SUMMARY</b></p><p>
The Permanent Supportive Housing Case Manager is an experienced, social services professional responsible for providing high-quality, comprehensive, and intensive case management services to chronically homeless families and individuals residing in the community. The role of the Permanent Supportive Housing Case Manager is to build trusting relationships with program participants which will enable the participants to maintain housing and reach personalized goals. The Case Manager is dedicated to supporting and empowering participants utilizing evidence-based practices, including a Housing First philosophy, Motivational Interviewing, Harm Reduction, and Trauma Informed Care.</p><p>
The Case Manager collaborates with a wide range of service providers with the goal of assisting participants to achieve tenancy stability, increase financial independence, connect to wellness resources, and increase community integration. Supportive services are offered to best meet participant needs specifically in the areas of physical and/or mental health disability, domestic violence, and substance use. The overall goal is to maximize housing stability for the participants, and to put measures in place to prevent participants from returning to homelessness.</p><p><b>
ESSENTIAL DUTIES AND RESPONSIBILITIES </b>include the following:</p><ul><li>
Provide case management services that facilitate client self-sufficiency and housing retention.</li><li>
Ability to build positive and trusting relationships with diverse populations, particularly participants experiencing trauma, homelessness, physical and/or mental health disabilities, and substance use challenges.</li><li>
Use evidence-based practices in service delivery, such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, Housing First, and Person-Centered Planning.</li><li>
Provide field-based case management services in the areas of, but not limited to: independent living skills, community integration, connection to mainstream benefits, employment services, medical and mental health care, substance use treatment, and all other services needed to support participants in reaching their service plan goals.</li><li>
Maintain on-going case load of 16-19 households participating in program by meeting with each participant on a weekly basis.</li><li>
Assist participants in developing an Individualized Service Plan. Actively involve participants in the design and delivery of supportive services by ensuring they have an active voice in their goal/service plans. Refer participants to necessary services, and coordinate with other providers to help participants to access services and achieve goals set in his/her plan. Work toward strengthening each participant&rsquo;s sustainability in permanent housing.</li><li>
Complete a biopsychosocial assessment with each household on caseload.</li><li>
Conduct crisis assessments and provide appropriate interventions.</li><li>
Work in collaboration with property management to help resolve any concerns or issues that may arise with participant to help participant remain stably housed.</li><li>
Maintain necessary documentation, reports and statistics in the Homeless Management Information System (HMIS) database and enter all data within timeframes as set forth by the HMIS Data Quality Plan.</li><li>
Plan and participate in community-building activities. Plan regular times to recognize participant achievements, encourage leadership, and implement other ways to help participants recognize and build on strengths.</li><li>
Learn and model sound conflict resolution techniques for the purposes of mediating conflicts within the dynamic of participant to landlord relationship.</li><li>
Assist with orienting new clients and exiting departing clients, ensuring adequate transition, referrals, and continuity of care.</li><li>
Identifies opportunities for volunteers.</li><li>

QUALIFICATIONS</b></p><p>
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Previous case management experience and working knowledge of local resources and social services available are highly desirable. Excellent time management skills and the ability to effectively communicate both orally and written is essential. A valid Texas Driver&rsquo;s License, a reliable motor vehicle, and liability insurance is also needed. The individual must have a high level of energy, creativity and flexibility.</p><p><b>
EDUCATION and/or EXPERIENCE</b></p><p>
This position requires a Bachelor&rsquo;s Degree. Degrees in social work, human services, or other related fields are preferred, but not required. At least 2 years of case management experience, and experience using HMIS is preferred. Extensive work history with at-risk populations and crisis intervention is a must.</p><p><b>
LANGUAGE SKILLS</b></p><p>
Bilingual (English/Spanish) is preferred.</p><p><b>

<div><p>Under the supervision of the Support Services Program Manager, the <b>Education &amp; Employment Case Manager </b>will be responsible for supporting all aspects of 1:1 education programming throughout Urban Peak. The Education Case Manager will also be responsible for collaboration with Program Case Management to ensure ongoing education &amp; employment needs are met, the creation of education &amp; employment case plans for youth, as well as collaboration with the Education &amp; Employment Coordinator. This includes but is not limited to: GED subject test preparation and tutoring, high school re-enrollment support, additional tutoring and homework support, being a direct contact with education community partners who work in our building with youth (Ex: Field Academy, Zero Drop Outs), and providing classroom support during designated classroom times.</p><p>
This position will also develop individual employment plans with youth and collaborate with other agencies and Urban Peak departments to identify and meet career goals with Urban Peak youth. Working under the Refugee Support Services Employment Program he/she is responsible for fulfilling all contractual obligations of the state-funded grant. This role is contingent on grant funding.

<b>Position Responsibilities:</b><br>

<li>Conduct program orientation, intake, and assessment for clients; develop individual employment plans and provide case management to remove barriers to self-sufficiency.</li>
<li>Enroll clients into ESL classes & vocational trainings to increase employability.</li>
<li>Connect clients with job opportunities.</li>
<li>Prepares clients to enter the workforce within 90 days of arrival. Provide services to include orientation to the job market, skills assessment, career guidance, resume development, job application assistance, job referrals, interview preparation, job placement and follow up.</li>
<li>Educate clients about resources available in the community, including public benefits; make internal and external referrals to support client self-sufficiency and integration.</li>
<li>Educate clients about legal documents required for work, provide referrals for adjustment of status and naturalization.</li>
<li>In alignment with agency standards, document all services in case notes, and maintain required eligibility documentation; enter client data into agency databases in accordance with grant requirements.</li>
