URI also develops permanent affordable and supportive housing, advancing long-term pathways to safety, healing, and stability for families and communities.

Innovative Urban Village, opening in Summer 2026, is URI&rsquo;s first supportive housing program, developed in partnership with Christian Cultural Center. Located in East New York, Brooklyn, Phase 1A includes 376 income based rental residences, with 94 units dedicated to families who have experienced homelessness due to domestic violence and who qualify for ESSHI services. The program provides permanent supportive housing and trauma informed, culturally responsive services designed to promote long term housing stability and independent living. As URI&rsquo;s inaugural supportive housing initiative, Innovative Urban Village requires the development of a comprehensive program model, policies, procedures, and service framework from inception through full implementation.

POSITION OVERVIEW

The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence.

This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices. Case Managers collaborate closely with Housing Specialists, the Economic Empowerment Specialist, and the Director of Clinical Services to ensure integrated, high quality service delivery aligned with ESSHI and OTDA standards.

Salary: $60,000 - $70,000

MAJOR DUTIES AND RESPONSIBILITIES

Direct Service &amp; Case Management
<ul>
<li>Conduct comprehensive intakes, biopsychosocial assessments, and individualized service plans for assigned residents.</li>
<li>Provide supportive counseling, crisis intervention, and safety planning using trauma‑informed, survivor‑centered approaches.</li>
<li>Facilitate groups and workshops focused on parenting, wellness, DV recovery, tenancy skills, and life skills.</li>
<li>Conduct required home visits to assess safety, tenancy conditions, and service needs.</li>
<li>Provide advocacy and accompaniment for residents navigating benefits, legal systems, education, childcare, and healthcare.</li>
</ul>
Housing Stability &amp; Tenancy Support
<ul>
<li>Support residents in maintaining long‑term housing stability through proactive engagement and early identification of risk factors.</li>
<li>Educate residents on lease compliance, communication with property management, conflict resolution, and household management.</li>
<li>Collaborate with Housing Specialists and property management to address tenancy concerns and prevent eviction.</li>
<li>Assist residents with recertifications, inspections, and housing‑related documentation.</li>
</ul>
ESSHI, OTDA &amp; Supportive Housing Compliance
<ul>
<li>Ensure all services align with ESSHI service categories, including case management, employment support, benefits access, crisis intervention, and tenancy support.</li>
<li>Maintain accurate and timely documentation in CAPS, HMIS, and other required data systems.</li>
<li>Complete service plans, progress notes, assessments, and incident reports in accordance with agency and funder standards.</li>
<li>Participate in audits, case reviews, and quality assurance activities.</li>
</ul>
Crisis Intervention &amp; Risk Management
<ul>
<li>Respond to crises using de‑escalation, safety planning, and trauma‑informed strategies.</li>
<li>Conduct risk assessments related to DV lethality, mental health, child safety, and tenancy stability.</li>
<li>Coordinate emergency responses with internal leadership and external partners, including ACS, NYPD, mobile crisis teams, and hospitals.</li>
</ul>
Resource Navigation &amp; Community Linkages
<ul>
<li>Connect residents to community‑based services including mental health treatment, substance use programs, childcare, legal services, and medical care.</li>
<li>Provide benefits navigation for SNAP, Medicaid, SSI/SSD, childcare subsidies, and other entitlements.</li>
<li>Support residents in accessing education, vocational training, and employment programs in collaboration with the Economic Empowerment Specialist.</li>
</ul>
Interdisciplinary Collaboration
<ul>
<li>Participate in interdisciplinary case conferences and service coordination meetings.</li>
<li>Collaborate with the Director of Clinical Services to ensure clinical needs are addressed and referrals are completed.</li>
<li>Work closely with Housing Specialists to support move‑ins, housing retention, and tenancy success.</li>
<li>Maintain strong communication with operations and property management teams.</li>
</ul>
Training &amp; Professional Development

<li>Participate in ongoing training on trauma‑informed care, motivational interviewing, DV best practices, crisis intervention, and supportive housing standards.</li>

SPECIFICATIONS FOR EDUCATION/CERTIFICATIONS/LICENSES

<li>Bachelor&rsquo;s degree required.</li>
<li>Master&rsquo;s degree in Social Work or related field preferred.</li>

Care Lync is a Social Services agency focused on providing Support Coordination (Service Coordination/Case Management) services to adults diagnosed with Intellectual, Developmental and Physical Disabilities and Autism Spectrum Disorder throughout Pennsylvania who are enrolled in the Department of Human Services, Medicaid Waiver Program. General Summary
The Support Coordinator / Service Coordinator / Case Manager provides case management activities necessary to meet the needs of clients assigned to the consolidated care team. This position is responsible for the development of individualized care management plans; implementation of the care plan through brokering and coordinating services; and the monitoring and evaluation of all clinical outcomes to ensure that services are provided in a professional, comprehensive, and cost-effective manner. Essential Duties and Responsibilities
Complete assessments to assist in identifying each participant needs and desires for service delivery.
Develops a Person-Centered Service Plan for each participant and modify at least annually or when there is a significant medical or social change.
Ensure the participant choice of providers by providing information for the participant to make a fully informed decision.
Ensure the Behavioral Support Plan and Crisis Plan (if the participant has a Behavior Specialist) is consistent with the ISP.
Report all incidents of participant abuse, neglect or exploitation immediately to supervisor/director.
Actively coordinate with other individuals and/or entities essential in the physical and/or behavioral care delivery for the participant, including other care coordinators, to ensure seamless coordination between physical, behavioral and support services.
Responsible for researching and developing alternative solutions to participants needs.
Ensures confidentiality regarding sensitive material and private health information of each individual served.

