As Overnight Support Specialist for Intercept Health's Youth Quest, you will serve as a positive role model for residents to observe and learn socially effective values, attitudes and behaviors. They contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties.
RESPONSIBILITIES:<ul><li>
Use good interpersonal communication skills to gain resident cooperation and resolve resident conflicts. Interact with all residents while maintaining appropriate relationships which do not allow self-disclosure of personal or social information to residents.
</li><li>Provide transportation, accompany, and directly supervise residents on outings, activities and appointments in the community.</li><li>
Guide residents in personal hygiene techniques and the proper care of their condominiums. Assist residents in meal preparation steps. Guide and instruct residents in proper housekeeping and laundry procedures; review completion of weekly chores.</li><li>
Exercise fair, firm and consistent use of behavioral system.</li><li>
Supervise the administration of medications as appropriate.</li><li>
Check each condominium to ensure residents are in by curfew, working, or on an approved pass. Document all incidences in communication log and contact the Life Skills Coach or Program Supervisor. Provide individualized, therapeutic-based support to youth and young adults (YYAs) experiencing homelessness. Support YYAs in quickly securing safe and stable housing while connecting them to resources that promote long-term stability and overall well-being. The Care Coordinator builds trusting relationships with YYAs and collaborates with service providers to connect clients to housing, employment, healthcare, and personal development opportunities. Duties and Responsibilities
Case Management and Client Support<ul><li>
Ensure all care coordination and housing-focused services are rooted in Housing First for Youth principles, prioritizing rapid placement in stable housing without preconditions while offering flexible, youth-centered support that promotes independence and resilience for long-term success.</li><li>
Provide individualized case management services, life skills education, and linkage to educational, employment, financial, medical, housing, and other social resources tailored to the YYA&rsquo;s treatment plan goals.</li><li>
Foster life skill development, social skills enhancement, and housing stability through personalized support, empowering YYAs to overcome challenges and achieve their goals.</li><li>
Develop and oversee the implementation of individualized treatment plans with YYAs and their service teams, identifying strategies or treatment options to ensure progress toward YYA goals.</li><li>
Engage, mentor, and meet with YYAs regularly as determined by program standards, promoting trusting relationships while delivering care coordination services that align with Housing First for Youth principles, including YYA choice and trauma-informed approaches.</li><li>
Conduct home visits as needed, ensuring at least one visit per month to the YYA&rsquo;s apartment to assess progress and identify areas for growth as they work towards independence.</li><li>

Program Overview:
Youth Villages&rsquo; Treatment Foster Care program provides evidence-based, intensive in-home treatment for youth in foster care while supporting foster parents and advocating for the best interests of each youth. The program serves children and adolescents (ages 0 to18) who have experienced trauma, abuse, or neglect, and may have behavioral and mental health needs.

Intercept&reg;, developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children. 
 
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family&rsquo;s home and community&ndash;Intercept&reg; is not an office-based program. 
 
Intercept provides intensive, strength-based support tailored to each family&rsquo;s unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.</div><div>
