.
Maintain CPR/First Aid certification and updated state related Clearances.
Successfully complete and maintain training courses as required or amended by program regulations.
Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration, copies must be maintained on file with Care Lync.