POSITION SUMMARY 
The Workforce Development and Jericho Education Fund Case Manager supports participants in meeting their employment, career pathways and educational goals by providing case management, supportive services, and career counseling to participants considering or enrolled in Inspiration Kitchens foodservice training program or receiving education and training tuition support from the Lori &amp; Ron Jericho Memorial Education Fund.

ESSENTIAL DUTIES AND RESPONSIBILITIES:<ul><li>
Manage enrollment procedures for both the Jericho Education Fund and the Foodservice Training program.</li><li>
Screen applicant eligibility based on agency and grant requirements.
</li><li>Collaborate with the Workforce Development Manager to develop and execute recruitment</li><li>
Enroll, orient, and maintain an active, rolling caseload of program participants. Meet with enrolled participants at least once a month and refer to support services as necessary.</li><li>
Develop individualized service plans with participants, identifying goals, barriers and strategies to address barriers to meeting their goals; update individualized service plans to reflect updated goals, barriers and strategies.</li><li>
Support participants in meeting employment and job retention goals, collaborating with culinary and employment team members.</li><li>
Write case notes to document participant interactions, progress on their goals and outcomes. Accurate, complete and timely documentation and use of agency (currently Service Point) and funder data systems to demonstrate participant engagement, services and progress toward individual goals.</li><li>
Contribute to, prepare and submit accurate and timely data collection on program metrics reporting for internal and external stakeholders (e.g. development team, funders, etc.)</li><li>
Provide and document client support resources per eligibility, funder guidance and agency standards (e.g. transportation, clothing, etc.).</li><li>
Identify organizations to partner with to gain and send referrals.</li><li>
Identify resources to connect program participants to supportive services for which they are eligible (e.g. rental assistance, mental health, etc.).</li><li>
Develop and participate in programming that offers peer support and builds community among program participants (in-person, and/or virtual). Collaborate with team members to develop and update materials to support participants&rsquo; success, including program handbooks, graduation certificates and other program materials.</li><li>
Support program continuous improvement and coordination through participating in planning, team meetings, scheduling and site coordination.</li><li>
Support participants in entering and completing the food service training program and Jericho Education Fund and in meeting their education and/or training, employment placement and job retention goals, using trauma-informed, strengths-based, low-barrier, harm-reduction approaches.</li><li>
Recruit program prospects, prioritizing persons experiencing homelessness or housing instability, returning to the community from incarceration, poverty and other systemic barriers.</li></ul>

Support participants in demonstrating effective communication and other workplace skills.</li><li>
Assess participants&rsquo; levels of job readiness, provide career counseling, and assist in the development of short- and long-term goals.</li><li>

For Jericho Education Fund:<ul><li>
Support Jericho Scholars in meeting their educational and vocational training and employment goals so that they can secure sustainable employment and navigate a career pathway.</li><li>
Communicate with Inspiration Corporation staff, referring partner organization staff and prospective scholars, providing details and support for the referral process.</li><li>
Establish referral, application and other programmatic timelines.</li><li>
Meet enrollment goals.</li><li>
Maintain ongoing contact with participants during their course of study, track their progress, and help them address any challenges affecting their success in and completion of the program.</li><li>
Report participant status and outcomes monthly. Enter participant enrollment, progress, including completion of education or vocational training and obtaining and maintaining employment, and case notes in internal reporting systems.</li><li>
Complete financial transactions for scholar books, supplies, tuition, and other necessary items.</li><li>
Collaborate with the Workforce Development Manager in managing the selection process, fund budget and tracking expenses.</li><li>
Maintain contact with participants after completion of their education or training.</li><li>
Collect information about participant&rsquo;s initial employment outcomes and outcomes up to one year after starting employment.</li><li>
Collect feedback from scholars on how the Jericho Education Fund has been a support to them and produce success stories that will be presented to the donor.</li></ul>

EDUCATION, EXPERIENCE &amp; SKILLS: The following requirements are representative of the education, experience and skills necessary for the position. A Bachelor&rsquo;s degree in social work or related field preferred. Two years of work experience in nonprofit case management and/or significant life experience required. Candidates who have worked with persons experiencing homelessness or housing instability, citizens returning to the community from incarceration, or other vulnerable populations are preferred for this position. Experience with college access services or scholarship programs preferred. Proficiency in Windows XP and Microsoft Office required.

General Summary
The Support Coordinator / Service Coordinator / Case Manager provides case management activities necessary to meet the needs of clients assigned to the consolidated care team. The Support Coordinator / Service Coordinator / Case Manager will assist the person(s) receiving services in identifying and communicating his/her requests and needs for services and supports, which may include housing, medical, social, vocational and other community services.
Essential Duties and Responsibilities
Complete assessments to assist in identifying each participant needs and desires for service delivery.
Develops a Person-Centered Service Plan for each participant and modify at least annually or when there is a significant medical or social change.
Ensure the participant choice of providers by providing information for the participant to make a fully informed decision.
Ensure the Behavioral Support Plan and Crisis Plan (if the participant has a Behavior Specialist) is consistent with the ISP.
Report all incidents of participant abuse, neglect or exploitation immediately to supervisor/director.
Actively coordinate with other individuals and/or entities essential in the physical and/or behavioral care delivery for the participant, including other care coordinators, to ensure seamless coordination between physical, behavioral and support services.
Responsible for researching and developing alternative solutions to participants needs.
Ensures confidentiality regarding sensitive material and private health information of each individual served.
Attends required trainings and recertification classes.
Confers regularly with supervisor/ director(s) to understand and consider factors such as participants needs, abilities, learning levels, and physical limitations.
