Your Role & Responsibilities:
- Conduct comprehensive assessments and develop individualized plans of care for a diverse patient population.
- Provide skilled physical therapy services to patients in their homes to improve mobility, reduce pain, and prevent disability.
- Educate patients and their families on treatment plans and home exercise programs.
- Collaborate with a multidisciplinary team of nurses, aides, and other healthcare professionals.
- Maintain accurate and timely clinical documentation using our state-of-the-art EMR system.
We're Looking For Someone With:
For Physical Therapists (PT):
- Current CA State license in good standing as a Physical Therapist.
- Doctorate or Master's degree in Physical Therapy from an accredited program.
- Strong clinical skills and a passion for patient-centered care.
For Physical Therapy Assistants (PTA):
- Current CA State license/certification in good standing as a Physical Therapy Assistant.
- Associate's degree from an accredited PTA program.
- A team-player mindset and excellent communication skills.
Both Positions Require:
- Reliable transportation, a valid driver's license, and auto insurance.
- Compassion, empathy, and a commitment to the highest ethical standards.
