JOB DUTIES
<br>Conducts job duties in accordance with departmental policies and procedures.
<br>Develops rapport with patients and their families using approach congruent with that of health care team for purpose of obtaining maximum participation in activities.
<br>Develops activities plan reflecting cultural and religious interests of resident population and appeal to both men and women of all age groups. Posts activity plan for each resident in designated place within patient's room. Provides in room activities for patients confined to or choosing to remain in their room. Plans activities that reflects interests and ability levels of patient.
<br>Determines and documents degree to which interventions achieve or enhance patient outcome.
<br>Accepts additional responsibility. Assists willingly in coverage of other patients. Facilitates or offers assistance in transport of patients to group activities.
<br>Advises manager of equipment needs so that appropriate items may be ordered in timely manner.
<br>Develops discharge recommendations reflecting patients' interests and community resources available.
<br>Demonstrates knowledge of contra-indications and/or safety issues with various patient types and applies them to treatment.
<br>Instructs volunteers or staff in activities program plan. Solicits their help in working with patients on in-room activities.
<br>Organizes patient treatment time efficiently. Organizes unscheduled time efficiently. Assists in on-job training, education, and supervision of Physical Therapy technicians, students and/or nursing.
<br>Uses sound judgment in stressful and emergency situations involving patient, family, or staff. Represents hospital in professional manner at community function.

</p><ul><li>Assesses each patient/resident for interests, talents, and previous patterns of time use, and develops activity approaches based on that assessment.</li><li>
Establishes and maintains necessary records regarding activities' attendance and activity care plan approaches.</li><li>
Develops activity goals and objectives for each patient/resident and integrates the activity approaches within the Patient/Resident Care Plan.</li><li>
Identifies facility and community resources that can contribute to the recreation program. Plans and schedules activities and develops a monthly calendar of activities.</li><li>
Secures entertainment, program leaders, volunteers, materials, equipment, and transportation well in advance of scheduled dates.</li><li>
Assures all patients/residents are involved in activities: group, individual (bedside) and/or independent on a weekly basis.</li><li>
Ensures 6 activities are held daily and at least 2 evening activities are held per week.</li><li>
Recruits and trains a volunteer activities staff and maintains volunteer programs that utilize community involvement.</li><li>
Evaluates effectiveness of programs in terms of patient/resident outcomes.</li><li>
Develops and maintains a budget for activities programs.</li><li>
Coordinate Facility newsletter or other promotions of activities programs through local media.</li><li>
Serves on various committees as requested by Administrator.</li><li>

POSITION SUMMARY:</b></p><p>
The Memory Care Director position champions Charter Senior Living&rsquo;s Flourish Memory Care philosophy and lives the core values and mission of Charter Senior Living; Enhancing the Human Spirit. This position is responsible for the day-to-day operations and leadership of the Memory Care Neighborhood and is responsible for training, development, and ongoing compliance of the program standards for the Flourish Memory Care Programs, community policies and procedures and applicable state regulations as they apply to memory care. This position also works collaboratively with the management team to ensure the planning and implementation of all therapeutic programming, entertainment, service planning and achievement of occupancy development goals. The Memory Care Director is the process owner to implement and oversee the points of distinction established in the areas of Flourish Restore, Flourish Enrich, Flourish Connect, Flourish Create, Flourish Engage, and Flourish Contribute. This role must possess compassion, and a genuine concern for the well-being and interest of seniors, along with experience in dementia care and life enrichment programming.</p><p><b>
Knowledge, Skills, Abilities, and Experience:</b></p><p><b>
Area of study</b>: Recreation Therapy, Social Work, Human Development, Psychology, or related field.<br>
<b>Years of Experience</b>: Mid-Level. Minimum of three years-experience working with Seniors with at least one year being with persons with Dementia.
<br>
<b>Type of Experience</b>: Three to five years of experience working with individuals with Dementia including leading programs, handling complex behavioral issues, and educating staff on families on Alzheimer&rsquo;s or Dementia related topics.
<br>
<b>Special Certifications</b>: Activities Director Certification, Certified Therapeutic Recreation Specialist, Certified Dementia Practitioner, or other related Certifications.<br>
<b>Level of Formal Education</b>: Associates degree or equivalent experience required. Bachelor&rsquo;s degree or equivalent experience preferred.
<br>
<b>Skills and Abilities:</b> Demonstrate leadership capabilities and interpersonal abilities. Change leadership skills, problem solving and expertise in planning and organizing.
<br>
<b>Personal Attributes</b>: Results oriented, detail oriented, proactive, focused, adaptable, compassionate, balanced, flexible, and professional.
