Family Health Centers of San Diego is looking for a Case Manager II (Mobile Harm Reduction Team) to work under limited supervision, perform duties providing intensive, therapeutic case management services to assigned individuals. Participates in, and supports, the planning, development, implementation, and evaluation of services in accordance with contractual and departmental requirements and guidelines. Specific assignments vary depending on program, grant, and patient demographics, with a focus on providing community-based healthcare services to the medically underserved. The Case Manager II offers intensive case management services to clients and conducts therapeutic interventions.<h3 class="jobSectionHeader">
Responsibilities:</h3><ul><li>
Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required.</li><li>
Conducts initial and on-going assessment of client's health and/or support service needs. Sets level of client need.</li><li>
Creates formalized case plans and goals with clients; clients are high-need and require more intense services. Develops a written care-plan with the client, identifying problems and needs. Plan includes areas assessed, intended interventions, and expected results in measurable terms, with short and long term goals. Updates plan as client's needs change.</li><li>
Provides therapeutic interventions to clients. Provides intensive individual support to high need clients. Support may include providing therapeutic interventions, providing internal and community services referrals, and more intensive support may include accompanying clients to housing services, appointments, social services, etc. Focus on medical adherence, support, and appointments designed to maintaining necessary care.</li><li>
Performs other duties as assigned.</li></ul>
Requirements:<ul><li>
3 years of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.</li><li>
Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required. Master's degree in relevant field preferred.</li><li>
Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.</li><li>

Overview:

The Targeted Case Management Specialist plays a key role in connecting clients to essential medical and support services, focusing on individuals who speak English as a second language. QUALIFICATIONS

POPULATION SERVED: ACT Teams provide intensive, community-based services to individuals with severe mental illness. Individuals served by ACT Teams often have complex mental health needs, including acute psychosis, histories of complex trauma, and co-occurring substance use disorders. The primary function of ACT Teams is to help reduce unnecessary hospitalizations and incarcerations, by providing individuals the resources essential to thrive and maintain independence in their natural environment. 


POSITION SPECIFIC DUTIES &amp; RESPONSIBILITIES:
<ul>
<li>Maintain a strong working knowledge of the core ACT principles and ensure clinical practices reflect a high standard of model fidelity.</li>
<li>Attend and participate in daily organizational meetings and weekly treatment team meetings with multi-disciplinary team, utilizing a team approach in working with ACT clients.</li>
<li>Provide routine medication delivery and support to clients in community.</li>
<li>Promote healthy lifestyles, including general wellness education and information specific to chronic medical conditions.</li>
<li>Have good working knowledge of community resources for basic needs such as food, shelter, and clothing. Refer and link ACT clients as needed.</li>
<li>Provide clients with psychosocial rehabilitation services to help support successful functioning in the community, including the areas of ADL's, recreation, education, employment, and family functioning.</li>
<li>Assist interdisciplinary team with the completion of ACT initial comprehensive assessments and ongoing clinical assessments as indicated.</li>
<li>Provide assessment for high-risk clients and when appropriate follow agency-established protocols for ensuring client safety.</li>
<li>Be available to respond to crises in the community, utilizing agency protocols for crisis diversion. May include completion of Third-Party Affidavits and coordination with law enforcement to ensure client safety.</li>
<li>Be available to work non-traditional hours, which may include evenings, weekends, Holidays, and on-call overnight (varies by team).</li>
<li>Utilize agency issued cell phone during work hours for ACT work. Staff are expected to be responsive to cell calls and agency approved electronic communication, when scheduled to work. This may include overnight for on-call shifts (varies by team).</li>
<li>Have reliable transportation and be willing to utilize personal vehicle for community-based work, which may include transporting clients when appropriate.</li>
<li>Demonstrate comfort and flexibility while working with clients in difficult life circumstances, which may include individuals, who are homeless or living in substandard conditions.</li>
<li>Coordinate with community partners to ensure continuity of care and advocate for client needs. Example of community partners include: housing providers, inpatient hospital staff, shelter staff, and law enforcement.</li>
<li>Must have a strong commitment to the rights and ability of each person with a severe and persistent mental illness. Skills to establish supportive, trusting relationships and respect for client preferences in treatment are essential.</li></ul> 


QUALIFICATIONS: Case Managers for ACT programs must have a Bachelor's degree in Social Work, Psychology or a related social services major, which qualifies for Case Management II certification with the Oklahoma Department of Mental Health. Candidates must have CM-II certification or obtain CM-II certification within 60 days of employment. Applicants with a previous background working in community mental health, inpatient psychiatric facilities, criminal justice, home health, or similar settings are preferred. Strong candidates with a passion for helping individuals with severe mental illness will also be considered. Must have a valid driver&rsquo;s license and satisfactory driving record; use of personal automobile will required for travel to locations outside of primary office.

This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.

Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to:

<ul><li>Expanded traditional community mental health and substance use services.</li></ul>
<ul><li>Added Care Coordination and physical health screening for mental health clients</li></ul>
<ul><li>Greater access to Social Services for clients&rsquo; economic and social needs</li></ul>
<ul><li>Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.</li></ul></div>

This job is classified as a &ldquo;safety-sensitive&rdquo; position as defined by the Oklahoma Medical Marijuana and Patient Protection Act.
