<ul><li>Provides mentorship and advocacy services to youth ages 16 and 24 with a consistent caseload of 75 youth.</li><li>
Assists with recruiting and retaining 75 youth.
</li><li>Works with each youth to develop an Individualized Service Plan to address areas for improvement such as housing, academics, social emotional learning, counseling referrals, life skills, etc.</li><li>
Works with Job Developer to support employment opportunities, site visits, and retention efforts.</li><li>
Ensures files remain up-to-date and in compliance.</li><li>
Completes employability skills checklist, conduct all Illinois WorkNet modules, &amp; ensure pre/post assessment.</li><li>
Assist in-school youth to improve educational outcomes.</li><li>
Provide information and referrals to internal and external resources</li><li>
Re-engage out-of-school youth in educational activity.</li><li>
Collects, reviews, and analyzes all transcripts for youth to determine GED or High School Diploma track.</li><li>
Utilizes Labor Market Data and information to help guide youth in career and education goals.</li><li>
Works to assist youth to determine what factors put them at risk of engaging in or becoming victims of aggressive behavior and other anxieties.</li><li>
Helps to determine what methods each youth should use in order to avoid risk-taking factors and reach their goals.</li><li>
Works in collaboration within the targeted communities to cultivate relationships with local community resources.</li><li>
Reaches out and re-engage youth that have inconsistent attendance, work with families to bring resources to the home, and connect with programs.</li><li>
Conducts Home visits.</li><li>
Support outreach events, referral systems, and programs facilitated by other departments within agency.</li><li>
Transporting students in vehicle to and from program related activities may be required.</li><li>
Implements other services requested by SGA or funder.</li><li>
Maintain detailed record of activities to regularly track impact of outreach efforts and to facilitate appropriate reports to funders and government agencies.</li><li>
Enters daily attendance, case notes into ILWorknet.</li><li>
Coordinates with the Job Developer to provide ongoing program support and implementation.</li><li>
Conducts required reports and data material to SGA such as the Employability/Life Assessment, TABE Test etc.</li><li>
Ability and experience in short and long-range planning and strategy Detail oriented with the ability to multi-task and self-direct your work</li><li>

Requirements:

</li></ul>The Transporter Specialist for La Jornada (The Journey) Program focusing on transitional foster care (LTFC), will be responsible for coordinating transportation and physically escorting children who have fled their country of origin and arrived in the U.S. without a parent or legal guardian as they continue their journey to be united with sponsors.
RESPONSIBILITIES
The Transportation Specialist will perform duties including but not limited to:
<ul><li>Must be able to travel out of state up to 40%-50% for reunification efforts.
</li><li>Performs all work in a manner consistent with the National Youth Advocate Program&rsquo;s Mission, Values and Philosophies .
</li><li>Coordinates transportation including organizing and navigating travel destinations, physically escorts, and supervises youth while in transit to scheduled and emergency appointments. Coordinates medical transport with Charge Nurse. Coordinates clinical transport with clinical staff. Coordinates reunification transport with team.
</li><li>Tracks and assigns all doctor appointments via supervisors - attends weekly supervisor meeting.
</li><li>Maintains a daily roster of times, dates, destinations, transporters and length of transport.
</li><li>Provides documentation of all missed appointments or unusual events to the Management Team.
</li><li>Tracks and schedules all group outings weekly via transportation outing schedule.
</li><li>Maintains cell phone numbers of staff on transport.
</li><li>Ensures daily cleanliness and upkeep of all vehicles assigned to the campus &ndash; includes inspecting for cleanliness, damage, proper equipment and safe operation of vehicle.
</li><li>Ensures vehicles are properly fueled.
</li><li>Maintains security over the vehicles and keys.
</li><li>Reports vehicle needs and damage to maintenance and tracks all vehicle issues in regard to repair.
</li><li>Maintains notebooks &ndash; includes inspecting logs for completion and accuracy.
</li><li>Provides safety training to all new hires every three weeks.
</li><li>Collects and tracks all safety checklist sheets and provides weekly report.
</li><li>The supervision and transport of youth requires prolonged periods of walking and standing and physical agility. May be required to run and climb flights of stairs.

