This is an exciting opportunity to join a growing organization and play a key role in establishing our presence in a new region.</p><p>We are seeking clinicians who are passionate about improving access to care, working with diverse patient populations, and delivering thoughtful, individualized treatment.</p><p>If you enjoy flexibility, autonomy, and the opportunity to help shape healthcare in a growing organization, we would love to hear from you.</p><p>Key Responsibilities</p><ul><li>Perform comprehensive health assessments and physical examinations during <b>in-home visits and telehealth appointments</b>.</li><li>Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.</li><li>Develop and implement individualized treatment plans tailored to each patient&rsquo;s needs.</li><li>Educate patients and families on health management, wellness, and disease prevention.</li><li>Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care.</li><li>Perform minor clinical procedures such as wound care as appropriate during home visits.</li><li>Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms).</li><li>Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities.</li></ul><p>Qualifications</p><ul><li>Active, unrestricted Nurse Practitioner license (required).</li><li>Experience in geriatrics, internal medicine, or primary care preferred.</li><li>Previous home health or home-based care experience strongly preferred.</li><li>Experience caring for patients with disabilities is highly desirable.</li><li>Comfortable delivering care in both <b>in-home and telehealth settings</b>.</li><li>Excellent communication skills and a collaborative, team-oriented mindset.</li></ul><p>Medical Specialties</p><ul><li>Geriatrics</li><li>Internal Medicine</li><li>Primary Care</li></ul><p>Why Join ACE Physicians?</p><p>This is a unique opportunity to join ACE Physicians as we expand into Tennessee.
