About us: Since 2006, Collaborative Geriatrics Inc., a medical group of 40 providers, has been delivering healthcare to elderly patients in the greater Houston area in various settings including post-acute, long-term care and assisted living facilities. Your Role:

• Conduct physical exams, manage both acute and chronic conditions.
• Manage medication regimens and polypharmacy.
• Develop comprehensive care plans including collaboration with primary care physicians, home health or hospice services.
• Offer personalized patient and family education and support, with on-call availability for nursing facility staff.

Who We’re Looking For:

• APRN licensed in Texas, with effective communication skills and experience in geriatric, adult, acute, or family medicine.
• Professionals eager to contribute to a team while capable of independent decision-making.
• Candidates passionate about improving elderly patient care, demonstrating flexibility and initiative.
