</p><p><b>PROGRAM OVERVIEW</b></p><p>
The Individuals Rapid Rehousing Case Manager is responsible for supporting program participants in developing housing stability plans and addressing barriers to housing. To this end, the Individuals Rapid Rehousing Case Manager participates in our collective effort to end and prevent homelessness.</p><p>

</p><p><b>REPORTING RELATIONSHIP: </b>The position reports to the on-site Program Manager and is a vital member of the participant&rsquo;s care team.</p><p>
</p><p><b>PRIMARY RESPONSIBILITIES:</b></p><ul><li>
Maintain a caseload of 20-30 participants.</li><li>
Provide comprehensive case management services. This may include:<ul><li>
Initial eligibility verification and enrollment</li><li>
Ongoing needs and status assessments</li><li>
Budgeting</li><li>
Administration of financial assistance</li><li>
Information and referral</li><li>
Crisis de-escalation</li><li>
Safety planning, risk assessment, and mandated reporting</li><li>
Employment and benefits assistance</li><li>
Advocacy</li><li>
Other direct service provision</li></ul></li><li>
Meet with participants at a frequency and location determined by their needs.</li><li>
Engage in the consistent application of evidence-based practices.</li><li>
Support internal program support specialist activities as needed. This may include:<ul><li>
Street-based or community outreach efforts</li><li>
Landlord engagement</li><li>
Workshop coordination</li></ul></li><li>
Participate in case conferencing and collaborative care coordination.</li><li>
Maintain compliance with contract, agency, and licensing requirements.</li><li>
Achieve identified program and position deliverables.</li><li>

Maintain thorough and accurate documentation of participant services in accordance with agency and contractual standards.</li><li>
Attend all job-related meetings, including program staff meetings and agency-wide meetings.</li><li>
Participate in opportunities for learning and skill maintenance/development.</li><li>
Utilize supervision appropriately, maintaining open lines of communication and providing updates on activity.</li><li>
