SAJE Strategic Solutions is seeking a compassionate, proactive, and leadership-driven Social Service Director to oversee the psycho-social care and discharge planning department for a premier skilled nursing facility in Pasadena, CA.
The Role
In this full-time leadership position, you will head the social services department to support the physical, psychological, and social well-being of our residents. The Director plays a vital role in enhancing the quality of life of our residents by developing meaningful social service interventions, leading comprehensive discharge planning, and collaborating with a multidisciplinary clinical team to advocate for resident rights and dignity.
Key Responsibilities
- Clinical Assessments: Oversee and perform comprehensive psycho-social assessments upon admission, quarterly, annually, and as significant changes occur.
- Discharge Planning & Care Coordination: Lead robust discharge planning initiatives from the point of admission, including developing, updating, and executing customized discharge care plans.
- Interdisciplinary Collaboration: Coordinate closely with the internal interdisciplinary team (IDT), outside case managers, physicians, and family members to ensure integrated resident care.
- Care Conferences: Schedule, conduct, and document formal care conferences and routine IDT meetings.
- Psychosocial & Behavioral Support: Formulate interventions addressing the nuanced psychosocial needs of residents, including behavioral support tracking, grief counseling references, and family education.
- Continuum of Care Coordination: Assist families and residents with hospice decisions, clergy referrals, and end-of-life care coordination.
- Financial & Social Program Aid: Facilitate applications and navigation paths for Medi-Cal, SSI, and alternative social service programs.
- Advocacy & Compliance: Organize family council meetings at least biannually. Maintain mandatory theft and loss files, ensure functional communication boards are available for residents with impairments, and provide staff training/in-services regarding resident rights and psycho-social issues.
- Quality Assurance: Active participate and contribute data to Quality Assurance (QA) programs, behavior management committees, and direct psychotropic medication review meetings.
Required Qualifications
- Education: Bachelor’s Degree is required (Degree in Social Work, Psychology, or a related human services field preferred).
- Certifications: Social Service Designee Certificate or relevant regulatory social work credentials required.
- Experience: Minimum of 1 year of experience in discharge planning and care coordination specifically within a skilled nursing facility (SNF) or long-term healthcare setting is highly preferred.
- Skills: Exceptional interpersonal and written/verbal communication skills. Proven ability to lead healthcare teams, manage sensitive family dynamics, and navigate complex clinical crises with empathy and professionalism.

- Pay Rate: Competitive hourly rate ranging from $32.00 – $40.00 per hour (based on experience and depth of SNF background).
