Perform intake assessment of assigned residents on day of move-in to include a review of health status, psychosocial history, financial status, history of drug and alcohol use, housing history, etc.</li><li>
Outreach to each resident on assigned caseload regularly, as well as in response to incidents or documentation indicating the resident may be in crisis, need behavioral health or medical intervention, or may be jeopardizing their housing (late rent notices, lease violations, etc.). Direct, in-person engagement is always the goal.
</li><li>Work with each resident to develop and implement an individualized Service Plan based on each resident&rsquo;s goals and needs, tailored to their preferences.</li><li>
If unable to offer on-site resources, provide referrals to other in-house providers or outside community resources regarding issues related to substance use, vocational training, legal services, immigration services, physical and mental health services, housing resources, money management, etc.</li><li>
Assess each resident&rsquo;s eligibility for benefits and entitlements and assist residents in applying for and enrolling in these programs.
</li><li>Work with residents regarding budgeting issues, including the development of voluntary money management plans.</li><li>
Advocate on behalf of residents with Property Management regarding issues that may be jeopardizing residents&rsquo; housing (late rent notices, lease violations, etc.). Under the direction of the SSM, work with Property Management to avoid eviction, when possible.

Participate in twice-per-week Wellbeing Checks, and as necessary, subsequent Emergency Safety Checks to ensure the safety and wellbeing of residents.
</li><li>Develop and facilitate at least one weekly therapeutic / psycho-educational group at the site. As directed by the SSM, assist with the organization and carrying-out of other program events and community-building activities, such as Residents&rsquo; Council, BINGO, BBQs, holiday celebrations, community meetings and meals, movie viewings, outings, etc.</li><li>
Maintain up-to-date documentation in resident files regarding services provided in a complete, confidential, and professional manner. Complete all necessary paperwork and administrative requirements, including completing Significant Encounters, outreach letters, Service Plans, recertification, filing, etc., within the required timeframe.
</li><li>Assist residents with planned exits from the program to engage in comprehensive discharge planning, that includes referrals for case management, housing, food, clothing, medical treatment, detox, and/or other services as necessary and appropriate.</li><li>

Provide back-up coverage for other case managers, as directed by the SSM.</li><li>

The Sacramento Fire Department is seeking a highly skilled and detail-oriented Program Specialist to lead Support Services, the operational and logistical foundation of the Department. This role oversees key functional areas such as Fleet, Data Management/IT, and Logistics under the Support Services Division. Additionally, this position is responsible for ensuring seamless support for 24 fire stations and one Chief&rsquo;s Fire Station that function as both workplaces and living quarters for approximately 178 on duty personnel each day; this position requires demonstrated experience in facility oversight. Experience: 
 
Four years of progressively responsible professional-level administrative experience.

As a Prevention Services Manager, you will be responsible for providing leadership and coordination of prevention programming, including mobile health unit outreach, HIV, STI, HCV screening, PrEP services, and condom distribution. Serves as a liaison and provides coordination with other departments of PHC, community partners, and funding and oversight agencies to integrate prevention services across PHC and within the community. What You Will Do<ul><li>
Manages day-to-day functioning of outreach activities with PHC&rsquo;s mobile health unit including scheduling, inventory, policy/procedure changes and quality control, ensuring maintenance and upkeep of mobile unit. Manages and coordinates clinical components of the prevention program including rapid HIV and HCV testing, STI screening, pre-exposure prophylaxis (PrEP) services and condom distribution program.</li><li>
Supervises the program&rsquo;s prevention specialists to facilitate the continuum of care. Indirectly supervises other prevention and outreach related staff.</li><li>
Interfaces with all other PHC departments, clinics, and outside state and local agencies regarding any HIV and STI prevention activities in order to integrate routine HIV and STI testing, PrEP, and other initiatives. Establishes working relationships with community partners, other PHC departments, and funding plus oversight agencies. Participates in appropriate advisory boards, task forces, coalitions, and committees.</li><li>
Facilitates program development, leading program staff in goal setting and implementation based on the project work plan and other identified program objectives. Monitors and evaluate completion of goals, using outcomes to move the team forward. Coordinates quality management efforts for assigned programs by developing and maintaining program policies and procedures, participating in quality assurance activities, chart reviews, program and staff evaluation, staff development trainings, and outcome analysis.</li><li>
