Job Title: Service Coordinator

Department: Service Coordination

Reports To: Service Coordination Program Manager

Job Summary:

Provide targeted case management services to improve the quality of life for individuals with developmental and intellectual disabilities who are served through the development of person-centered planning to identify current and future needs which will ensure independence in the least restrictive environment; link individuals and families to needed resources and services; advocate on behalf of individuals and families as requested; monitor services received to ensure the desired outcomes are achieved; adherence to local, state, and federal rules and regulations.

Duties and Responsibilities

  • Proactively identify, utilize, and maintain positive relationships with providers of community resources and supports relevant to the needs and goals of individuals served.
  • Engage all parties in development of person-centered plan and selection of appropriate supports, services, and providers. The service coordinator is responsible for the documentation of plans and updates.
  • Establish clear and courteous lines of communication to develop beneficial partnerships with individuals, families, service providers, public officials, and associates.
  • Monitor effectiveness of services received; review services to ensure contractual compliance; timely and thorough completion of necessary documentation, reports, and correspondence.
  • Promote involvement by individuals, families, and service providers to ensure each individual served has continuous opportunities within the following quality outcome areas: Daily Life, Community Living, Social Spirituality, Healthy Living, Safety and Security, Citizenship and Advocacy, and Supports for Families.
  • Understand the position of advocacy, protection of rights, and service as a mandated reporter to ensure the health and safety of individuals served, while maintaining appropriate boundaries; empathize with characteristics, needs, ideas, experiences, socio-economic backgrounds, and cultural beliefs of individuals and families that may differ from themselves.
  • Compliance with HIPAA (Health Insurance Portability and Accountability Act of 1996) confidentiality regulations.
  • Demonstrate a working knowledge of contractual and legal requirements of TCM services.
  • Continue to engage in professional development and training opportunities.

Required Skills and Qualifications

  • Bachelor’s degree from an accredited college or university
  • Bachelor’s degree in psychology, sociology, or a related field preferred
  • Experience working with individuals with developmental or intellectual disabilities preferred
  • Strong interpersonal and communication skills
  • Ability to work effectively with individuals, families, and multidisciplinary teams
  • Ability to maintain confidentiality and demonstrate professionalism

Physical Requirements

  • Ability to sit, stand, and move for extended periods
  • Must be able to lift up to 50 pounds when necessary
  • Must be clean of all illegal substances and marijuana

Pay: From $21.75 per hour
