Job Summary
Join our dedicated team as a Social Services professional, where you will play a vital role in supporting individuals and families through comprehensive case management, counseling, and community outreach. Responsibilities

  • Conduct thorough patient assessments to identify needs related to mental health, addiction, developmental disabilities, or aging-related conditions such as dementia or Alzheimer's care
  • Provide individual and group counseling sessions utilizing evidence-based practices like cognitive behavioral therapy (CBT), behavioral therapy, motivational interviewing, and applied behavior analysis (ABA)
  • Develop personalized care plans and coordinate services including crisis intervention, case management, and community resource linkage
  • Maintain accurate medical documentation in compliance with HIPAA regulations and ensure proper management of medical records using Electronic Medical Record (EMR) systems
  • Collaborate with healthcare providers, social workers, educators, and families to facilitate comprehensive treatment plans and support systems
  • Perform diagnostic evaluations utilizing ICD-9/10 coding systems for accurate classification of mental health or developmental disorders
  • Engage in clinical research activities related to behavioral health or social services initiatives to improve program effectiveness and client outcomes

Experience

  • Proven experience working with children, adults, seniors, or individuals with disabilities in settings such as outpatient clinics, inpatient units, nursing homes, or assisted living facilities
  • Familiarity with hospital procedures, hospice care protocols, post-acute care environments, or home & community care programs
  • Hands-on experience with crisis management, intake procedures, mental health counseling, and behavioral health interventions
  • Knowledge of medical terminology, CPT coding for billing purposes, ICD coding systems for diagnoses, and HIPAA compliance standards
  • Demonstrated ability to perform patient care activities including assessments and case management across diverse populations such as geriatrics, pediatrics, or individuals with autism or PTSD
  • Experience working within EMR systems and managing medical documentation accurately in fast-paced clinical settings
