Stabilization & Community Integration Specialist

Jericho Community Services is seeking a compassionate, highly organized, and motivated Social Service Specialist III to provide direct support services to individuals with developmental disabilities and other complex support needs throughout Yakima County and surrounding areas.

This position primarily serves clients receiving Developmental Disabilities Administration (DDA) Stabilization services, Community Choice Guide (CCG) services, FCS Supported Housing/Employment program participants, and other individuals experiencing significant barriers to maintaining safe, successful participation in their homes and communities. The ideal candidate is comfortable working independently, managing complex situations, coordinating with multiple service providers, and supporting individuals through periods of transition, crisis prevention, and stabilization.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide direct community-based support to individuals with developmental disabilities and complex support needs.
  • Assist individuals in maintaining stability within their homes, employment settings, and communities.
  • Support individuals experiencing housing instability, community placement challenges, behavioral concerns, or other barriers to successful community living.
  • Coordinate services with DDA/DSHS Case Resource Managers, guardians, family members, healthcare providers, behavioral health professionals, landlords, employers, and community partners.
  • Assist clients with accessing community resources, benefits, housing opportunities, transportation, and other needed supports.
  • Provide skill-building and coaching related to independent living, community participation, self-advocacy, problem-solving, communication, and daily living activities.
  • Implement individualized support strategies and behavior support plans when applicable.
  • Assist with crisis prevention, de-escalation, and stabilization efforts while maintaining client dignity and safety.
  • Support individuals during transitions into new homes, community settings, employment opportunities, or other significant life changes.
  • Maintain accurate, timely, and compliant documentation in accordance with DDA, Medicaid, agency, and funding source requirements.
  • Participate in team meetings, trainings, case consultations, and quality assurance activities.
  • Transport clients when authorized and appropriate.

QUALIFICATIONS

  • Bachelor's degree or higher in Social Services, Human Services, Psychology, Education, Behavioral Sciences, or related field required.
  • Minimum 2+ years of experience providing direct support, case management, behavioral support, social services, or related services to individuals with developmental disabilities, behavioral health needs, or complex support needs preferred.
  • Experience working with individuals who have co-occurring developmental disabilities and mental health conditions preferred.
  • Experience working with DDA, Medicaid-funded programs, housing services, crisis intervention, or community-based supports strongly preferred.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to work independently with minimal supervision.
  • Ability to maintain professional boundaries and confidentiality.
  • Ability to effectively manage multiple priorities and complex situations.
  • Proficient with computers, electronic documentation systems, and basic office software.

REQUIREMENTS

  • Valid driver's license and acceptable driving record.
  • Reliable transportation and proof of insurance.
  • Ability to pass state and federal background checks.
  • Ability to complete all required DSHS, DDA, and agency trainings.
  • Ability to work flexible hours based on client needs.

WORK ENVIRONMENT

This position is primarily community-based and involves working in clients' homes, community settings, businesses, healthcare facilities, and other service locations. Staff are responsible for managing their own schedules while ensuring client needs and service requirements are met.
