Position Title: CYS Shelter Case Manager
Department: Family Management Center Shelter
Reports To: Supervisor/Program Manager
Employment Status: Full-Time
FLSA Status: Exempt
Work Schedule: 8:00 a.m. – 4:30 p.m. or 8:30 a.m. – 5:00 p.m.

Position Summary

This position is responsible for managing a caseload of families involved with Children and Youth Services (CYS) who are residing in the Family Management Center Shelter. Participating families may face barriers that require intensive case management, supportive services, and coordinated care to secure and maintain housing, achieve financial stability, and preserve family stability.

The CYS Shelter Case Manager works collaboratively with CYS caseworkers, administrators, and other service providers to support clients in meeting established CYS goals, shelter requirements, stability plans, and self-sufficiency goals.

Primary Duties and Responsibilities

  • Manage a caseload of up to eleven homeless and economically disadvantaged individuals and families requiring case management and supportive services within the shelter facility.
  • Conduct client intakes and develop and update stability plans upon entrance into the shelter in collaboration with CYS, established CYS goals, and client self-sufficiency plans.
  • Coordinate ongoing collaboration with CYS caseworkers, administrators, and other appropriate service providers.
  • Conduct formal inspections of client units and provide housekeeping education, guidance, and support as needed.
  • Develop, update, and monitor individual service plans and self-sufficiency goals for each client.
  • Maintain, at minimum, weekly contact with clients to support compliance with goal plans, shelter rules, and program expectations.
  • Document ongoing case management work with each client, including progress notes, service plan development, updates, referrals, client progress, and efforts to identify and alleviate barriers to client self-sufficiency.
  • Develop and maintain a network of social service resources; refer clients to appropriate community resources; coordinate ongoing community linkages; and provide advocacy and support.
  • Assist with instruction in basic life skills, including home maintenance, comparative shopping, budgeting, laundry, health and hygiene, nutrition, parenting, self-esteem, communication, and other topics as needed or requested.
  • Monitor and support client progress throughout shelter participation and conduct six-month post-exit follow-up.
  • Attend required CYS-mandated meetings and trainings.
  • Complete reports as required by funding sources, monitoring agencies, the COO, CEO, and other designated leadership.
  • Participate in required staff meetings, case conferences, and client review meetings.
  • Perform other related duties as assigned.

Qualifications

This position requires a minimum of a Bachelor’s Degree in Human Services, Social Work, Psychology, or a related field from an accredited college or university. A Master’s Degree is preferred.

A combination of relevant education and experience may be considered, provided the candidate demonstrates the knowledge, skills, and abilities required for the position.

The successful candidate must demonstrate strong organizational skills and the ability to manage multiple priorities, deadlines, and client needs. This position requires excellent case management skills and the ability to work effectively with individuals and families from diverse cultural, racial, ethnic, and socio-economic backgrounds.

The individual must also possess excellent oral and written communication skills, strong interpersonal skills, sound judgment, professionalism, and the ability to maintain appropriate boundaries with clients.

Required Clearances and Licenses

This position requires completion and maintenance of all required clearances, including but not limited to applicable child abuse, criminal background, FBI, and mandated reporter requirements.

A valid driver’s license is required. The employee must maintain a valid driver’s license throughout employment and must be able to travel as needed for work-related duties, meetings, trainings, and client/program needs.

Physical Demands

The physical demands described in this section are representative of those that must be met by an employee to successfully perform the essential functions of this position. While performing the duties described herein, the employee is regularly required to talk, hear, stand, walk, maneuver stairs, and sit.

The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this position include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

Pay: $49,000.00 - $58,000.00 per year
