Memphis Allies, launched by Youth Villages, is a bold, collaborative initiative designed to reduce gun violence and build safer, stronger neighborhoods across Memphis. The program brings together community groups, organizations, and local resources to expand opportunities for adults, youth, and their families while addressing the root causes of violence.</p><p></p><p>
At the heart of this effort is the Support with Intent To Create Hope (SWITCH) model&mdash;an innovative approach that combines street outreach and life coaching with intensive clinical services and case management. Through SWITCH and SWITCH Youth, Memphis Allies staff identify, connect with, and support adults (ages 19 to 30) and youth (ages 12 to 19) at the highest risk of involvement in gun violence, who may have been recently arrested, on probation, or are re-entering the community after incarceration. By meeting individuals where they are and surrounding them with consistent support, Memphis Allies is working to build stability and safety, change lives, reduce gun violence, and strengthen communities.</p><p></p><p><b>
Essential Duties and Responsibilities:</b></p><p><br>
The SWITCH Case Manager:</p><ul><li>
Carries a caseload of approximately 24 youth and young adults</li><li>
Conducts weekly in-person sessions as needed</li><li>
Provides therapeutic services in the home as well as other community-based locations</li><li>
Connects the youth, young adult, family, and caregivers with resources as needed</li><li>
Works to collaborate with other partners and services the youth, young adult, family, and caregivers are involved in</li><li>
Provides accurate and detailed information to the Supervisor in a timely manner in the event of a crisis</li><li>
Adheres to guidelines set as a mandated reporter under the guidance of the Clinical Supervisor, Regional Supervisor, and Licensed Program Expert</li><li>
Adheres to Youth Villages' confidentiality, ethical, and legal guidelines</li><li>
Develops and implements a Professional Development Plan to increase the skills necessary for professional growth</li><li>

SWITCH Case Manager - Memphis Allies

<div><p><b>PRIMARY FUNCTION:</b></p><p>
Responsible for managing certificate program, daily attendance, and activity tracking for all members. Plans, implements, supervises, and evaluates activities provided within multiple program areas following all licensing requirements. Provides guidance for parents and visitors upon entering the building.</p><p><b>
PRIMARY RESPONSIBILITIES:</b></p><ul><li>
Greet visitors and offer pertinent information on programs, activities, and procedures for joining the club.</li><li>
Implements programs and activities in areas such as tutoring, technology, games, health and recreation, arts and crafts, etc. for youth of all ages.</li><li>
Print and maintain attendance logs, bus rosters, and sign-in/out sheets as needed.
