Case Manager, Transitional Housing
<br><br>
Reporting to the Clinical Coordinator, the Case Manager, Transitional Housing will provide case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing.
<br><br>
This position has a 35-hour work week.
<br><br>
<b>ESSENTIAL DUTIES:</b><br>
<ul>
<li>Provide direct services to a caseload of approximately 15 clients, including conducting necessary assessments, benefits assistance, and other services necessary to support the client in achieving housing and recovery goals</li>
<li>Conduct psychosocial evaluations</li>
<li>Connect clients to psychiatric services</li>
<li>Connect clients to medical treatment and substance use treatment facilities when applicable</li>
<li>Obtain identification, public benefits, and other applicable income sources for clients</li>
<li>Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans</li>
<li>Prepare all documentation for housing applications</li>
<li>Assist and counsel clients in the process of obtaining permanent housing</li>
<li>Maintain record keeping in compliance with agency standards as well as those of City and State agencies</li>
<li>Intervene in crisis situations</li>
<li>Escort clients to appointments when needed</li>
<li>Upon training, provide assistance with self-administration of medication consistent with Breaking Ground's policies and procedures</li>

<b>MINIMUM QUALIFICATIONS:</b><br>

<li>Bachelor’s Degree or equivalent work experience</li>
<li>Two years of experience working with homeless adults and substance abuse issues preferred</li>
<li>Experience with harm reduction and motivational interviewing techniques strongly preferred</li>
<li>Valid NY, NJ, CT, or PA driver’s license with a good driving record, as defined by Breaking Ground is preferred</li>

The Housing Case Manager provides trauma-informed, youth-centered support to young people transitioning into or maintaining housing. Housing Case Managers will work to form supportive relationships and walk alongside clients as they develop the skills to maintain housing and live independently. Housing Case Managers may assist in any of The Place&rsquo;s housing programs including Permanent Supportive Housing, Rapid Rehousing or Transitional Living.</p><p>
Housing Case Managers collaborate with landlords, community partners, and other staff to ensure youth are supported in safe, stable living environments.</p><p>

<b>Position:</b> Case Manager
<br><br>
The Case Manager is a key provider of services within Volunteers of America’s Case Management Model. The Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client and monitor progress. S/he must assure that clients receive appropriate, competent and professional information and advice, referrals and advocacy services to achieve successful outcomes. May include supervision of direct care staff.

Jamaica Women’s Employment Shelter is a transitional shelter for women who are experiencing homelessness and are employed or deemed employable, and; referred by the New York City Department of Homeless Services (DHS). <b>Location:</b> Queens Village, NY 11429
<br><br>
<b>Minimum Qualifications:</b><br>
<ul>
<li>Associates Degree or 60 college credits courses in Human Services field or High School Diploma or GED with 2 year of relevant shelter experience.</li>
<li>The position also requires a demonstrated ability in verbal and written communication, including basic computer skills.</li>
</ul>
<b>Case Manager Principal Responsibilities:</b><br>
1. Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
<br>2. Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
<br>3. Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
<br>4. Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.
<br>5. Oversee achievement of targets related to employment, housing and all other program goals.
