SNF Nurse Case Manager

Overview:
- Coordinate and manage care plans for long-term care residents
- Serve as a liaison between residents, families, and healthcare providers
- Ensure compliance with all regulatory and company standards
- Monitor resident outcomes and support care teams in achieving goals
- Provide guidance, education, and advocacy for residents and families
Requirements:
- Active RN or LPN license in the state of Ohio in good standing
- Minimum 3 years experience in long-term care, skilled nursing, or case management required
- Strong communication, organizational, and problem-solving skills
- Comfort working independently in a remote environment
- Compassionate, resident-centered mindset
