Administer outcome measurement tools (e.g., GAD-7, PHQ-9, and other evidence-based assessments). Monitor client progress and communicate updates to the clinical team. Maintain accurate and timely documentation, including progress notes and case management records. Collaborate with licensed clinicians to support treatment goals and service delivery. Support implementation of Individual Service Plans (ISPs).
