Your Role & Responsibilities:

• Conduct comprehensive assessments and develop individualized plans of care for a diverse patient population.
• Provide skilled physical therapy services to patients in their homes to improve mobility, reduce pain, and prevent disability.
• Educate patients and their families on treatment plans and home exercise programs.
• Collaborate with a multidisciplinary team of nurses, aides, and other healthcare professionals.
• Maintain accurate and timely clinical documentation using our state-of-the-art EMR system.

We're Looking For Someone With:

For Physical Therapists (PT):

• Current CA State license in good standing as a Physical Therapist.
• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Strong clinical skills and a passion for patient-centered care.

For Physical Therapy Assistants (PTA):

• Current CA State license/certification in good standing as a Physical Therapy Assistant.
• Associate's degree from an accredited PTA program.
• A team-player mindset and excellent communication skills.

Both Positions Require:

• Reliable transportation, a valid driver's license, and auto insurance.
• Compassion, empathy, and a commitment to the highest ethical standards.
