Overview</b><br>
Oversees the day to day clinical care provided by Physical, Occupational, and Speech services in their facility. Specific management tasks include: orientation and training of new employees; coordinating patient assignments, staff performance evaluations, interdisciplinary interaction, program planning, therapy hours productivity, and staffing issues. The Rehab Services Manager ensures that the highest standard of rehabilitation services are delivered and maintained.

<b>What you will do</b><br>
<ul><li>
Demonstrates intermediate to advanced technical knowledge/clinical competencies in nursing as evidenced by the ability to assume a role in the provision of patient care as needed. Assures that unit policies and procedures are current and consistent with regulatory guidelines. Monitors for adherence to policies and procedures by use of audits, observations, and timely feedback. Sustains an environment of progressive and evidence-based nursing practice.</li>
<li>Participates positively in hiring, oversight, training and support of all team members, volunteers and students to include positive acknowledgment and disciplinary action as appropriate. Completes accurate staff evaluations in a timely manner. Promotes satisfactory staff retention and is competent in labor relations matters. Seeks guidance from Director and HR in sensitive or complicated matters.</li>
<li>Continuously improves the delivery and outcomes of comprehensive, quality, personalized patient/customer care economically and in accordance with patient safety standards, standards of excellence and the Riverside Code of Conduct. Monitors clinical activities to identify both expected and unexpected risks.</li>
<li>Builds credibility with physicians as a champion for patient care, quality, and professional practice. Collaborates with medical staff to implement evidence based practice and process improvement (protocols, policies, CPOE, etc.) Demonstrates the ability to address physician issues/concerns and follow through within 72 hours.</li>
<li>Demonstrates understanding of both internal and external diverse patient care populations served. Recognizes and embraces age diversity in the patients/customers as well as the workforce. Collaborates with System Director of Rehab and Therapy as well as other Rehab leaders across the Riverside Health System.</li>
<li>The Manager is effective at educating and coaching their team to achieve satisfactory performance in documentation and billing practices. Documentation of clinical care performed on the unit/clinic meets all applicable requirements and supports the necessity of the services provided. Billing practices of the clinic/unit meet compliance standards and appropriate policies to timeliness and accuracy.</li>
<li>Participates/oversees the planning and monitoring of the budget for defined areas to include appropriate resource allocation determination. Completes variance review, analysis, and action plans based on approved departmental expense and productivity budget. Understands and shares with staff updates in healthcare economics and public policy as it applies to the delivery of patient care.</li></ul><br>

<li>Instructs and supervises COTAs, HHAs, LPNs and RNs in therapy program when appropriate.</li>

<li>Provides families with information, support and encouragement to help motivate clients in their progress. Instructs client's family on home therapy program.</li>
<li>Assists individuals and families to accept and adjust positively to physical, mental and social limitations.</li>
<li>Participates in the development of community resources to meet the needs of clients.</li>
<li>Maintains positive and effective communication with all employees and others. Understands and participates in team concept.</li>
<li>Maintains absolute confidentiality of all information pertaining to clients, families and employees.</li>
<li>Maintains a safe client environment and identifies and reports to the office any suspected vulnerable client abuse, neglect, or financial exploitation.</li>

PHYSICAL/ENVIRONMENTAL DEMANDS
<br><ul>
<li>Heavy to Very Heavy. Involves lifting clients, bending, stooping, stretching, and setting up equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.</li>
<li>Exerting in excess of 100 pounds of force occasionally, in excess of 50 pounds of force frequently, and in excess of 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for heavy work.</li>

QUALIFICATIONS
<br><ul>
<li>Currently licensed as an Occupational Therapist in the state of Minnesota. The license must not have been revoked, suspended, and without limitations or restrictions.</li>
<li>Graduate of AOTA approved school of occupational therapy and currently certified by the AOTA to practice occupational therapy.</li>
<li>Have at least two (2) years experience in a health care setting or equivalent experience. Home care experience preferred.</li>
<li>Certification with National Board of Occupational Therapy strongly recommended.</li>
<li>Strong interpersonal communication and teaching skills.</li>
<li>Strong organizational skills and the ability to work independently.</li>
<li>Excellent written and oral communication skills.</li>
<li>Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day.</li>
<li>A valid driver’s license and proof of car insurance when using a personal vehicle for company business.</li>
<li>Proof of negative mantoux or documentation of negative chest x-ray.</li>
<li>Current CPR certification recommended.</li>
<li>Have U.S. Citizenship or evidence of valid Alien Work Permit.</li>
<li>Discloses any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of such crimes will not be hired.</li>

The Physical Therapy (PT) Case Manager provides and oversees home health services for clients within the community. Collaborates with the client, their family, and the health care team to develop and implement goal-centered care plans taking into consideration quality measures and financial aspects of treatment. Provides skilled care to clients and clinical oversight to professional and unlicensed personnel. Builds and maintains respectful relationships with others. The position reports to the Clinical Supervisor.

SCHEDULE

<li>This is a full time position, Monday - Friday. As PT Case Manager, you will manage your schedule during office hours.</li>
<li>This position will require you to travel to client's homes to provide 1:1 client care that may take 1-3 hours per client.</li>

<li>This position reports to the St. Cloud, MN office and provides services in Cold Spring, St. Joseph, Paynesville, St. Augusta and surrounding cities.</li>

DUTIES AND RESPONSIBILITIES
<br><ul>
<li>Reflects the company’s mission and values in developing and maintaining professional working relationship with others.</li>
<li>Performs initial and periodic assessment of clients to determine care needs, implements interventions, and evaluates the effectiveness of care to achieve positive outcomes.</li>
<li>Collaborates with physicians/non-physician practitioners and the home health team to develop and update the client’s home health plan of care at least every 60 days or more often as changes occur.</li>
<li>Serves as a client advocate ensuring safety needs are addressed by assessing the home environment, developing and implementing individualized abuse prevention plans, and reporting any suspected vulnerability such as abuse, neglect or financial exploitation.</li>
<li>Meets documentation standards ensuring accuracy, timeliness, and compliance with state and payer/billing requirements.</li>
<li>Provides education to the client, family, caregivers, licensed clinicians (as indicated), and unlicensed personnel.</li>
<li>Provides direct care to clients of varying ages, clinical acuity levels, and diagnoses in accordance with the company’s policies, procedures, processes and the Minnesota Physical Therapy Practice Act.</li>
<li>Coordinates client care with family, caregivers, community resources, and other health care</li>
</ul>
members.

<li>Provides clinical oversight, delegation, and leadership to licensed clinicians and unlicensed personnel.</li>
<li>Ensures effective and appropriate financial management of the client’s home health services are met.</li>

<li>Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments while establishing priorities and ensuring the necessary duties are completed.</li>
<li>Ability to build and maintain professional relationships.</li>
<li>Excellent written and oral communication skills; interpersonal communication and teaching skills.</li>
<li>Basic computer skills and willing to learn new software programs; experience with Electronic Medical Records and Microsoft Outlook preferred.</li>
<li>Current CPR certification highly recommended.</li>