</li></ul>The Case Manager I for La Jornada is responsible for the completion and submission of timely assessments and service plans, ensuring document uploads into ORR&rsquo;s UC Portal and maintaining required comprehensive case files compliant with ORR Policy and NYAP&rsquo;s CQI Team. Case Manager I is required to maintain a flexible, organized and efficient work schedule and is subject to work extended hours, weekends, and be on-call.
 RESPONSIBILITIES
The Case Manager I will perform duties including, but not limited to:
<ul><li>Perform all work in a manner consistent with the National Youth Advocate Program&rsquo;s Mission, Values and Philosophies.
</li><li>Ensure case management assessments are completed within ORR&rsquo;s allotted timeframes.
</li><li>Conduct on-site admission and initial intake interviews of youth to include gathering familial, possible sponsorship information and to establish age of the youth.
</li><li>Conduct interviews of sponsors/family members, friends of family to vet sponsor&rsquo;s ability to care for the minor(s).
</li><li>Act as UC case POC for assigned Federal Field Specialist, Case Coordinator and Contract Field Specialist.
</li><li>Prepare case for, conduct and lead pre staffing process with multidisciplinary team of professionals including case managers, clinical counselors, medical coordinators and other designated stakeholders.
</li><li>Attend staffing(s) with Case Coordinators, Federal Field Specialists and other ORR stakeholders as needed.
</li><li>Coordinate with local pro bono attorneys for the timely provision of &ldquo;Know Your Rights&rdquo; presentations and legal screenings to children in care.
</li><li>Work with program administration, clinical, medical and educational staff in identifying best case management practices while maintaining a collaborative multi-disciplinary environment.
</li><li>Work to ensure children in care are provided a safe environment and safe and timely release from ORR care pursuant to ORR MAP Section 2; this may include completing online address searches, obtaining birth certificates to prove relationship, income verifications, background checks, and other actions to ensure proper vetting of the Sponsor, household members and adult caregivers.
</li><li>Document all actions taken and contacts with youth, sponsor, and stakeholders in the form of progress notes as required by NYAP.
</li><li>Complete and submit reunification packets and Release Requests for initial review to Lead Case Manager or Program designee.
</li><li>Submit completed reunification packet with appropriate referral made by Case Manager for the timely release of youth to designated sponsor, including referrals for home studies and post release services (PRS).
</li><li>Provide weekly face to face updates to youth and telephonic updates to family members/sponsor.
</li><li>Ensure the provision of two weekly telephonic contacts with family in the US or COO, primary caregiver and/or sponsor.
</li><li>Facilitate incoming calls to minors with the appropriate family members and other approved caregivers.
</li><li>Facilitate attorney to client contact as requested by youth.
</li><li>Coordinate weekly treatment team meetings with representatives from all departments at the Program.
</li><li>Establish and maintain a strong relationship with assigned foster parents, attend meetings with foster parents and act as program liaison with foster caregivers.
</li><li>Drive children to facilitate program services, may include transportation to court appointments, attorney visits, other appointments as needed per contractual duties; this may also include transporting youth within the United States for reunification purposes.
</li><li>Coordinate case management and family reunification services for children, including active involvement in discharge planning.
</li><li>Actively participate in documenting safety plans and Post 18 age out plans.
</li><li>Oversee and/ or actively participate in the process of reporting significant incidents (SIRs) in accordance with existing policies and procedures.
</li><li>Ensure maintenance of UC electronic and physical files, including uploading documents in UC Portal, Evolve, and maintaining the corresponding physical file.
</li><li>Perform other duties as assigned.
</li></ul>MINIMUM QUALIFICATIONS
<ul><li>Bachelor&rsquo;s degree required in Social Work, Psychology, Human Services, Counseling or other social service field. One (1) year experience preferred working with children and adolescents or in the youth services field. (volunteer and internship experienced included).
</li><li>Excellent case management, verbal and written communication skills. Critical assessment and analysis skills. Motivated, organized, flexible and able to navigate multiple service priorities. Ability to work under stress and multitasking.

REPORTS TO: Unit Director
HOURS OF WORK: Part Time, 20 hours per week

The Boys &amp; Girls Club of Worcester is a nonprofit, charitable, educational organization recognized as a leading advocate for youth development, working to help youth of all backgrounds.

We provide girls and boys with effective diverse programs and services; and demonstrate that caring adults can truly help young people grow into contributing human beings with the necessary skills for living.
Reporting to the Unit Director, the Administrative Support Staff will assist with admissions paperwork, auditing of member files, attendance, billing and coordinating events. Occasionally assisting in program coverage for children ages 5 -13. Additional responsibilities include complying with EEC regulations.
Position is on-site, 20 hours per week, 1:30pm-5:30pm during the school year and 10am-2pm during Worcester Public Schools closures.

JOB SUMMARY:
This position works within the Intensive Aftercare Program (IAP) and In-Home Safety Services (IHSS) program to provide services to families open to child welfare where children are at-risk for removal. Our programming also works with youth who have been reunified with their family after being placed in corrections, residential treatment, or foster care treatment. As the Safety Services Community Worker, you will work closely with youth and families to promote safety, parenting support, service acquisition, and child/family wellness.

ESSENTIAL FUNCTIONS OF THE SAFETY SERVICES COMMUNITY WORKER:<ul><li>
Interview and evaluate clients to complete the intake process.</li><li>
Evaluate youth and families for treatment needs and refer to appropriate resources.</li><li>
Analyze client needs and strengths to determine services required control safety threats.</li><li>
Create, implement, and review safety assessments and plans.</li><li>
Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.</li><li>
Provide parenting education, support, and advocacy to program clientele through individual, family, and group work.</li><li>
Conduct face-to-face contact with families to ensure safety and cooperation with safety plan.</li><li>
Act as a resource to evaluate and determine necessary responses in de-escalating crisis situations with youth and families.</li><li>
Safely transport clients as needed.</li></ul>
LOCATION: Kenosha, WI. Local travel is required to meet with clients at the home or other community spaces, with mileage reimbursement provided.
SCHEDULE: Full-time 40 hours per week. Monday-Friday 8:00am-5:00pm with one evening shift per week (1:00-9:00pm) and 1-2 Saturdays per month.
