The Case Manager provides person centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders within Ramsey County. Duties &amp; Responsibilities<ul><li>
Develop and implement the treatment plan that is based on the diagnostic and functional assessment which is approved by the clinical supervisor.</li><li>
Assume with other members of Team responsibility for assuring compliance with Minnesota Rule Chapter 9520. Provide or ensure access to needed and eligible medical, vocational, social, educational, financial, legal, and housing and chemical dependency services.</li><li>

The CCM Case Manager is responsible for professional work in coordinating and conducting a substance abuse community intervention program through the Anne Arundel County Health Department. 
 
The CCM Case Manager provides comprehensive case management services to clients enrolled in the program. Position Summary
The Virtual IOP Case Manager provides care management services to adults with substance use disorders, serious emotional disturbances, and severe and disabling mental illnesses. Under the supervision of the Program Supervisor, the Case Manager partners with consumers to assess needs, develop individualized treatment plans, coordinate care across service systems, and support recovery-focused goals. This role requires meeting caseload and billing expectations on a monthly, quarterly, and annual basis.
Essential Job Functions<ul><li>
Assess and evaluate adults to determine appropriateness and need for care management services</li><li>
Assist consumers with entry into services and monitor progress throughout the continuum of care</li><li>
Develop, coordinate, and monitor Individual Treatment Plans (ITPs) in alignment with agency policy and procedure</li><li>

Community Based Psychiatric Rehabilitation Services (CBPRS)

Skills training that includes clear description, modeling, practice, and performance feedback</li><li>

They are seeking to hire a Strategic Account Manager responsible for managing and building key relationships, developing sales strategies and managing 8 key accounts to penetrate specific markets within physician offices and hospitals. In this role, the Account Manager will expand the organization&rsquo;s reach, adoption and utilization of products/solutions.
 
 
 
 
 
 
 Responsibilities:

 Meet/exceed sales goal
 </li>
 <li>
 Develop strategies to penetrate specific accounts/markets expanding adoption and utilization of the company&rsquo;s products/solutions

 Collaborate with IDN teams to coordinate amendments, additions and/or requests to contracts
 </li>
 <li>
 Identify opportunities to advance partnerships with IDN teams and sales leaders
